    Abstract Aims: To investigate the association between cardiovascular risk factors and cardiovascular risk score with the thyroid status of patients with Hashimoto’s Thyroiditis. Methods: Thirty-eight consenting adults with Hashimoto’s Thyroiditis participated in this cross-sectional study. The cardiovascular risk factors considered included age, sex, blood pressure, body mass index, fast blood glucose, lipid profile, cardiovascular comorbidities, C reactive protein, and erythrocyte sedimentation rate. The Framingham CV risk score was performed. The sample was classified into euthyroid (n = 15), clinical hypothyroidism (n = 9), and subclinical hypothyroidism (n = 13), and included the presence of antithyroid antibodies. Fisher’s exact test was used to determine the association between the variables studied. Results: 100% of the sample were women; a mean age between 39-59 years old. The category with low risk was the largest (n = 30), equivalent to 78.9%; moderate risk, no patient was obtained; high risk (n = 8) constituted 21.1%. Statistical significance between age and CV risk score in patients with clinical hypothyroidism was found (p < 1), 95% CI. The glucose level in the subclinical hypothyroidism and clinical hypothyroidism had statistical significance. The presence of anti-Thyroglobulin (antiTg) was shown to be closely related to the level of CV risk in patients with subclinical hypothyroidism. Conclusion: Age, glycemia, anti-Tg, history of DM, dyslipidemia, or cerebrovascular accidents have been linked to raising the risk of developing CVD in up to 10 years depending on their thyroid profile. No evidence of a direct relationship between CV risk score and thyroid state was found in the participants of this study.

    Abstract Purpose: Process improvement in service industries, like the registration process in a hospital, can be achieved with the application of lean principles. In this specific case, lean principles were essential to analyze and improve patient satisfaction in a hospital in Montana. The clinics involved in the study included pediatrics (P), internal medicine (IM), and cardiology/pulmonary (CP). The purpose of this study was to address difficulties regarding patient satisfaction on the registration and check-in processes. Design/methodology/approach: Direct observations and patient surveys were initially made to understand the processes and identify the initial causes of dissatisfaction. A value stream map (VSM) was then used to break down even further the complexity of the patient flow. A Fishbone diagram and a concept map were completed to get to the root of the dissatisfaction. Findings: The information obtained from the lean tools showed that patients questioned the need for having a central registration in combination with the check-in process when this combination generates duplications of steps that cause unwanted delays. Several recommendations were explored by the engineering team to mitigate these delays and improve the registration process reducing the number of patients complaints by 40%. Practical implications: This project illustrated the application of Lean principles to resolve issues regarding a central registration format in a healthcare facility. Originality/value: A concept map was used as a tool that helps business organizations in the development of creative and new ways of looking and solving process deficiencies.

    Abstract Software-Defined Networking (SDN) and Internet of Things (IoT) are the trends of network evolution. SDN mainly focuses on the upper level control and management of networks, while IoT aims to bring devices together to enable sharing and monitoring of real-time behaviours through network connectivity. On the one hand, IoT enables us to gather status of devices and networks and to control them remotely. On the other hand, the rapidly growing number of devices challenges the management at the access and backbone layer and raises security concerns of network attacks, such as Distributed Denial of Service (DDoS). The combination of SDN and IoT leads to a promising approach that could alleviate the management issue. Indeed, the flexibility and programmability of SDN could help in simplifying the network setup. However, there is a need to make a security enhancement in the SDN-based IoT network for mitigating attacks involving IoT devices. In this article, we discuss and analyse state-of-the-art DDoS attacks under SDN-based IoT scenarios. Furthermore, we verify our SDN sEcure COntrol and Data plane (SECOD) algorithm to resist DDoS attacks on the real SDN-based IoT testbed. Our results demonstrate that DDoS attacks in the SDN-based IoT network are easier to detect than in the traditional network due to IoT traffic predictability. We observed that random traffic (UDP or TCP) is more affected during DDoS attacks. Our results also show that the probability of a controller becoming halt is 10%, while the probability of a switch getting unresponsive is 40%.

