    Abstract This book adopts an interdisciplinary approach with a wide scope of perspectives on primary healthcare, describing related principles, care models, practices and social contexts. It combines aspects of development, research and education applied in primary health care, providing practitioners and scholars with a comprehensive overview of the current knowledge and delivery models of healthcare in community settings. It covers the practical, philosophical and scholarly issues pertinent to the delivery, financing, planning, ethics, health politics, professional and technological development, resources, and monitoring in primary health care. Contributors are from a diverse range of academic and professional backgrounds, bringing together collective expertise in mainstream medicine, nursing, allied health, Chinese medicine, health economics, administration, law, public policy, housing management, information technology and mass communications. As such, the book does not follow the common clinical practice or service-based approach found in most texts on primary care.The contents will serve as a useful reference work for policymakers, researchers, community health practitioners, health executives and higher education students.

    Abstract This paper discusses the rapidly evolving healthcare risk landscape and considers how emerging trends-such as advancement of medical technology, cyber security, pandemic risks, increasing prevalence of noncommunicable health conditions, and the shift towards patient autonomy-are shaping the nature of liabilities faced by doctors and healthcare professionals.     Abstract Studies have shown that adolescents now have a higher exposure to health risks than those in the past, and Hong Kong adolescents are no exception, particularly with the social crisis in 2019 and then the COVID-19 pandemic in 2020. Data from health care services for children and adolescents only represent the tip of the clinical iceberg, and health profiles, including living habits, lifestyles, data on health status, and health service utilization, are not always readily available for effective planning to cultivate a healthy living environment. In this paper, an exploratory study on secondary school students was conducted in one district of Hong Kong that has the potential and resources to become a healthy city. Students were found to have a high prevalence of poor sleep quality and quantity; excess screen time; physical inactivity; an insufficient intake of healthy food; emotional disturbance, including symptoms of post-traumatic stress disorders (PTSDs); and irregular attendance to student health services. A multivariate analysis showed that excessive time on social media websites, short sleeping hours, symptoms of PTSD, and a lack of regular exercise were independently associated with emotional distress. Youth service providers should re-orientate student health services, moving away from routine services to be more outreaching in order to cultivate a supportive living and learning environment, promoting better health for adolescents.

    Abstract In this pilot study we investigated the vocal strategies of Cantonese women when addressing an attractive vs. unattractive male. We recruited 19 young female native speakers of Hong Kong Cantonese who completed an attractiveness rating task, followed by a speech production task where they were presented a subset of the same faces. By comparing the rating results and corresponding acoustic data of the facial stimuli, we found that when young Cantonese women spoke to an attractive male, they were less breathy, lower in fundamental frequency, and with denser formants, all of which are considered to project a larger body. Participants who were more satisfied with their own height used these vocal strategies more actively. These results are discussed in terms of the body size projection principle.

    Abstract This case presents a patient with two transposed rare diagnoses developed within 10 years. Thrombotic thrombocytopenic purpura (TTP) and essential thrombocythemia (ET) are disease processes that present with opposite clinical and laboratory findings. The patient was diagnosed with ET over a decade after the initial TTP diagnosis when she was found to have extreme thrombocytosis during routine laboratory monitoring. The patient was found to have the calreticulin (CALR) mutation variant of ET which is associated with increased platelet production and she was started on hydroxyurea and aspirin. Subsequent management of the patient's TTP relapses and large fluctuations in her platelet counts necessitated adjustments to the standard ET treatment regimen. There is little to no literature on this rare comorbidity and further investigation is needed for the association between these diseases and modifications to standard treatment to prevent relapses and sequelae.

    Abstract Machado-Joseph disease (MJD) is a devastating and incurable neurodegenerative disease characterised by progressive ataxia, difficulty speaking and swallowing. Consequently, affected individuals ultimately become wheelchair dependent, require constant care, and face a shortened life expectancy. The monogenic cause of MJD is expansion of a trinucleotide (CAG) repeat region within the ATXN3 gene, which results in polyglutamine (polyQ) expansion within the resultant ataxin-3 protein. While it is well established that the ataxin-3 protein functions as a deubiquitinating (DUB) enzyme and is therefore critically involved in proteostasis, several unanswered questions remain regarding the impact of polyQ expansion in ataxin-3 on its DUB function. Here we review the current literature surrounding ataxin-3's DUB function, its DUB targets, and what is known regarding the impact of polyQ expansion on ataxin-3's DUB function. We also consider the potential neuroprotective effects of ataxin-3's DUB function, and the intersection of ataxin-3's role as a DUB enzyme and regulator of gene transcription. Ataxin-3 is the principal pathogenic protein in MJD and also appears to be involved in cancer. As aberrant deubiquitination has been linked to both neurodegeneration and cancer, a comprehensive understanding of ataxin-3's DUB function is important for elucidating potential therapeutic targets in these complex conditions. In this review, we aim to consolidate knowledge of ataxin-3 as a DUB and unveil areas for future research to aid therapeutic targeting of ataxin-3's DUB function for the treatment of MJD and other diseases.
