    Abstract There has been a major growth in global health networks in recent decades to address health issues including tobacco use, alcohol abuse, and maternal mortality. Most conditions that incur high costs have provoked networks of advocates working to mitigate the impact, increase investment in research, and establish campaigns. Global health networks often work simultaneously across policy, knowledge creation, and advocacy. Until recently there has been limited activity in global health networks and advocacy addressing hearing loss and deafness which has contributed to its relatively low visibility with policymakers compared to other health conditions. This discussion paper reports on a global consultation that explored the views of advocacy groups and individuals on advocacy for the management of hearing loss, and cochlear implantation (CI). It focussed on stakeholders' views of current advocacy endeavours, opportunities and barriers, and the possible development of a global advocacy network to improve access to cochlear implantation and the supporting services. The subsequent development of a global health network, the Cochlear Implant International Community of Action (CIICA) is discussed and the conditions necessary for the successful development of health networks are explored. This paper will be of interest to those wishing to understand the factors influencing the development of health networks and advocacy.

    Abstract In January 2020, NHS England and NHS Improvement, in the United Kingdom, issued a permissive framework for streamlining cancer multidisciplinary (MDT) meetings. Streamlining is defined as a process whereby complex cases are prioritized for full discussion by an MDT in an MDT meeting (MDM), while the management of straightforward cases is expedited using Standards of Care (SoC). SoC are points in the pathway of patient management where there are recognized guidelines and clear clinical consensus on the options for management and should be regionally agreed and uniformly applied by regional Cancer Alliances. While this report marks the first major change in cancer MDT management since the Calman-Hine report in 1995, its implementation, nationally, has been slow with now nearly four years since its publication. It is argued however that streamlining is a necessary step in ensuring the viability of MDT processes, and therefore maintaining patient care in the current socioeconomic context of rising workload and cancer incidence, financial pressures, and workforce shortages. In this mini review, we offer a succinct summary of the recent developments around the implementation of the 2020 streamlining framework, including challenges and barriers to its implementation, and the potential future directions in this field, which we propose should increase utilisation of implementation science. We conclude that ensuring successful implementation of the framework and the SOC requires securing a buy-in from key stakeholders, including MDTs and hospital management teams, with clearly defined (a) management approaches that include triage (e.g. through a mini MDT meeting), (b) assessment of case complexity (something that directly feeds into the SOC), and (c) roles of the MDT lead and the members, while acknowledging that the SOC cannot be universally applied without the consideration of individual variations across teams and hospital Trusts.

    Abstract Since the National Institute of Health and Care Excellence (NICE) review of cochlear implantation in 2009, (NICE, 2009) there have been a number of significant changes to our understanding of the impact of severe-to-profound hearing loss on quality of life and comorbidity with life limiting illness. There have been questions about the validity of current methods of assessing candidacy for cochlear implants. There have also been significant improvements in the effectiveness of implants, the age of successful implantation and a reduction in costs. Additionally, the costs to the health and welfare system of not addressing severe-to-profound hearing loss are often not considered when assessing costs and benefits of this technology and when assessing candidacy criteria. Consideration of these changes since the NICE review suggests the need for an urgent review of the current guidance.

    Abstract Thoracic outlet syndrome (TOS) most commonly manifests in overhead athletes (e.g., baseball pitchers, swimmers, weight lifters) due to nerve compression caused by skeletal abnormalities. We present the case of a 43-year-old recreational runner with unilateral upper extremity pain while running. Vascular imaging identified an aberrant subclavian artery origin with positional compression in the absence of cervical bone anomalies confirming arterial TOS. A first rib resection and anterior scalenectomy led to symptom resolution. This case emphasizes the importance of a broad differential and complete workup in non-overhead athletes presenting with symptoms consistent with neurogenic TOS, as vascular interventions may be necessary to prevent future complications.

    Abstract Objectives: Orthotopic bladder substitution (OBS) is a management option for urinary diversion in men and women undergoing cystectomy. The aim of the procedure is to provide a functional continent urinary reservoir of adequate capacity, compliance and low pressure. We have provided a narrative review of the existing literature and highlighted areas where improvement and standardization can be recommended.
    Methods: Literature search included database search for publications from January 1970 to November 2020, using keywords including OBS, bladder reconstruction, neobladder, radical cystectomy, robotic cystectomy, intracorporeal neobladder, surgical technique, patient selection and outcomes.
    Results: Due to various factors including indications, operative technique and risk of complications, OBS is an enormous undertaking and commitment for patients, surgeons and health professionals involved in the care pathway. The main considerations for patient selection, the technical elements of the procedure and the rationale behind these are discussed. Previously considered to be a choice for a select few, the inclusion criteria have expanded over the last decade. Similarly, surgical techniques including the choice and configuration of bowel segments, construction of anastomosis and nerve or organ sparing procedures have evolved over the years. Minimally invasive laparoscopic and robotic assisted surgery has added further perspectives to the existing literature on OBS. Understanding the principles of operative techniques and assessing the best evidence to influence patient management is crucial as it has a major impact on clinical outcomes. Peri- and post-operative care, focused on the prevention of complications and morbidity, affects long-term functional and oncological outcomes, which ultimately dictates the quality of life.
    Conclusions: This concise overview of OBS literature highlights the importance of pre-operative, peri-operative, and post-operative aspects with regards to the optimization of patient care. To achieve the best results, meticulous attention should be paid in all these areas, surgical and multi-disciplinary. Patient education and counseling, with shared decision making are central to the success of the procedure.

    Abstract Stromal tumour of uncertain malignant potential (STUMP) is a rare tumour of the prostate with variable and unpredictable risk and clinical progression. There is no clear consensus on how it should be managed. We describe two cases of patients who presented to their general practitioners with raised prostate-specific antigen in one instance and an abnormal digital rectal examination in the other. Biopsies were carried out and a diagnosis of STUMP was made. Both cases were managed with robot-assisted radical prostatectomy and after 4.8 years of follow-up, there has not been any recurrence. This is the first reported UK case in which STUMP has been successfully managed with robotic surgery with excellent medium-term results. Radical prostatectomy can be used to treat STUMP tumour with good oncological and functional outcomes.
