    Abstract Pressures from the healthcare systems have led to decreased lengths of hospital stay and home-based long-term rehabilitation for the elderly hospitalized patients, while the elderly patients with home-based rehabilitation remain at high risk of hospital re-admission, adverse events and mortality. As the modern technology advances, previous research has proposed the concept of smart hospital and smart home for the elderly's health. However, no evidence of smart homes tailored to home-based rehabilitation for the elderly patients discharged from hospital is available. Moreover, how to link smart hospitals and smart homes to generate a joint environment for elderly patients' accelerated recovery, is a worthwhile endeavor. To address this important gap between smart hospitals and smart homes for the elderly who require home-based recovery after discharge, Guangdong Second Provincial General Hospital recently initiated the development of a novel model, named smart home wards (SHWs). The SHWs may have the potential to extend homes as part of the hospital wards by breaking down the information silos between smart hospitals and smart homes, and eventually improve the elderly patients' health outcomes in a cost-effective approach for our aging population.

    Abstract The COVID-19 pandemic has accelerated a long-term trend of smart hospital development. However, there is no consistent conceptualization of what a smart hospital entails. Few hospitals have genuinely reached being “smart,” primarily failing to bring systems together and consider implications from all perspectives. Hospital Intelligent Twins, a new technology integration powered by IoT, AI, cloud computing, and 5G application to create all-scenario intelligence for health care and hospital management. This communication presented a smart hospital for all-scenario intelligence by creating the hospital Intelligent Twins. Intelligent Twins is widely involved in medical activities. However, solving the medical ethics, protecting patient privacy, and reducing security risks involved are significant challenges for all-scenario intelligence applications. This exploration of creating hospital Intelligent Twins that can be a worthwhile endeavor to assess how to inform evidence-based decision-making better and enhance patient satisfaction and outcomes.

    Abstract Abstract Background China has empowered and continues to empower internet hospitals, which saw an increase in their development due to the pandemic, to fight against COVID-19. The construction and operational models of internet hospitals can be categorized as self-constructed and self-managed models, self-constructed and enterprise-run models, hospital and enterprise joint-owned models, and hosted by a third-party platform. Despite the growing importance of internet hospitals, there have been few systematic summaries of their construction and operational models. The primary purpose of the study was to understand the construction and operational models of internet hospitals in China. Methods Data was collected from 39 internet hospitals and 356 medical staff between September 2020 and April 2021, via internet hospital and hospital staff surveys. T-tests were used to compare the continuous variables, while Chi-square tests were employed to compare the proportions of categorical variables. The self-perception of the internet hospitals’ services was assessed using a 5-point Likert scale on 16 aspects and a root cause analysis was conducted to identify the root causes and influencing factors of current deficiencies experienced by internet hospitals. Results Among the 39 internet hospitals, 22 (56.4%) were self-constructed and self-managed. Compared to other models of Internet hospitals, self-constructed and self-managed hospitals had lower percentages of professionals providing online services (P = 0.006), numbers of doctors outside of the entity (P = 0.006), numbers of online nurses (P = 0.004), and the ratio of online nurses to offline doctors (P < 0.001). Of the 16 aspects evaluated with regards to the medical staff’s self-perception of the internet hospital services, the highest scores were given for fee transparency, fee rationality, travel cost capital, patience and responsibility, and consultation behaviors.     Abstract The ongoing Coronavirus Disease 2019 (COVID-19) broke out in China since December 2019, and rapidly spread worldwide. To contain the disease, unessential businesses had been shut down in several countries to a varying extent. Nowadays, the enterprises are resuming productions and businesses. While the resumption of production is crucial to social development, it elevates the risk of cluster-infections at the workplaces. Guangdong Second Provincial General Hospital therefore set up the Smart Safeguard System for COVID-19, aiming to provide rapid screening and consistent protection to assist the local enterprises with resumption. The system has received positive feedback as being helpful and practical. It has the potential to be widely used to prevent the cluster-infection of COVID-19 at workplaces during the pandemic.

    Abstract The mechanism underlying the association between the development of head and neck squamous cell carcinoma (HNSCC) and ferroptosis is unclear. We analyzed the transcriptomes of 5902 single cells from a single-cell RNA-sequencing (scRNA-seq) dataset. They then aggregate into B cells, epithelial cells, fibroblasts, germ cells, mesenchymal cells, cancer stem cells, stem cells, T cells and endometrial cells, respectively. Our study shows that multiple pathways are significantly enriched in HNSCC development including extracellular matrix structural components, humoral immune responses, and muscle contraction. Differentially expressed genes analysis in Pseudotime analysis, pathway and biological function indicated that there was a significant correlation in the ferroptosis pathway. Furthermore, higher ferroptosis potential index (FPI) scores were significantly associated with worse overall survival prognosis in HNSCC patients. Pseudo-temporal, survival analyses and immunohistochemistry identified multiple central genes in HNSCC development, including ACSL1, SLC39A14, TFRC, and PRNP genes, and indicated associated ferroptosis. Overall, our study detected ferroptosis-related features is closely correlated with HNSCC prognosis and development, and deserved candidates suitable for immunotherapy treatment strategies determination for HNSCC patients.

