    Abstract Purpose: Helicobacter pylori
    Patients: US Veterans who completed H. pylori testing between 1999 and 2018.
    Methods: We conducted a retrospective cohort analysis among adults within the Veterans Health Administration who completed testing for H. pylori. The primary exposures were (1) H. pylori test result (positive/negative) and (2) H. pylori treatment (untreated/treated) among H. pylori-positive individuals. The primary outcomes were CRC incidence and mortality. Follow-up started at the first H. pylori testing and continued until the earliest of incident or fatal CRC, non-CRC death, or December 31, 2019.
    Results: Among 812,736 individuals tested for H. pylori, 205,178 (25.2%) tested positive. Being H. pylori-positive versus H. pylori-negative was associated with higher CRC incidence and mortality. H. pylori treatment versus no treatment was associated with lower CRC incidence and mortality (absolute risk reduction 0.23%-0.35%) through 15-year follow-up. Being H. pylori-positive versus H. pylori-negative was associated with an 18% (adjusted hazard ratio [adjusted HR], 1.18 [95% CI, 1.12 to 1.24]) and 12% (adjusted HR, 1.12 [95% CI, 1.03 to 1.21]) higher incident and fatal CRC risk, respectively. Individuals with untreated versus treated H. pylori infection had 23% (adjusted HR, 1.23 [95% CI, 1.13 to 1.34]) and 40% (adjusted HR, 1.40 [95% CI, 1.24 to 1.58]) higher incident and fatal CRC risk, respectively. The results were more pronounced in the analysis restricted to individuals with nonserologic testing.
    Conclusion: H. pylori positivity may be associated with small but statistically significant higher CRC incidence and mortality; untreated individuals, especially those with confirmed active infection, appear to be most at risk.

    Abstract Deep NLP models have been shown to learn spurious correlations, leaving them brittle to input perturbations. Recent work has shown that counterfactual or contrastive data -- i.e. minimally perturbed inputs -- can reveal these weaknesses, and that data augmentation using counterfactuals can help ameliorate them. Proposed techniques for generating counterfactuals rely on human annotations, perturbations based on simple heuristics, and meaning representation frameworks. We focus on the task of creating counterfactuals for question answering, which presents unique challenges related to world knowledge, semantic diversity, and answerability. To address these challenges, we develop a Retrieve-Generate-Filter(RGF) technique to create counterfactual evaluation and training data with minimal human supervision. Using an open-domain QA framework and question generation model trained on original task data, we create counterfactuals that are fluent, semantically diverse, and automatically labeled. Data augmentation with RGF counterfactuals improves performance on out-of-domain and challenging evaluation sets over and above existing methods, in both the reading comprehension and open-domain QA settings. Moreover, we find that RGF data leads to significant improvements in a model's robustness to local perturbations.

    Abstract Background and purpose - Long-term fixation of cemented femoral stems relies on several factors including cement adhesion and fatigue. Hi-Fatigue is a newer third-generation bone cement with low-viscosity properties at room temperature, good mechanical strength, and stable bone-cement interface in a laboratory testing environment. Palacos bone cement has excellent 10-year survival and is considered gold standard. We compared stem subsidence after fixation with Hi-Fatigue and Palacos bone cements using radiostereometry. Patients and methods - In a patient-blinded randomized controlled trial, 52 patients (30 women) at mean age 76 years (71-87) with osteoarthrosis and no osteoporosis received Hi-Fatigue G or Palacos R + G cement fixation of collarless, polished, double-tapered stems (CPT). Tantalum beads were inserted in the periprosthetic bone. Supine stereoradiographs were obtained postoperatively, 3 months, 6 months, 1 year, and 2 years after surgery. Oxford Hip Score (OHS) and VAS pain were recorded preoperatively and 1 and 2 years after surgery. Cement working times and properties were registered. Results - At 2 years, mean stem subsidence of 1.12 mm (95% CI 0.96-1.29) for Hi-Fatigue and 1.19 mm (CI 1.03-1.34) for Palacos was similar. Likewise, stem version was comparable between cement groups. Mean OHS and VAS pain were similar between cement groups. Cement working times were similar between cement groups, but the mean curing time was longer for Hi-Fatigue (13.7 min) than for Palacos (11.6 min). Interpretation - We found similar and generally low migration of CPT femoral stems inserted with Hi-Fatigue and Palacos bone cement until 2 years' follow-up, which indicates a good long-term survival of polished taper femoral stems inserted with both cement types.

