Abstract The World Anti-Doping Agency now allows cannabidiol (CBD) to be taken in elite sport, but has retained all other cannabinoids on its banned list. This, together with liberalisation of cannabis laws, has led to an increasing interest in its use in sport. This includes commercial sponsorship of sporting events and highlights the need for accurate information to be readily available to athletes and their advisers. This narrative review confirms that CBD and tetrahydrocannabinol (THC) do not enhance performance. CBD possesses anti-inflammatory and analgesic properties that have not been adequately evaluated in the area of sport. Some formulations of CBD contain THC or other cannabinoids that may result in a positive drug test.

Abstract A systematic review was undertaken to evaluate studies on the effects of theophylline and theobromine on exercise performance in normal (healthy) subjects. Theophylline was found to have been studied on eight occasions and theobromine on one, the number of subjects per study ranging from seven to 15. In two exercise investigations, theophylline was superior to placebo at a p < 0.05 level and no different in four including one that was conducted in artificial hypoxia. In a treadmill time trial over 3 km, theobromine was superior to placebo and equal to caffeine, at the <0.05 level. In strength studies, theophylline increased wrist strength in one and showed a slight but not statistically significant increase in limb strength in four of the seven subjects in the other. Theophylline caused adverse effects in six participants. There were no adverse effects in the theobromine investigation. Although the studies showed contradicting results and/or insufficient data to draw solid conclusions, it appears both drugs have potential to enhance performance and could be considered for inclusion on the WADA banned list.

Abstract Complementary medicines are a multibillion-dollar industry of which vitamin supplements are an important component. Most of the claims of benefit are not evidenced-based, and vitamin supplements may be associated with severe adverse reactions that are uncommon but can occur with high doses of some vitamins. There is no case for vitamin supplementation in normal, healthy, non-pregnant or lactating adults who are receiving the recommended daily intake of nutrients.

Abstract The impact of exercise-specific face masks (ESFMs) in aerobically fit individuals on physiological, perceptual, respiratory, and performance responses remains unclear. How ESFMs mitigate exercise-induced bronchoconstriction (EIB) is also unknown. Thus, this study aimed to determine how an ESFM altered within-exercise physiological, perceptual, respiratory, and performance responses to graded treadmill exercise. Twenty-four individuals (11 females) completed a discontinuous graded exercise test on a treadmill under two conditions (ESFM and unmasked). Physiological, respiratory function, and perceptual measures were assessed. Performance was determined by time to exhaustion. Statistical analyses included linear mixed-effects modeling, repeated measures analysis of variance, and pairwise comparisons using an alpha value of 0.05. ESFM use significantly impaired performance (median = -150.5 s) and decreased arterial oxygen saturation at maximal intensity (mean = -3.7%). Perceptions of air hunger and work of breathing were elevated across submaximal and maximal intensities. Perceived exertion and breathing discomfort were significantly elevated submaximally but not maximally. Spirometry measures were not significantly different at termination but were significantly improved at submaximal intensities in participants with and without EIB. ESFM use in fit individuals increased perceptual discomfort, impaired performance, and augmented arterial desaturation. Respiratory function improvements were observed but were accompanied by adverse perceptual sensations. Despite this, performance impairments may limit the real-world utility of ESFMs for athletes.
