Abstract India has one of the highest oral cancer burdens and accounts for one out of every five cervical cancer incidences worldwide. Majority of these preventable cancers are diagnosed in advanced stages with poor prognosis and survival. World Health Organization supports health literacy as a measure for accomplishing sustainable development goals. Community trials have reported that health literacy-focused interventions improve cancer screening participation and adherence. In India health literacy research is unutilized for cancer screening. Majority of the research utilized proxy information using disease-specific knowledge, attitude, and socio-demographic characteristics for screening participation. Through this correspondence, we discuss the poor cancer screening coverage in India and the research gap in health literacy in Indian context. Without an understanding of the distribution of the components of health literacy and the development of context-specific interventions for improvement, it will be difficult for any technology or innovation to penetrate the community and increase screening coverage.

Abstract The electronic cigarette (EC) was developed as an alternative to cigarette smoking. In less than a decade, the prevalence of past-month EC usage increased from 1.5 to 27.5 % among US high-school students. In the coming years, Asia-Pacific countries will have the highest sales of electronic nicotine/non-nicotine delivery systems (ENDS/ENNDS) after Western Europe. Based on the World Health Organization and Indian Council of Medical Research recommendations, India approved a complete ban on EC in 2019. Even though it has been three years since the ban, EC is still being sold in India's grey markets, where marketing is not regulated. In this narrative review, we discuss that vaping is not just a harm reduction strategy for tobacco smoking cessation but poses a serious threat to India's existing tobacco control efforts as well as the health of the country's young people.

Abstract Context: Cervical cancer is the most common cancer among rural women of India. However, awareness of cancer of the uterine cervix and its screening coverage among the general population of India remains insufficient.
Aims: The study aims to assess awareness of cervical cancer and its screening among women attending a rural health care center in northern India and determine factors associated with satisfactory knowledge.
Settings and Design: A cross-sectional observational study was done among women attending a rural secondary health care center from Uttar Pradesh, India.
Materials and Methods: A total of 1088 women aged ≥30 years were interviewed using a pretested schedule. Data were collected for biosocial, reproductive, sexual, and personal habits of participants and their partners. Scoring for knowledge related to cervical cancer and its screening was done.
Statistical Analysis Used: Descriptive statistics were calculated. Chi-square test was applied to detect the significant difference in distribution of bio-socio-demographic variables with knowledge score. Statistically significant variables were subjected to multinomial logistic regression. Unadjusted and adjusted odds ratios with 95% confidence interval were calculated as odds of having poor cervical cancer awareness. p < 0.05 was considered statistically significant.
Results: Most participants knew about cervical cancer as a type of cancer in women. Very few knew about symptoms, risk factors, and screening of the disease. Illiteracy and multiple sexual contacts were significant predictors of awareness.
Conclusions: The study demonstrates a lack of awareness in women regarding cervical cancer and its prevention, especially among those women who belonged to weaker sections of the society, because of illiteracy and poor socioeconomic status. Lack of awareness is a potential limiting step for a woman to seek cervical cancer screening. Multipronged strategies are needed to improve the level of cervical cancer awareness among women.

Abstract The coronavirus disease 2019 (COVID-19), caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), was declared as pandemic by World Health Organization (WHO) in March 2020. The outbreak has caused 5,232,562 deaths worldwide until December 3rd, 2021. Though primarily affecting the respiratory system, involvement of other organ systems have been reported in severe disease. Venous thromboembolism (VTE) has been recognized as an important complication. Previous studies have reported the prevalence of VTE in intensive care unit (ICU) patients between 7 and 85% and in non-ICU patients between 0 and 19%. COVID-19 patients that are at high risk for VTE are also at increased risk for bleeding. In such cases, anticoagulation may potentially be harmful. Thereby, it is important to understand the risk factors for VTE predisposition in the COVID-19 patients, timing of VTE, and the rate of occurrence of VTE in hospitalized patients post-discharge. Comparison of the rate of occurrence of VTE in COVID-19 patients with the non-COVID-19 patients with similar disease severity is required to truly interpret the reportedly high rates of VTE in COVID-19 patients. Several pathophysiological mechanisms have been reported for the development of VTE in COVID-19. Autopsy-based studies have contributed to the existing knowledge. d-dimer, presently, seems to be the most suitable investigation for risk-identification of VTE supported by Doppler studies and overall clinical context. Further, prospective studies and clinical trials are essentially required to fill the gaps in evidence for occurrence, risk prediction and management of VTE in COVID-19 patients.