    Abstract Two-stage revision with 'StageOne™ Select Hip Cement Spacer' is used as treatment of periprosthetic hip joint infection. The aim is to evaluate complications and restoration the joint with the StageOne Select Spacer. From 2013 to 2018 twenty-nine StageOne Select Spacers was inserted. Data was obtained through medical records and radiographs. 3.5% dislocated the spacer, 6.9% sustained a femoral fracture, 6.9% got reinfected and 6.9% failed to control the infection. Leg length discrepancy was 1.5 mm (-4,25-5.25) and offset discrepancy was 4 mm (-0.5-9). The spacer shows promising results with a low reinfection and dislocation rate and allowance of restoration of the joint.

    Abstract Snow water equivalent (SWE) can be measured using low-cost Global Navigation Satellite System (GNSS) sensors with one antenna placed below the snowpack and another one serving as a reference above the snow. The underlying GNSS signal-based algorithm for SWE determination for dry- and wet-snow conditions processes the carrier phases and signal strengths and additionally derives liquid water content (LWC) and snow depth (HS). So far, the algorithm was tested intensively for high-alpine conditions with distinct seasonal accumulation and ablation phases. In general, snow occurrence, snow amount, snow density and LWC can vary considerably with climatic conditions and elevation. Regarding alpine regions, lower elevations mean generally earlier and faster melting, more rain-on-snow events, and shallower snowpack. Therefore, we assessed the applicability of the GNSS-based SWE measurement at four stations along a steep elevation gradient (820, 1185, 1510 and 2540 m a.s.l.) in the eastern Swiss Alps during two winter seasons (2018–2020). Reference data of SWE, LWC and HS were collected manually and with additional automated sensors at all locations. The GNSS-derived SWE estimates agreed very well with manual reference measurements along the elevation gradient, and the accuracy (RMSE = 34 mm, RMSRE = 11 %) was similar under wet- and dry-snow conditions, although significant differences in snow density and meteorological conditions existed between the locations. The GNSS-derived SWE was more accurate than measured with other automated SWE sensors. However, with the current version of the GNSS algorithm, the determination of daily changes of SWE was found to be less suitable compared to manual measurements or pluviometer recordings and needs further refinement. The values of the GNSS-derived LWC were robust and within the precision of the manual and radar measurements. The additionally derived HS correlated well with the validation data. We conclude that SWE can reliably be determined using low-cost GNSS sensors under a broad range of climatic conditions, and LWC and HS are valuable add-ons.

    Abstract Background: The clinical significance of GLI1 expression either through canonical Hedgehog signal transduction or through non-canonical mechanisms in rhabdomyosarcoma (RMS) or Ewing sarcoma (EWS) is incompletely understood. We tested a role for Hedgehog (HH) signal transduction and GL11 expression in development of vincristine (VCR) resistance in RMS and EWS.
    Methods: We characterized baseline expression and activity of HH pathway components in 5 RMS (RD, Rh18, Ruch-2, Rh30, and Rh41) and 5 EWS (CHLA9, CHLA10, TC32, CHLA258, and TC71) cell lines. We then established VCR-resistant RMS and EWS cell lines by exposing cells to serially increasing concentrations of VCR and determining the IC
    Results: We found evidence for HH pathway activity and GLI1 expression in RMS and EWS cell lines at baseline, and evidence that GLI1 contributes to survival and proliferation of these sarcoma cells. We were able to establish 4 VCR-resistant cell lines (Ruch-2VR, Rh30VR, Rh41VR, and TC71VR). GLI1 was significantly up-regulated in the Rh30VR, Rh41VR, and TC71VR cells. The only other gene in the drug resistance panel that was significantly up-regulated in each of these VCR-resistant cell lines compared with their corresponding parental cells was the GLI1 direct target and multidrug resistance gene, ATP-binding cassette sub-family B member 1 (MDR1). We established major vault protein (MVP), which was up-regulated in both vincristine-resistant alveolar RMS cell lines (Rh30VR and Rh41VR), as another direct target of GLI1 during development of drug resistance. Treatment of the VCR-resistant cell lines with the small molecule inhibitor GANT61 or GLI1 siRNA together with VCR significantly decreased cell viability at doses that did not reduce viability individually.
    Conclusions: These experiments demonstrate that GLI1 up-regulation contributes to VCR resistance in RMS and EWS cell lines and suggest that targeting GLI1 may benefit patients with RMS or EWS by reducing multidrug resistance.

    Abstract This case report describes an 18-year-old woman, who had symptomatic bilateral external and internal snapping hip and completed 28 of 30 training sessions during a 12-week resistance training intervention. The intervention proved to be feasible and improved maximal strength in hip abduction and extension measured with Isokinetic Dynamometry. Improvements were also seen in The Copenhagen Hip and Groin Outcome Score, and The Forgotten Joint Score. Furthermore, visual analog scale pain markedly decreased after the intervention.
