    Abstract IgA vasculitis (IgAV), formerly known as Henoch-Scholein purpura, is a small vessel vasculitis, most commonly seen in pediatric patients, that can affect numerous internal organs including the kidneys, lungs, gastrointestinal tract, and the central nervous system (CNS). CNS manifestations of this condition include hypertensive encephalopathy, thrombosis, optic neuropathy, seizures, CNS vasculitis, and a more recently described phenomenon known as posterior reversible encephalopathy syndrome (PRES). Symptoms of PRES include hypertension, altered mental status, and seizures caused by vasogenic disruption of the blood-brain barrier, and the condition is diagnosed by characteristic edema-related gray-white matter changes in the parieto-occipital lobes on magnetic resonance imaging. Herein, we present a rare case of PRES as a presenting sign of IgAV to increase awareness about this unusual association.

    Abstract As physicians, we spend time learning about diseases and their management. The key to successful outcomes is the involvement of patients and their families in the care of the conditions for which they seek our assistance. For atopic dermatitis (AD), all patients require frequent emollients for xerotic skin and care of the inflammation, which comes and goes frequently. The levels of care required vary by time and patient. Younger patients require the help of parents or other family members. A keen understanding of the timing and appropriate use of the various treatment modalities is required and is often confusing. Action plans, as used in the care of asthma, for instance, can be effectively integrated into the overall care instructions for AD and have been indispensable for many of my patients and their families. They are one concrete method for patient/family engagement in the overall care planning. The action plans can certainly be translated into languages appropriate for any practice environment. The following brief summary should be of use to those professionals considering the use of this technique.

    Abstract We present the case of a 20-month-old girl with Schimmelpenning-Feuerstein-Mims (SFM) syndrome with extensive head, neck, and torso skin involvement successfully managed with topical trametinib. Trametinib interferes downstream of KRAS and HRAS in the MAPK signaling pathway, of which KRAS was implicated in our child's pathogenic variant. Although other dermatologic conditions have shown benefit from oral trametinib, its topical use has not been well reported. Our patient showed benefit from the use of twice-daily topical trametinib, applied to the epidermal and sebaceous nevi over a 16-month period, leading to decreased pruritus and thinning of the plaques.

    Abstract Hemangiomas are common vascular birthmarks that usually present a predictable pattern of proliferation and ultimate involution. Most do not require any treatment. When intervention is clinically indicated, medical and surgical options exist. Historically, corticosteroids have been used and have been shown to slow or stop the growth of a majority of hemangiomas; however, growth concerns and infectious complications have complicated their use. In 2008, a letter to the editor in The New England Journal of Medicine described another serendipitous observation of the effect of the nonselective beta-blocker, propranolol, on hemangiomas in 9 cases. This finding has been expanded by the authors of this original observation as well as others.