Abstract Objective: Despite the established cancer screening programme for oral, breast and cervical cancer by the Government of India, the screening coverage remains inadequate. This study aimed to describe the determinants for oral, breast and cervical cancer prevention in a rural community at the primary care level of Northern India and its policy implications.
Design: This was a camp-based project conducted for 1 year, using oral visual examination, clinical breast examination and visual inspection of cervix by application of 5% acetic acid according to primary healthcare operational guidelines. During the project, screen-positive participants were followed through reverse navigation. Information about socio-demographic profile, clinical and behavioural history and screening were collected. Predictors for screen-positivity and follow-up compliance were identified through multivariable analysis.
Settings: Based on the aim of project, one of the remotely located and low socioeconomic rural blocks, having 148 villages (estimated population of 254 285) in Varanasi district, India was selected as the service site. There is an established healthcare delivery and referral system as per the National Health Mission of Government of India. Oral, breast, gallbladder and cervical cancers are the leading cancers in the district.
Participants: We invited all men and women aged 30-65 years residing in the selected block for the last 6 months for the screening camps. Unmarried women, women with active vaginal bleeding, those currently pregnant and those who have undergone hysterectomy were excluded from cervical cancer screening.
Results: A total of 14 338 participants were screened through 190 camps and the majority (61.9%) were women. Hindu religion, tobacco use, intention to quit tobacco and presence of symptoms were significantly associated with screen-positivity. Nearly one-third (220; 30.1%) of the screened-positives complied with follow-up. Young age and illiteracy were significantly associated with lower compliance.
Conclusion: Poor follow-up compliance, despite the availability of tertiary cancer care, patient navigation, free transportation and diagnostic services, calls for research to explore the role of contextual factors and develop pragmatic interventions to justify 'close the care gap'. Community cancer screening needs strengthening through cancer awareness, establishing referral system and integration with the National Tobacco Control and Cancer Registry Programmes.

Abstract It has been 46 years since the launch of cancer control programs in India and yet the recent National Family Health Survey (fifth round, 2019-2021) has reported that just 1.9% of women aged 30-49 years have ever undergone cervical cancer screening. The cost of delayed diagnosis of cervical cancer and its treatment is overwhelming, and the rural population takes the worst hit. It is the need of the hour that the Indian health system and policymakers identify the barriers and facilitators for cervical cancer early detection and provide pragmatic solutions so that the targets of cervical cancer elimination can be achieved in a timely manner.

Abstract The major intent of peptide vaccine designs, immunodiagnosis and antibody productions is to accurately identify linear B-cell epitopes. The determination of epitopes through experimental analysis is highly expensive. Therefore, it is desirable to develop a reliable model with significant improvement in prediction models. In this study, a hybrid model has been designed by using stacked generalisation ensemble technique for prediction of linear B-cell epitopes. The goal of using stacked generalisation ensemble approach is to refine predictions of base classifiers and to get rid of the worse predictions. In this study, six machine learning models are fused to predict variable length epitopes (6-49 mers). The proposed ensemble model achieves 76.6% accuracy and average accuracy of repeated 10-fold cross-validation is 73.14%. The trained ensemble model has been tested on the benchmark dataset and compared with existing sequential B-cell epitope prediction techniques including APCpred, ABCpred, BCpred and [inline-formula removed].

Abstract Background: Surgeons are commonly evaluated by surgical skills and outcomes rather than their character traits. We sought to examine role model behaviours of senior surgeons through the lens of Aristotelian (virtue) ethics.
Methods: Semi-structured focus group interviews were undertaken of anaesthetic trainees at a large university hospital NHS Foundation Trust and transcripts were subjected to thematic analysis to yield themes and subthemes. Participation of the trainees was entirely voluntary and focus groups were conducted using Zoom™.
Results: The overarching themes identified were 'Teamwork makes the dream work', 'Captain of the ship' and 'Strong foundations'.
Conclusion: We hope to take lessons learnt in conjunction with our previous work to help refocus surgical training towards a process of character reformation, rather than simply imparting technical skills to trainees.