    Abstract Objectives: Insomnia disorder has been reclassified into short-term/acute and chronic subtypes based on recent etiological advances. However, understanding the similarities and differences in the neural mechanisms underlying the two subtypes and accurately predicting the sleep quality remain challenging. Methods: Using 29 short-term/acute insomnia participants and 44 chronic insomnia participants, we used whole-brain regional functional connectivity strength to predict unseen individuals’ Pittsburgh sleep quality index (PSQI), applying the multivariate relevance vector regression method. Evaluated using both leave-one-out and 10-fold cross-validation, the pattern of whole-brain regional functional connectivity strength significantly predicted an unseen individual’s PSQI in both datasets. Results: There were both similarities and differences in the regions that contributed the most to PSQI prediction between the two groups. Further functional connectivity analysis suggested that between-network connectivity was re-organized between short-term/acute insomnia and chronic insomnia. Conclusions: The present study may have clinical value by informing the prediction of sleep quality and providing novel insights into the neural basis underlying the heterogeneity of insomnia.

    Abstract Abstract The aim of this study was to analyze initial chest computed tomography (CT) findings in COVID-19 pneumonia and identify features associated with poor prognosis. Patients with RT-PCR-confirmed COVID-19 infection were assigned to recovery group if they made a full recovery and to death group if they died within 2 months of hospitalization. Chest CT examinations for ground-glass opacity, crazy-paving pattern, consolidation, and fibrosis were scored by two reviewers. The total CT score comprised the sum of lung involvement (5 lobes, scores 1–5 for each lobe, range; 0, none; 25, maximum). 40 patients who recovered from COVID-19 and six patients who died were enrolled. The initial chest CTs showed 27 (58.7%) patients had ground-glass opacity, 19 (41.3%) had ground glass and consolidation, and 35 (76.1%) patients had crazy-paving pattern. None of the patients who died had fibrosis in contrast to six (15%) patients who recovered from COVID-19. Most patients had subpleural lesions (89.0%) as well as bilateral (87.0%) and lower (93.0%) lung lobe involvement. Diffuse lesions were present in four (67%) patients who succumbed to coronavirus but only one (2.5%) patient who recovered (p < 0.001). In the death group of patients, the total CT score was higher than that of the recovery group (p = 0.005). Patients in the death group had lower lymphocyte count and higher C-reactive protein than those in the recovery group (p = 0.011 and p = 0.041, respectively). A high CT score and diffuse distribution of lung lesions in COVID-19 are indicative of disease severity and short-term mortality.

    Abstract Abstract Background Evidence on the relationship between different particle size fractions and emergency ambulance dispatches (EAD) remains limited and sparse. Methods We collected daily data of EAD, ambient air pollution and meteorological data from 2014 to 2018 in Guangzhou, China. We used a generalized additive model with covariate adjustments to estimate the associations between different particle size fractions and EAD related to all-cause, cardiovascular diseases, and respiratory diseases. Several subgroup and sensitivity analyses were also performed. Results Significant associations were observed between PM2.5, PM2.5–10, PM10 and EADs. A 10 μg/m3 increase of PM2.5, PM2.5–10, and PM10 was associated with an increase of 0.98% (95% CI: 0.67, 1.28%), 2.06% (95% CI: 1.44, 2.68%), and 0.75% (95%CI: 0.53, 0.96%) in all-cause EAD, with an increase of 0.69% (95% CI: 0.00, 1.39%), 2.04% (95% CI: 0.64, 3.45%), and 0.60% (95%CI: 0.11,1.10%) in cardiovascular-related EAD, and an increase of 1.14% (95% CI: 0.25, 2.04%), 2.52% (95% CI: 0.72, 4.35%), and 0.89% (95%CI: 0.25,1.52%) in respiratory-related EAD at lag03, respectively. The results were robust in subgroup and sensitivity analyses. Conclusions This study revealed that PM2.5, PM2.5–10 and PM10 were significantly related with risks of all-cause and cause-specific EAD. More evidence of high quality may be needed to further support our results in this ecological study.

    Abstract Objectives Hyperuricaemia has been reported to be significantly associated with risk of obesity. However, previous studies on the association between serum uric acid (SUA) and body mass index (BMI) yielded conflicting results. The present study examined the relationship between SUA and obesity among Chinese adults.Methods Data were collected at Guangdong Second Provincial General Hospital in Guangzhou City, China, between January 2010 and December 2018. Participants with ≥2 medical check-up times were included in our analyses. Physical examinations and laboratory measurement variables were obtained from the medical check-up system. The high SUA level group was classified as participants with hyperuricaemia, and obesity was defined as BMI ≥28 kg/m2. Logistic regression model was performed for data at baseline. For all participants, generalised estimation equation (GEE) model was used to assess the association between SUA and obesity, where the data were repeatedly measured over the 9-year study period. Subgroup analyses were performed by gender and age group. We calculated the cut-off values for SUA of obesity using the receiver operating characteristic curves (ROC) technique.Results A total of 15 959 participants (10 023 men and 5936 women) were included in this study, with an average age of 37.38 years (SD: 13.27) and average SUA of 367.05 μmol/L (SD: 97.97) at baseline, respectively. Finally, 1078 participants developed obesity over the 9-year period. The prevalence of obesity was approximately 14.2% for high SUA level. In logistic regression analysis at baseline, we observed a positive association between SUA and risk of obesity: OR=1.84 (95% CI: 1.77 to 1.90) for per-SD increase in SUA. Considering repeated measures over 9 year for all participants in the GEE model, the per-SD OR was 1.85 (95% CI: 1.77 to 1.91) for SUA and the increased risk of obesity were greater for men (OR=1.45) and elderly participants (OR=1.01).
