    Abstract Background and aim Pityriasis versicolor is a common chronic superficial fungal infection that manifests during or after puberty in hot humid temperatures. Topical treatments, which are the first line of treatment, are divided into nonspecific and specific antifungal agents; however, the therapeutic approaches for pityriasis versicolor depend mainly on synthetic antifungal drugs, in particular ketoconazole. However, due to the frequent recurrence of this disease, and the widespread application of azole drugs, resistant strains have emerged leading to increasingly failed treatment rates. Retinoic acid creams are also effective against pityriasis versicolor. Especially adapalene gel that has less irritation compared with other topical retinoid products and also has rapid onset of action. Therefore, we aimed to evaluate the efficacy and safety of a combination treatment of ketoconazole cream 2% and adapalene gel 0.01% in the treatment of pityriasis versicolor. Patients and methods In all, 100 patients were divided equally into two groups: group 1 patients were treated with topical application ketoconazole 2% cream and adapalene 0.1% gel once daily for a total duration of 2 weeks. Group 2 patients were treated with a topical application ketoconazole 2% cream for a total duration of 2 weeks. Results There was significant improvement rates in the combined group than ketoconazole-only group with P value=0.023. Adepalene+ketoconacole group had more irritation as a side effect than the ketoconazole group with significant P value=0.001. Conclusion The combination of adapalene 0.1% gel and ketoconazole 2% cream is safe, rapid, and effective in the treatment of pityriasis versicolor.

    Abstract Introduction: Psoriasis is a chronic skin condition that demonstrates a high burden and an impaired quality of life. The role of excimer light therapy and its modes of action are not completely understood in the treatment of psoriasis. Aim: To evaluate the clinical efficacy and safety of Monochromatic Excimer Light (MEL) in the treatment of psoriasis. Materials and Methods: Ninety Egyptian psoriasis patients with plaque-type psoriasis were treated with MEL twice a week, for a total of 24 sessions or until complete improvement. The Psoriasis Severity Index (PSI) was assessed for each patient. Results: At the final visit, 96/360 (26.67%) patches of psoriasis showed complete clearance, 75/360 (20.83%) patches achieved more than 75% improvement, 132/360 (36.67%) patches achieved 51-75% improvement and 57/360 (15.83%) patches achieved 26-50% improvement. The best result occurred on the trunk followed by lower limbs, upper limbs, palms and soles than the scalp. There was inverse statistical correlation between the percentage of improvement and skin phototype and no correlation with patient’s age, sex or duration of disease. Side effects reported were erythema in 45 patches, post inflammatory hyperpigmentation in 102 patches, perilesional skin was covered by protective sheet, so no side effect occurred. No recurrence was observed six months after treatment. Conclusion: The clinical data indicate that 308 nm excimer light is safe and effective in the treatment of psoriasis and the outcome of treatment is affected by skin phototype and site of the disease. Post inflammatory hyperpigmentation was the most common side effect.

    Abstract Background Chronic urticaria is a common immunological disorder, with a prevalence of 15–25%. Several exogenous and endogenous causes have been proposed as causative agents; one of them is infection of the gastrointestinal tract. Despite thorough investigation, the etiology remains unresolved in more than 80% of the cases. Objective The aim of the present study was to assess the prevalence of chronic idiopathic urticarial (CIU) patients presented with Helicobacter pylori infection and to study the effect of H. pylori eradication on the skin lesion in patients with CIU. Methodology This prospective study was carried out at the outpatient clinic of Dermatology Department of Al Azhar Assuit University Hospital on 30 patients with chronic urticaria (20 women and 10 men); their ages ranged from 16 to 65 years. The duration of the disease ranged from 6 weeks to 18 years. A primary diagnosis of CIU was made when an etiological factor accounting for the majority of the lesions could not be elucidated during the history-taking and physical examination. All the patients were subjected to careful history-taking at initial visit; physical examination to assess the number of urticaria wheals, size, and distribution, and associated angioedema; gastroscopy to obtain gastric mucosal biopsy from the antrum; and histopathological assessment of the gastric mucosal biopsy using hematoxylin and eosin, and Giemsa stains. The patients were scored according to the severity of disease (number of wheals, area of distribution, and the duration of the disease; itching and style of life; history of angioedema). The patients were treated for 2 weeks with amoxicillin (1 g/twice daily), metronidazole (500 mg/three doses daily), and omeprazole (20 mg/twice daily). All patients were followed up during the study duration of 3 months.
