    Abstract Background: Previous studies have focused on the involvement of small nucleolar RNAs (snoRNAs) and SNHGs in tumor cell proliferation, apoptosis, invasion, and metastasis via multiple pathways, including phosphatidylinositol-3-kinase/protein kinase B (PI3K/AKT), Wnt/β catenin, and mitogen-activated protein kinase (MAPK). These molecular mechanisms affect the integrity of the intestinal mucosal barrier. Aim of review: Current evidence regarding snoRNAs and SNHGs in the context of the mucosal barrier and modulation of homeostasis is fragmented. In this review, we collate the established information on snoRNAs and SNHGs as well as discuss the major pathways affecting the mucosal barrier. Key scientific concepts of review: Intestinal mucosal immunity, microflora, and the physical barrier are altered in non-neoplastic diseases such as inflammatory bowel diseases. Dysregulated snoRNAs and SNHGs may impact the intestinal mucosal barrier to promote the pathogenesis and progression of multiple diseases. SnoRNAs or SNHGs has been shown to be associated with poor disease behaviors, indicating that they may be exploited as prognostic biomarkers. Additionally, clarifying the complicated interactions between snoRNAs or SNHGs and the mucosal barrier may provide novel insights for the therapeutic treatment targeting strengthen the intestinal mucosal barrier.

    Abstract The gut vascular barrier (GVB) is the deepest layer of the gut barrier. It mainly comprised gut vascular endothelial cells, enteric glial cells, and pericytes. The GVB facilitates nutrient absorption and blocks bacterial translocation through its size-restricted permeability. Accumulating evidence suggests that dysfunction of this barrier correlates with several clinical pathologies including Crohn's disease (CD). Significant progress has been made to elucidate the mechanism of GVB dysfunction and to confirm the participation of disrupted GVB in the course of CD. However, further analyses are required to pinpoint the specific roles of GVB in CD pathogenesis. Many preclinical models and clinical trials have demonstrated that various agents are effective in protecting the GVB integrity and thus providing a potential CD treatment strategy. Through this review, we established a systemic understanding of the role of GVB in CD pathogenesis and provided novel insights for GVB-targeting strategies in CD treatment.

    Abstract Summary: The mission of the IBD Quality Care Evaluation Center (IBDQCC) is to establish indicators of quality of care (QoC), certify IBD units to generate a network of IBD quality care, and eventually improve the national level of IBD healthcare. The final list of 28 core and 13 secondary IBD QoC indicators suitable for the healthcare system in China were selected using a Delphi consensus methodology. Units that met all core indicators were qualified as “regional”; units that met all core indicators together with more than 50% of the secondary indicators received a rating of “excellence.” Using the selected QoC core indicators for certifying IBD units, a network of IBD quality care units covering the majority of IBD patients in China was established.     Abstract Abstract Federated learning(FL) is a new kind of Artificial Intelligence(AI) aimed at data privacy preservation that builds on decentralizing the training data for the deep learning model. This new technique of data security and privacy sheds light on many critical domains with highly sensitive data, including medical image analysis. Developing a strong, scalable, and precise deep learning model has proven to count on a variety of high-quality data from different centers. However, data holders may not willing to share their data considering the restriction of privacy. In this paper, we approach this challenge with a federated learning paradigm. Specifically, we present a case study on the whole slide image classification problem. At each local client center, a multiple-instance learning classifier is developed to conduct whole slide image classification. We introduce a privacy-preserving federated learning framework based on hyper-network to update the global model. Hyper-network is deployed at the global center that produces the weights of the local network conditioned on its input. In this way, hyper-networks can simultaneously learn a family of the local client networks. Instead of communicating raw data with the local client, only model parameters injected with noise are transferred between the local client and the global model. By using a large scale of whole slide images with only slide-level labels, we mensurated our way on two different whole slide image classification problems. The results demonstrate that our proposed federated learning model based on hyper-network can effectively leverage multi-center data to develop a more accurate model which can be used to classify a whole slide image. Its improvements in terms of over the isolated local centers and the commonly used federated averaging baseline are significant. Code will be available.

    Abstract Intrinsic capacity is central to the maintenance of function in older adults, and maintaining optimal intrinsic capacity is of great importance to promote healthy aging. The purpose of this systematic review and meta-analysis was to analyze the impact of multi-domain interventions on intrinsic capacity in older adults, intervention components, and potential interactions between components. A total of 6740 published articles were screened until August 2022, and the review included 25 randomized controlled trials that analyzed populations, interventions, control groups, and outcomes. The meta-analysis showed improvements in the primary outcome indicators in the intervention group compared to the control group. These included increased scores on the Mini-Mental State Examination as an indicator of cognitive function, decreased scores on the Geriatric Depression Scale (GDS-15) as an indicator of psychological ability and increased scores on the Short Physical Performance Battery (SPPB) as an indicator of physical performance, with only the SPPB indicator analyzed showing greater heterogeneity. Significant improvements were also seen in the secondary indicators Time-to-Walk Test (TUG), gait speed, Chair Stand Test (CST), grip strength values and BMI. There was insufficient data for the Mini Nutritional Assessment (MNA) as an indicator of vitality to conduct a meta-analysis. Studies were of moderate to high quality. The results of this review indicate that multi-domain interventions can maintain the level of intrinsic capacity in older adults and are equally effective in older adults with declining self-care abilities.

