    Abstract Crush injuries of the hand involve damage to multiple structures within the hand, loss of tissue, devascularisation and possibly amputation of digits. They are complex and difficult to classify. Each injury has a unique pattern and requires a unique plan of management. There are no set procedures that can be described, however fundamental principles can be laid down to guide surgical management. Optimum management requires a planned and decisive approach. The surgeon or the team must be well versed with techniques necessary for management of all the structures within the hand, microsurgery and free tissue transfer. The essential components of management are, accurate assessment of the injury and creating a reconstructive plan by structures. The essential components for primary surgery are precise and complete debridement, skeletal stabiliation, vascular repair and if vessels are exposed, soft tissue cover. Secondary surgery should include procedures to enhance the function of the hand or to improve the aesthetics of the hand. These include bone grafting, fusion, tendon and nerve reconstruction, flap de-bulking and toe to hand transfers. The following article is a concise synthesis of the fundamental principles which a hand surgeon must understand while undertaking these challenging procedures.

    Abstract Background Injecting drug use (IDU) is associated with several cardiometabolic risks. We aimed to measure the independent effects of IDU behaviour and related factors on serum lipid profile among people who inject drugs (PWIDs). Methods We did a longitudinal study with six follow-up measurements at an interval of 2 months among 104 PWIDs from 11 selected hotspots under two blocks in West Bengal, India. Generalized estimating equations with robust standard errors analysed the effect of addiction habits on lipid profile parameters. Results The mean (SD) age of the participants was 27.6 (5.24) years, 36.5% married and 44.3% were unemployed at the time of recruitment. At the baseline, the mean (SD) body mass index (BMI) and fasting blood sugar (FBS) were 20.0 (1.82) kg/m2 and 112.0 (15.90) mg/dl, respectively. The mean duration of drug use was 2.5 (1.20) years. While 62.5% had normal triglyceride (TG), 14.4% had high total cholesterol (TC) and 69.2% had dyslipidaemia at the baseline. Adjusted for age, BMI, FBS and other addiction-related variables, models showed that longer duration of drug use (>3 years) resulted in higher levels of TG, higher TC-to-high-density lipoprotein ratio and dyslipidaemia. Tobacco use and high FBS level were also risk factors for dyslipidaemia. Conclusions Higher duration of IDU, tobacco use and higher FBS were associated with deranged lipid profile among PWIDs.

    Abstract Mutilating injuries include a wide and heterogeneous spectrum of clinical presentations, each being unique in terms of pattern of tissue damage, patient characteristics, and functional requirements. Understanding the principles of reconstruction of bone and soft tissues, a wide repertoire of surgical techniques, and the ability to plan the reconstructive journey leading to a functional hand are crucial. Management of these injuries involves several on-the-spot decisions by the surgeon. This article aims to equip the surgeon with the key principles and the bits of knowledge that are essential for effective planning and execution when dealing with such injuries.

    Abstract Background: Atrial flutter with 1:1 conduction to the ventricles is a dangerous cardiac arrhythmia. Contemporary guidelines recommend atrioventricular nodal blocking agents should be co-administered with class 1C anti-arrhythmics, as prophylaxis against 1:1 flutter. No guidance is provided on the type or strength of atrioventricular nodal blockade required, and in practice, these agents are frequently prescribed at low dose, or even omitted, due to their side effect profile.
    Case presentation: A 62 year old Caucasian man with a history of paroxysmal atrial fibrillation treated with flecainide, presented with atrial flutter with 1:1 conduction to the ventricles and was cardioverted. Diltiazem was added to prevent this complication and he again presented with atrial flutter with 1:1 conduction to the ventricles, despite prophylaxis with coadministration of diltiazem.
    Conclusions: This case report demonstrates failure of diltiazem to prevent 1:1 flutter in a patient chronically treated with flecainide for paroxysmal atrial fibrillation.

    Abstract The maximum edge colouring problem considers the maximum colour assignment to edges of a graph under the condition that every vertex has at most a fixed number of distinct coloured edges incident on it. If that fixed number is $q$ we call the colouring a maximum edge $q$-colouring. The problem models a non-overlapping frequency channel assignment question on wireless networks. The problem has also been studied from a purely combinatorial perspective in the graph theory literature. We study the question when the input graph is sparse. We show the problem remains $NP$-hard on $1$-apex graphs. We also show that there exists $PTAS$ for the problem on minor-free graphs. The $PTAS$ is based on a recently developed Baker game technique for proper minor-closed classes, thus avoiding the need to use any involved structural results. This further pushes the Baker game technique beyond the problems expressible in the first-order logic.

    Abstract Josephson tunnel junctions exhibit a supercurrent typically proportional to the sine of the superconducting phase difference ϕ. In general, a term proportional to cos(ϕ) is also present, alongside microscopic electronic retardation effects. We show that voltage pulses sharply varying in time prompt a significant impact of the cos(ϕ) term. Its interplay with the sin(ϕ) term results in a nonequilibrium fractional Josephson effect (NFJE) ∼sin(ϕ/2) in the presence of bound states close to zero frequency. Our microscopic analysis reveals that the interference of nonequilibrium virtual quasiparticle excitations is responsible for this phenomenon. We also analyze this phenomenon for topological Josephson junctions with Majorana bound states. Remarkably, the NFJE is independent of the ground state fermion parity unlike its equilibrium counterpart.
