    Abstract Introduction. Severe ocular injuries caused by fishing equipment are relatively rare. The visual prognosis for fishing-related injuries depends on the involved ocular structures, the presence of complications, and surgical techniques applied. Case outline. A 40-year-old man reported a sudden severe sharp pain and a loss of vision in his left eye while he was pulling the fishing rod during recreational fishing. At admission, his best corrected visual acuity (BCVA) of the left eye was 1/60. Clinical examination revealed a laceration in the temporal quadrant of the bulbar conjunctiva 0.2–0.3 mm in size and a dark tumefaction under the conjunctiva in the same region. It was identified during primary surgical exploration as a fishing sinker 1 cm in size lodged under the lateral rectus muscle. The bulbar wall was intact and the fishing sinker was safely removed. Phacoemulsification with the implantation of an artificial foldable intraocular lens and 23G pars plana vitrectomy were performed. During vitrectomy, subretinal hemorrhage in the macular region and large retinal dialysis in the temporal segment were revealed. Vitrectomy was finished with silicone oil tamponade. One week later, the patient’s BCVA of the left eye was 2/60. Eight months after surgery, spontaneous resorption of subretinal hemorrhage in the macular region and the attached retina was observed. The patient’s BCVA was 3/60 due to the destruction of photoreceptors and retinal pigment epithelium and the formation of epiretinal membrane. Conclusion. To our knowledge, this is the first case report of a severe blunt ocular injury associated with large retinal dialysis caused by a fishing sinker.

    Abstract Introduction. Leber’s hereditary optic neuropathy (LHON) typically affects young adults, with a higher prevalence in men, but can ultimately occur at any age, as well as in women. LHON is caused by point mutations in the mitochondrial DNA. Classically, LHON presents as a subacute unilateral loss of visual acuity, dyschromatopsia in the red–green axis and a central or centrocecal scotoma. The contralateral eye usually develops similar symptoms within 3–6 months of the disease onset. Case outline. A 55-year-old male patient presented to a neurologist 20 days after the onset of vision loss. The patient was admitted as an emergency case to the Clinic for Eye Diseases due to a sudden vision loss in both eyes. The best corrected visual acuity in both eyes was 4/60. The intraocular pressure on both eyes was normal. Oedema of the optic nerve head was found on the right eye and a disc with blurred borders was seen on the left eye. During hospitalization, several consultative examinations and diagnostic procedures were performed, together with blood laboratory and visual field perimetry. Genetic testing for LHON as well as antibodies to AQ4, immunoserology, virology, and lumbar puncture were performed, as well as the visual evoked potential and ultrasound examinations. Conclusion. In our patient, the presence of a heteroplasmic mutation m.11778 G>A (MT-ND4) in the mitochondrial DNA analyzed from a peripheral blood sample was shown. In the available literature, this is the first documented LHON case demonstrating complete restitution of visual acuity in both eyes.

    Abstract Background: Dry eye is a multifactorial disease defined less than 30 years ago. It is a relatively common disorder, affected by a number of well-known risk factors. Dry eye can be challenging to diagnose because of the possible discrepancy between patients' symptoms and clinical signs, and its overlap with other ocular surface diseases. Literature-wise, dry eye is usually associated with age and therefore investigated within older populations. Recently, studies focusing on young adult and student populations have demonstrated a higher prevalence of dry eye than previously expected.
    Aim: The study aims to determine the frequency of dry eye symptoms in the student population, and the impact of students' activities and habits as potential risk factors.
    Methodology: Our study involved 397 students from the medical school at the University of Belgrade, Serbia. Students were asked to complete an online survey that addressed general information, health, habits, and routine in everyday use of electronic devices. In addition, students completed a standard Ocular Surface Disease Index questionnaire.
    Results: The prevalence of dry eye was 60.5% (240/397) in our study population. Contact lens wear (p<0.001), allergies (p = 0.049) and increased number of hours per day using VD devices for studying purposes (p = 0.014) were associtated with a higher risk of dry eye disease. Risk factors that did not significantly impact dry eye were the use of oral contraceptives, smoking, systemic diseases, year of study and sex.
    Conclusion: In our study, the prevalence of dry eye disease was similar or slightly higher than in previous studies among young adults. In addition, contact lenses, allergies and visual display devices were associated with the development of the dry eye.

    Abstract Purpose: The study was conducted to determine the ocular pulse amplitude (OPA) changes, measured with a dynamic contour tonometer (DCT), after surgical retinal detachment repair.
    Methods: This was a prospective and comparative study. Thirty patients (30 eyes) who had undergone uncomplicated unilateral scleral buckling and encircling procedures for quadrant or half-retinal rhegmatogenous retinal detachment were referred for DCT one day before the surgery was performed, on the 1
    Results: OPA values decreased significantly after scleral buckling procedures (p < 0.0001), but regained near to preoperative values one month after the surgery.
    Conclusion: OPA tends to decrease after retinal detachment surgery. Restoring patients' vision with scleral buckling and encircling procedures gives early changes in blood supply to the choroid and ocular nerve, and since OPA is an indirect parameter of choroidal vascularization, measuring these values can help make an insight into ocular hemodynamics.