    Abstract Background. Chronic subdural hematoma is a common disease in neurosurgery, but organized chronic subdural hematoma is rarely seen clinically. This article reports a case of misdiagnosis of organized chronic subdural hematoma as acute epidural hematoma. Through literature review, the causes of misdiagnosis and the treatment methods of organized chronic subdural hematoma are discussed. Case Description. A 70-year-old male patient was admitted to the hospital due to headache and dizziness after head trauma. Emergency head CT reported “left frontotemporal parietal epidural hematoma.” Because the head CT showed that the hematoma occupying effect was obvious, an emergency “intracranial hematoma evacuation” was performed. After opening the skull during the operation, no epidural hematoma was seen. Upon incision of the dura mater, the outer membrane of organized chronic subdural hematoma was found. When the outer membrane was cut open, a large amount of reddish-brown silt-like materials was found in the capsule cavity. The inner membrane was not forcibly removed. Postoperative head CT showed that the organized chronic subdural hematoma was basically cleared. Conclusion. The early symptoms of organized chronic subdural hematoma are atypical, with insidious onset and easy misdiagnosis. By carefully inquiring about the medical history and carefully reading the head CT, such misdiagnosis can be avoided. Craniotomy is currently an important treatment option for organized chronic subdural hematoma.

    Abstract Objectives: Since a reliable model for predicting infliximab (IFX) benefits in bio-naïve Crohn's disease (CD) is still lacking, we constructed a magnetic resonance enterography (MRE)-based model to predict the risk of loss of response to IFX in bio-naïve patients with CD.
    Methods: This retrospective multicenter study enrolled 188 bio-naïve patients with CD who underwent MRE before IFX therapy. Therapeutic outcomes were determined based on clinical symptoms and endoscopic findings within 52 weeks. The areas of bowel wall segmentation were decided by two experienced radiologists in consensus. Texture features were extracted using the least absolute shrinkage and selection operator, and a radiomic model was built using multivariate logistic regression. The model performance was validated by receiver operating characteristic, calibration curve, and decision curve analysis.
    Results: The area under the curve of radiomic model was 0.88 (95% confidence interval: 0.82-0.95), and the model provided clinical net benefit in identifying the loss of response to IFX and exhibited remarkable robustness among centers, scanners, and disease characteristics. The high-risk patients defined by the radiomic model were more likely to develop IFX nonresponse than low-risk patients (all p < 0.05).
    Conclusions: This novel pretreatment MRE-based model could act as an effective tool for the early estimation of loss of response to IFX in bio-naïve patients with CD.

    Abstract The English composition is an important indicator of English learners’ overall language skills and is asked in large-scale English examinations, both in China’s college entrance examinations and graduate examinations and in the TOEFL, GRE, and IELTS examinations in Europe and the United States. Some automatic scoring systems for English writing have been created in the United States and internationally, however the systems still have issues with generalization, accuracy, and error correction. In this paper, we present a method to improve the accuracy of existing automatic composition scoring systems through deep learning techniques in a wireless network environment. Experiments reveal that the method can accurately assess the quality of English learners’ writings, paving the way for the creation of an automated composition scoring system for large-scale machine testing and web-based self-learning platforms.

    Abstract Not only have high-speed rail (HSR) services stimulated the economy of many countries, but they have also significantly uplifted quality of lives (QoL) of countless people. For many decades, the aspiration for HSR network development has dramatically risen, and HSR networks have inevitably become an icon of civilisation. However, only a few successful HSR networks globally can truly generate socio-economic impacts on their societies. This research aims to understand the impact of HSR networks on social and economic impacts and to provide recommendations for success. This study is the world’s first to examine the benefits of HSR across all community demographic groups, including young and elderly people. The findings will illustrate the QoL, economic, and educational elements’ advantages in explicit terms. It has established two interconnected models via Python to codify a novel customised model for socio-economic evaluation. ‘Pearson correlation coefficient’ and ‘K-Nearest Neighbour’ techniques are applied to bolster the reliability of the research findings. The outcomes have been reviewed by 30 international HSR specialists. The benchmarking exhibits that socio-economic impacts apparently occur across vast areas. The insight stemming from this benchmarking also offers policy implications and empirical data for long-term HSR improvement, assisting the government in developing new methods for sustainable communities.

    Abstract Accurate evaluation of the risk mitigation status of navigating ships is essential for guaranteeing navigational safety. This research mainly focuses on the feasibility and accuracy of evaluating the real effectiveness of a risk mitigation system for navigating ships, including addressing the problem of immeasurableness for risk mitigation capability and determining the degradation regulation of risk mitigation capability over time. The proposed method to solve the problem is an effectiveness evaluation model based on the capability perspective, composed of a capability measurement algorithm based on entropy theory and capability degradation regulation analysis based on numerical process fitting. First, combined with the theoretical framework of a comprehensive defence system, the risk mitigation system designed for navigating ships is reconstructed based on capability building. Second, using a numerical fitting method, the degradation regulation of risk mitigation capability with time is obtained to improve the accuracy of the dynamic evaluation. Finally, referring to entropy theory, the uncertainty of capability is calculated, and then the model is constructed based on this uncertainty to realize the effectiveness evaluation from a capability perspective. The results obtained in an application test of the proposed model indicate that using the entropy of capability can realize an accurate effectiveness evaluation of a risk mitigation system for navigating ships, with a 9% improvement in accuracy, and the Weibull curve fitting is more consistent with capability degradation regulation, with a signification level of 2.5%. The proposed model provides a new path for evaluating the effectiveness of a risk mitigation system for navigating ships from the entropy of capability, and compared with the traditional probabilistic method, the model is more realistic and accurate in actual applications.
