    Abstract In many applications of X-ray computed tomography, an unsupervised segmentation of the reconstructed 3D volumes forms an important step in the image processing chain for further investigation of the digitized object. Therefore, the goal is to train a clustering algorithm on the volume, which produces a voxelwise classification by assigning a cluster index to each voxel. However, clustering methods, e.g., K-Means, typically have an asymptotic polynomial runtime with respect to the dataset size, and thus, these techniques are rarely applicable to large volumes. In this work, we introduce a novel clustering technique based on random sampling, which allows for the voxelwise classification of arbitrarily large volumes. The presented method conducts efficient linear passes over the data to extract a representative random sample of a fixed size on which the classifier can be trained. Then, a final linear pass performs the segmentation and assigns a cluster index to each individual voxel. Quantitative and qualitative evaluations show that excellent results can be achieved even with a very small sample size. Consequently, the unsupervised segmentation by means of clustering becomes feasible for arbitrarily large volumes.

    Abstract Gallensteine führen bei Kindern und Jugendlichen selten zu klinischen Symptomen, können aber schon bei Säuglingen nachgewiesen werden. Unterschieden werden Pigmentsteine, welche durch Bindung von Bilirubin an Kalzium entstehen, Cholesterinsteine, welche durch eine Cholesterinkristallbildung in der Galle entstehen und sogenannte gemischte Steine, welche gehäuft bei Mukoviszidose beobachtet werden. In den meisten Fällen werden Gallensteine im Rahmen von Sonografien des Abdomens entdeckt, ohne dass sie bis dahin durch Symptome auffielen. Symptomatische Gallensteine manifestieren sich beim Säugling unspezifisch. Erst beim älteren Kind werden kolikartige Bauchschmerzen mit Ausstrahlung in die rechte Schulter beobachtet. Als Komplikationen treten Cholezystitis, die biliäre Pankreatitis und die Choledocholithiasis auf. Während man sich bei asymptomatischen Gallensteinen eher zurückhaltend hinsichtlich einer Therapie verhalten sollte, ist bei symptomatischen Steinen die Choleszystektomie indiziert. Eine Lyse mit Ursodeoxycholsäure ist nur bei einem geringen Anteil der Patienten erfolgversprechend, eine Lithotrypsie ist bei Kindern obsolet.

    Abstract Background: Gastrointestinal endoscopy is one of the most frequently performed diagnostic and therapeutic procedures in children nowadays.
    Methods: Since the size of endoscopes has been adapted to pediatric patients, endoscopic procedures are performed almost routinely in pediatric gastrointestinal patients. However, this is an invasive procedure done under deep sedation, and procedure-related or sedation-related complications can occur more easily in pediatric patients. The endoscopist must consider various aspects (e.g., indications and contraindications for endoscopy, monitoring during and after the procedure) to ensure the procedure is performed successfully and safely.
    Results: In our opinion, pediatric endoscopy should be carried out by pediatric endoscopists at specialized pediatric clinics. In Germany, there are regulatory demands dictating that in order to become a pediatric gastroenterologist, further education is necessary. Experienced centers which offer this education including training programs for pediatric endoscopy can be found on www.gpge.de.
    Conclusion: Provided these preconditions are adhered to, gastrointestinal endoscopy is a safe and efficient technique with minimal complications even in children and infants.
