    Abstract Chest pain complicated with electrocardiographic changes is not an uncommon scenario in emergency departments, which should be examined cautiously. We describe a 51-years-old man with a myocardial bridge of coronary artery presenting with simultaneous Mobitz type I atrioventricular block on electrocardiography. Echocardiography excluded valvular abnormality and systolic/diastolic dysfunction. Coronary angiography confirmed the diagnosis of a myocardial bridge at the middle segment of the left anterior descending artery, involving the most dominant septal perforator branch with marked systolic compression. The patient underwent coronary artery bypass grafting surgery and was followed up uneventfully at the outpatient department with medical treatment of diltiazem and clopidogrel. The present case is being reported to highlight that clinicians should be alert to such a congenital abnormality as a potential cause of repeated myocardial infarction and conduction abnormality.

    Abstract Introduction: Metabolic syndrome (MS) is strongly associated with cardiovascular diseases and diabetes but can be prevented with regular physical activity. This study aimed to assess the impact of a physical fitness training programme on MS among military personnel.
    Methods: This retrospective observational study included volunteer army soldiers who underwent annual health examinations between 2011 and 2014. In 2011, the reformed physical fitness training programme and physical fitness test were introduced to the participants. MS evaluation and physical fitness performances were evaluated before and after implementing the training programme using a mixed-effects model and generalised estimating equation, adjusted for sex, age and smoking.
    Results: From 2011 to 2014, 1720 soldiers underwent the annual health examination. In 2011, before the fitness programme, 246 soldiers (14.3%) had MS. After implementation, decreases in blood pressure and fasting glucose levels were observed and maintained for 3 years. Running performance was negatively correlated to triglycerides (ß=-11.37; p<0.001) and waist circumference (ß=-0.42; p<0.001) and positively correlated to high-density lipoprotein cholesterol levels (ß=2.14; p<0.001). The severity of MS was reduced following introduction of the physical fitness programme.
    Conclusions: MS and its components improved after introducing the reformed fitness programme, with running performance proving to be most relevant to MS. Clinicians should encourage increased physical activity to prevent MS among military personnel.

    Abstract The objective of this study was to evaluate the digestible energy (DE), metabolizable energy (ME) content, and apparent total tract digestibility (ATTD) of gross energy (GE) in extruded defatted rice bran and pelleted defatted rice bran, and to determine apparent ileal digestibility (AID) and standardized ileal digestibility (SID) of crude protein (CP) and amino acid (AA) in two types of defatted rice bran (DFRB) fed to growing pigs. Extruded DFRB is the rice bran co-product remaining after oil is removed by steam pressing and extracting whereas pelleted DFRB is produced by pelleting and extracting. In experiment (Exp) 1, 30 barrows [Duroc × (Large White × Landrace)] with an initial body weight (BW) of 37.6 ± 4.4 kg were allotted to one of five diets (n = 5) including a corn-soybean meal basal diet and four DFRB test diets containing 292 g/kg of two types of DFRB replacing corn or soybean meal in one basal diet. Pigs were placed in metabolism crates for 7 days of adaptation followed by a 5-day total collection period of feces and urine to detect DE, ME and ATTD of GE in DFRB. Experiment 2 was conducted to evaluate CP and AA digestibility of DFRB. Six growing pigs (24.7 ± 5.2 kg BW) were allocated to 1 of 3 treatments. Diets included one N-free diet and two test diets which contained 600 g/kg extruded DFRB or pelleted DFRB as the only source of CP and AA. On a dry matter (DM) basis, extruded DFRB contained less fibrous compounds than pelleted DFRB. Except for cysteine, the concentration of AA was less in extruded DFRB than in pelleted DFRB. On an as-fed basis, ME contents in extruded DFRB were greater (P < 0.05) than pelleted DFRB. Pelleted DFRB had greater (P < 0.05) AID of Trp and Gly than extruded DFRB, but for other AA, AID was not different. The SID of all AA was not different between the two sources of DFRB. In conclusion, compared with pelleted DFRB, extruded DFRB may be better for pigs considering its greater DE value.