    Abstract Introducción: la malnutrición infantil representa uno de los problemas de salud pública más importantes de la República Dominicana (RD) y el mundo. A pesar de esto, actualmente, no existen estudios en la RD que describa el estado nutricional en los niños de la escuela primaria. Material y métodos: este estudio transversal describió las principales variables antropométricas en niños de 1ero a 6to de primaria en Santiago, RD, además de determinar la relación existente entre la antropometría y variables sociodemográficas. Resultados: de los 2,271 estudiantes estudiados, la media del peso fue 33,2 ± 11,4 kg, la talla fue 1,36 ± 0,13 m. La media del percentil fue 65,33 %. El 3.92 % (n=89) estuvo en bajo peso, el 17.57 % (n=399) estuvo en sobrepeso y el 22.94 % (n=521) estuvo en obesidad. Conclusión: el sobrepeso y la obesidad infantil fueron los trastornos más comunes en zonas rurales y urbanas, y tanto en centros privados como públicos.

    Abstract Objetivo. El objetivo fue desarrollar una herramienta para medir los niveles de adherencia al tratamiento antirretroviral (la TARV) en un entorno de escasos recursos, a partir de la combinación de cuatro métodos de medición de adherencia. Métodos. Revisión retrospectiva de 500 expedientes médicos de personas que viven con VIH, elegidos de manera aleatoria desde octubre del 2017 hasta enero del 2020. Se midió la adherencia a la TARV combinando cuatro métodos de medición (porcentaje de cobertura de la TARV recetada, recogida de la TARV en farmacia, nivel de carga viral y autoinforme de adherencia). Se realizaron pruebas de chi al cuadrado con P <0,05 para diferencias estadísticamente significativas y regresión binaria logística para identificar probabilidades de adherencia óptima y subóptima. Realizamos pruebas de Spearman para correlación de categorías y alfa de Cronbach para medir la consistencia interna de la herramienta. Resultados. Obtuvimos 497 calificaciones de adherencia. De estas, 307 (61,8%) usuarios se calificaron como adherentes, 141(28,4%) como semiadherentes y 49 (9,8%) como no adherentes. Se encontró una mayor probabilidad de adherencia óptima en grupos de 60 años o más (odds ratio [OR]: 1,6; IC95%: 0,8-3,5) sin diferencia entre hombres y mujeres (OR: 0,9; IC95%: 0,7-1,4). La prueba de Spearman informó una relación (r = 0,8) entre los niveles de carga viral y la calificación final, y la prueba alfa de Cronbach arrojó una modesta consistencia interna (α = 0,7). Conclusiones. Se desarrolló una herramienta para medir adherencia en un entorno de escasos recursos. La herramienta presenta niveles modestos de consistencia interna y una correlación fuerte en la categoría de carga viral y adherencia.

    Abstract In human environments subjected to natural hazards, places such as plazas, parks and free areas can, after a catastrophe, be places for refuge which can satisfy survival needs and support adaptation. The relationships of such an open space system and the urban form, are explored in this study by focussing on the spatial context of two Chilean cities affected by earthquakes. Data was collected in interviews with people from emergency organisations using the Projecting Mapping Technique, and subjected to Content and Geographic Information System analyses to identify the type, utility and distribution of the open space system for earthquake recovery. The objective was to evaluate, by means of different spatial indexes, the extent the open space system of these cities impact on measures associated with urban resilience, namely overlap in governance and diversity. Findings suggest that the regularity of the grid and city density affect the adaptive capacity of cities, hence, resilience. Findings also shed light on a methodological approach, including participatory and geographical data, through which these resilient aspects can be explored and evaluated in other human environments prone to earthquakes.

    Abstract El presente estudio de mercado forma parte de la estrecha colaboración que existe entre la Universidad Tecnológica de la Costa con el sector productivo. El objetivo fue determinar la factibilidad de expandir la comercialización de productos lácteos de la empresa Villahgo en la ciudad de Santiago Ixcuintla, Nayarit. Se aplicaron 117 encuestas, para el cálculo de la muestra se fijó un error máximo de 8%, trabajando con 92% de nivel de confianza. El levantamiento de información en las colonias seleccionadas se hizo de forma aleatoria. Se determinó que en la mayoría de los hogares de Santiago Ixcuintla se acostumbra consumir productos lácteos por su sabor agradable y por los beneficios nutrimentales que perciben en ellos; se detecta así también que el porcentaje de las personas que no consumen lácteos es debido a la intolerancia a la lactosa, por lo que no representa amenaza alguna para la expansión de la comercialización de los productos de la empresa Villahgo.
