    Abstract Niger, one of the most impoverished countries in the world, has fared consistently poorly on a range of human development indicators, including women’s nutritional status. A combination of nutrition specific and nutrition sensitive interventions that also address women’s empowerment is essential to address nutritional goals, particularly in resource-constrained settings. Yet, there is not much clarity on what sectors policy makers should target. To identify the aspects of women’s empowerment that are associated with nutritional outcomes in Niger, we present a novel application of a regression decomposition technique, the Shapley-Owen decomposition. After categorizing the drivers of undernutrition into a series of domains (food, health, fertility and institutions) and dimensions (knowledge, resources, and agency), we predict women’s body mass index and anemia status based on these domain-dimensions and other controls using Demographic and Health Survey data. We find that access to health resources and fertility resources play a substantial role in women’s nutritional outcomes in Niger. By identifying the relative contribution of each domain-dimension, our approach offers a unique insight into what sectors are most closely related to the nutritional status of Nigerien women and can aid in identifying interventions that both empower women and improve nutritional wellbeing.

    Abstract Shannon Entropy is the preeminent tool for measuring the level of uncertainty (and conversely, information content) in a random variable. In the field of communications, entropy can be used to express the information content of given signals (represented as time series) by considering random variables which sample from specified subsequences. In this paper, we will discuss how an entropy variant, the \textit{permutation entropy} can be used to study and classify radio frequency signals in a noisy environment. The permutation entropy is the entropy of the random variable which samples occurrences of permutation patterns from time series given a fixed window length, making it a function of the distribution of permutation patterns. Since the permutation entropy is a function of the relative order of data, it is (global) amplitude agnostic and thus allows for comparison between signals at different scales. This article is intended to describe a permutation patterns approach to a data driven problem in radio frequency communications research, and includes a primer on all non-permutation pattern specific background. An empirical analysis of the methods herein on radio frequency data is included. No prior knowledge of signals analysis is assumed, and permutation pattern specific notation will be included. This article serves as a self-contained introduction to the relationship between permutation patterns, entropy, and signals analysis for studying radio frequency signals and includes results on a classification task.

    Abstract Background and aim: Vedolizumab is a novel monoclonal antibody used in patients with inflammatory bowel disease, often affecting women of child-bearing age. We aimed to compare maternal and fetal adverse outcomes in pregnancies of women with inflammatory bowel disease exposed to vedolizumab versus those on other treatment.
    Methods: We performed a systematic literature search through December 2020 looking for studies including outcomes from pregnancies of female inflammatory bowel disease patients treated with vedolizumab. Our primary outcome was a composite of adverse pregnancy-related events in pregnancies of female patients on vedolizumab compared with those of disease-matched controls on other medication regimens. Events of interest included preterm births, early loss of pregnancy, late fetal death, elective termination of pregnancy, and congenital anomalies.
    Results: Four studies were included in our review meeting criteria for our primary analysis. Compared with those with no vedolizumab exposure, pregnancies with vedolizumab exposure had an increase in overall adverse pregnancy-related outcomes (odds ratio [OR] 2.18, 95% confidence interval [CI], 1.52-3.13). The vedolizumab group also had increased preterm births (OR 2.16, 95% CI, 1.28-3.66), and early loss of pregnancies (OR 1.79, 95% CI, 1.06-3.01) but no difference in number of live births (OR 0.60, 95%CI, 0.36-1.00), or congenital malformations (OR 1.56, 95% CI, 0.56-4.37).
    Conclusions: Our systematic review highlights possible concern with the general safety of vedolizumab in pregnancy, as an increase in overall total unfavorable outcomes was observed. Premature births and early loss of pregnancy were also more prevalent in pregnant female patients on vedolizumab. It is possible these findings are confounded by disease activity, and further prospective cohort studies of vedolizumab and pregnancy outcomes are warranted.