    Abstract Background: This study surveyed the characteristics of white matter (WM) micro-integrity in patients who were diagnosed with major depressive disorder (MDD) without co-morbidities.
    Method: A total of 44 patients with MDD and 27 normal controls were enrolled in our study. Diffusion tensor imaging images of patients and controls were pre-processed and analysed to estimate differences in WM micro-integrity between patients and controls by performing comparisons of the values obtained from fractional anisotropy (FA). FA outputs of patients and controls were compared by a non-parametric permutation-based method with global brain volume, age and gender as covariates. In addition, the between-group differences of radial diffusivity (RD) and axial diffusivity (AD) were assessed to explain the alterations in FA values. Correlations between clinical variables (such as depression severity, anxiety severity, illness duration) and FA values were also estimated in each group and across both groups.
    Results: The patients with MDD had significantly lower FA values than the controls, for the left superior longitudinal fasciculus (SLF) and the right anterior thalamic radiation (ATR). The reductions in FA values occurred in combination with elevated RD values in the bilateral SLF and decreased AD values in the bilateral ATR. FA values were negatively correlated with depression severity in the SLF and with illness duration in the right SLF and ATR.
    Conclusions: MDD patients had significant alterations in the WM micro-integrity of the left SLF and the right ATR.

    Abstract In Computational Fluid Dynamics (CFD) studies composed of the coupling of different simulations, the uncertainty in one stage may be propagated to the following stage and affect the accuracy of the prediction. In this paper, a framework for uncertainty quantification is applied to the two-step simulation of the mechanical design of a swirling jet flow generated by a rotating pipe (\textit{Simulation 1}) impinging on a flat plate to provide convective heat transfer (\textit{Simulation 2}). The first approach is the Stochastic Collocation Method (SCM) with Clenshaw-Curtis sparse grids. The conclusion drawn from the analysis is that the simulated system does not exhibit a significant sensitivity to stochastic variations of model input parameters, over the tested uncertainty ranges. Additionally, a set of non-linear regression models for the stochastic velocity and turbulent profiles for the pipe nozzle are created and tested, since impinging jets at Reynolds number of $Re=23000$ are very frequent in the literature, but stochastic inlet conditions have never been provided. Numerical results demonstrate a negligible difference in the predicted convective heat transfer with respect to the use of the profiles simulated via CFD. These suggested surrogate models can be directly embedded onto other CFD applications (e.g arrays of jets or jet flows impinging on plates with different shapes) in which a realistic swirling flow under uncertainty can be of interest.

    Abstract Objective: We assumed that assistive technology in mobility devices (that is, wheelchairs with external power and driving modified vehicle (MV) with or without driving on wheelchair) may facilitate social participation for wheelchairs users who have spinal cord injuries (SCIs). This study examined the relationship between mobility devices and social participation in this population.
    Methods: We included 2986 individuals who had received initial rehabilitation at one of 18 regional centers of the Model Spinal Cord Injury System in the United States, had been interviewed between 2004 and 2010, and were wheelchair users (use a wheelchair > or = 40 h per week and cannot ambulate 150 feet at home). We performed secondary panel-data analysis using a mixed-effect model on data from 3498 follow-up interviews. Participation (measured by the Craig Handicap Assessment and Reporting Technique-Short Form (CHART-SF) and employment status) and the use of wheelchair and MV were recorded.
    Results: Among the participants, 33% drove an MV, and 44% used an external-powered wheelchair. The use of an MV was positively related to employment and CHART-SF score, regardless of driving directly or driving with a wheelchair. People who drove an MV were found to have approximately two more business associates to contact to once a month and ∼2 additional days out of home per week compared with those without an MV. No significant association was shown between the type of wheelchair used and participation.
    Conclusion: The use of an MV was found to be positively associated with social participation in an SCI population.