    Abstract Introduction/Objective. The aim of this study was to report visual, refractive, and tomographic outcomes of corneal collagen cross-linking (CXL) in pediatric keratoconus. Methods. This retrospective study included 17 eyes of 12 patients with progressive keratoconus who underwent epithelium-off CXL at the age ≤ 18 years. Following data were analyzed at baseline and postoperatively at one, three, six, nine, 12 months for all the patients, and annually where available: uncorrected distant visual acuity (UDVA) and best spectacle-corrected distant visual acuity (CDVA), refraction and corneal tomography. Results. Mean UDVA improved significantly from 0.52 ± 0.38 at baseline to 0.24 ± 0.29 logarithm of minimum angle resolution (log MAR) at one year (p = 0.011) and remained stable at two-year follow-up (0.21 ± 0.34 log MAR). Mean CDVA was 0.15 ± 0.21 at baseline and 0.06 ± 0.13 log MAR at one year (p = 0.248). Maximum keratometry showed a significant flattening of 1.30 ± 1.99 D (p = 0.011) after a year and remained stable two years after CXL. Minimum keratometry significantly decreased with a mean change of 1.34 ± 1.37 (p = 0.001). Mean reduction of corneal thickness after CXL was 55.35 ± 64.42 μm (P=0.003). After a year, seven (42%) eyes showed Kmax regression, nine (53%) stabilization, and one (5%) progression. Conclusion. In our study CXL effectively prevented progression of keratoconus in 95% of pediatric patients after a year, while improving UDVA and keratometry values. One patient with eye rubbing behavior showed signs of keratoconus progression after CXL treatment.

    Abstract Introduction. We report a rare case of cataract surgery in a patient with an extreme, widespread anterior staphyloma following severe bilateral necrotising anterior scleritis associated with granulomatosis with polyangiitis (GPA). Case report. A 61-year-old man with a history of GPA developed bilateral, rapidly progressive necrotising scleritis and peripheral ulcerative keratitis (PUK). Inflammation compromised the entire anterior globe and peripheral cornea in both eyes. More than 90% of the surface area healed within 8 weeks, following the treatment with 3 pulsed doses of methylprednisolone in addition to the cyclophosphamide treatment. Systemic steroid therapy was slowly tapered over a period of 6 months. Extraordinary scleral loss with a uveal bulge developed, following severe necrotising anterior scleritis associated with PUK. Once the full remission had been achieved after 6 months, uncomplicated phacoemulsification was performed in his left eye, the only functional one. Conclusion. Preoperative and postoperative control of inflammation, careful surgical planning, and meticulous surgical techniques are critically important for optimal surgical outcome in such patients. To our knowledge, phacoemulsification in a patient with coexisting uveitic cataract and severe anterior staphyloma has not been previously reported.

    Abstract Purpose of review: Ophthalmic herpes simplex virus (HSV) of the anterior segment is responsible for a range of corneal complications such as scarring, thinning, neovascularization, and severe loss of vision. This review provides current guidelines for treating anterior segment disease related to HSV.
    Recent findings: We first review findings from the Herpetic Eye Disease Study (HEDS) clinical trials, and then review new topical and antiviral therapies developed since the HEDS studies. The development of vaccines to prevent recurrent episodes of herpetic infection is briefly reviewed. New corneal surgical procedures, developed since HEDS, may put patients at risk for ocular HSV disease: cross-linking and excimer refractive surgery.
    Summary: HEDS established the standard of HSV ocular therapy and is still valid today. However, newer antivirals may provide easier compliance with improved bioavailability, efficacy, dosage, and tolerability. Further research is needed to prevent latency of HSV, decrease recurrences, and more effectively treat necrotizing keratitis associated with HSV.

    Abstract Introduction. Chemical injuries can occur under various circumstances and may cause serious damage to the anterior segment of the eye. Objective. The aim of the study was to analyze the frequency of chemical injuries treated in a tertiary referral centre. Methods. The medical records of consecutive patients admitted for the chemical injury of the eye to the Clinic for Eye Diseases in Belgrade between January 1999 and December 2008 were retrospectively analyzed in order to obtain data about proportions of injuries, demographical characteristics of patients, circumstances under which injuries happened, the chemicals involved, the severity of injury according to the Roper-Hall classification and the length of hospitalization. Results. Out of a total of 60,868 hospitalized patients, 148 (2.43/1,000) were treated for chemical injury of the eye, with the highest incidence observed during summer months. Men were over five times more often affected (84.5%) and most of the injured individuals belonged to the working-age population (mean age 44.4±16.2 years). The most common causative agents were alkalis (73.0%), while acids (18.2%) and other substances (8.8%) were less common. None of 35.1% patients for whom data were obtained used any protection while handling the chemicals. The vast majority of injuries were graded as grade II (31.1%) and III (42.6%) and the most severe ones were caused by alkalis. An average length of hospitalization was 17.7±24.1 days and correlated with the severity of injury. Conclusion. Chemical injuries are relatively common problem in the Emergency Ophthalmology. Constant education and usage of adequate protective equipment should be advised in order to prevent serious complications.