    Abstract The United Nations World Food Programme's (WFP's) Purchase for Progress (P4P), a multi-year, multi-country pilot, sought to improve smallholder farmer wellbeing through a combination of food purchases from farmer organizations and supply-side interventions. We examine the impacts of P4P on smallholder farmers in Tanzania, using panel data on members of participating and comparison farmer organizations. P4P targeted participants based on pre-existing warehouses locations. To account for possible selection on observables, we use propensity score matching, limiting the sample to participating farmers and their matches, and then compute conditional difference-in-differences. While participating small farmers increase their commercial farming activity relative to comparison farmers, we do not find evidence of increased income, increased food consumption scores or increased crop value. We discuss possible factors contributing to these findings, including WFP's diverse objectives and the targeting of farmer organizations rather than smallholders.

    Abstract Eine 27-jährige Frau zeigte multiple subkutane, teil bewegliche derbe Knötchen von bis zu 0,7 cm Größe an Ellenbögen, Stamm und Axillen. Vater und zwei der vier Geschwister hätten ähnliche Läsionen in geringerer Anzahl. Histologisch zeigten sich dermal und subkutan zystische Hohlräume, innenseitig von abgeflachtem Epithel ausgekleidet. Ein Stratum granulosum war nicht nachweisbar, in der Zystenwand eingelagert fanden sich herdförmig Talgdrüsenkomplexe. Angesichts von Klinik und Histologie ergab sich die Diagnose eines Steatocystoma multiplex. Analog zur Phlebektomie von Seitenastvarizen nahmen wir in Lokalanästhesie mittels Skalpell Stichinzisionen von wenigen Millimetern Größe über den Zysten vor. Mittels eines Phlebektomie-Häkchens wurden die Zysten von ihrer Umgebung freipräpariert und an die Hautoberfläche gezogen. Per Scherenschlag erfolgte die Abtragung. Aufgrund der Schnittgröße konnte auf eine Naht verzichtet werden. Der postoperative Verlauf war komplikationslos.
    Kommentar: Steatozystome sind Zysten des Talgdrüsenausführungsganges. Sie treten einzeln oder multipel v.a. an Brust, Bauchnabel und Axillen auf. Zugrunde liegt eine Mutation im Keratin 17-Gen. Die Vererbung erfolgt autosomal dominant. Meist sind die Läsionen asymptomatisch. Entzündliche Verläufe werden beschrieben. Eine zufriedenstellende Therapie existiert zurzeit nicht. Orales Isotretinoin kann bei entzündlichen Formen erwogen werden. Die Exzision mit Dehnungsplastik kommt wegen der zahlreichen Läsionen oft nicht in Betracht. Kryotherapie, Stanzbiopsie, Punktion und Laser werden vorgeschlagen. Darunter verbleiben jedoch oft Narben, Rezidive sind häufig. Die Exstirpation mittels Phlebektomie-Häkchen, wie sie zur Phlebektomie von Seitenastvarizen verwendet werden, stellt eine kosmetisch zufriedenstellende Alternative bei kleineren Zysten dar.

    Abstract To explore whether choices for transfers are influenced by peer monitoring, we examine private versus public choices among monetarily equivalent values of cash and food in northern Kenya. Many northern Kenyan communities face high-levels of chronic food insecurity and are tightly-knit, often sharing food aid transfers. Yet, humanitarian cash transfers are relatively new to the area. Whether cash will be subject to the same sharing norms is not well understood. Utilizing a randomized block experiment, we find that assigning a respondent to choose in front of peers decreases the likelihood of choosing a cash transfer relative to at least some food. We argue that peer monitoring decreases the value of cash relative to food in two inter-related ways. Choosing food in public, first, provides an opportunity for respondents to publicly demonstrate a commitment to local food aid sharing norms. Second, choosing cash in public may be riskier since cash does not yet have established sharing norms. A critical implication is that transfer choices and retargeting of resources within the community can be influenced by use of public or private discussions when eliciting community views, especially in communities where sharing is a salient social norm.
