    Title translation The edge of chaos: A nonlinear view of psychoanalytic technique
    Abstract Das Feld der nichtlinearen Dynamik (oder Chaostheorie) ermöglicht es, Konzepte des psychoanalytischen Prozesses weiterzuentwickeln. Diese Weiterentwicklungen besitzen Implikationen für die Technik der Psychoanalyse. Der Beitrag beschreibt, dass Konzepte wie "Grenzbereich zum Chaos", Emergenz, Attraktoren und gekoppelte Oszillatoren zur Herausbildung einer analytischen Technik beitragen können, die es ermöglicht, die Analyse auf eine Weise zu praktizieren, die freier ist als die heutigen behandlungstechnischen Empfehlungen und gleichzeitig fester in einem erweiterten Verständnis der Wirkungsweise der Psychoanalyse gründet. Illustriert wird dieses Vorgehen anhand der langjährigen Analyse eines Patienten mit Zwangsstörung.

    Abstract As preconceptions about the meaning of sexual orientation are increasingly abandoned, analysts are left with the question of how these matters are to be addressed in clinical work. These problems are explored as they emerged in analytic work between a "gay" man and a "straight" analyst. Emphasis is placed on how the search for personal meaning is impacted by the intellectual and social context inhabited by analyst and patient.

    Abstract Careful preoperative screening of candidates for indwelling drug administration systems for the relief of intractable pain can help to exclude patients who will not benefit from this technology and predict efficacy in others. Unfortunately, bias on the part of both the treating physician and the patient can inappropriately skew the results of subjective or improperly controlled trials and lead to the implantation of drug administration systems in patients who will not benefit from chronic intrathecal narcotic administration. The author and his coworkers have designed a quantitative, crossover, double-blind paradigm for screening patients who might otherwise be deemed eligible for chronic intraspinal narcotic administration. This paradigm has been used 31 times in 30 patients; based on the outcome of this testing, 22 patients (73%) underwent implantation of chronic infusion systems. Sixteen (80%) of 20 patients with pain related to cancer underwent pump implantation, whereas only six (60%) of the 10 patients with pain of nonmalignant origin were so treated. Sixteen of the patients (72%) have reported good to excellent relief after pump implantation; this includes 12 (75%) of the 16 patients with pain related to cancer and four (66%) of the six patients with pain of nonmalignant origin. This screening paradigm thus appears to be both reliable and easily applied and promises to be of assistance in the selection of patients appropriate for this mode of therapy.

    Abstract Notable contributions have been made recently in the diagnosis and management of brain abscesses and subdural empyema. Several articles present an international perspective, echoing much of the Western literature, including a profound decrease in mortality with the availability of computed tomography and early recognition and treatment. Diagnostic studies have highlighted the superiority of magnetic resonance imaging to computed tomography in the detection of subdural empyema and the potential value of positron emission tomography. Novel diagnostic tools, including 99mTc-hexamethylpropyleneamine oxime leukocyte scintigraphy and C-reactive protein levels, have been evaluated and show great promise. Surgical studies comparing stereotactic drainage with craniotomy for brain abscesses and burr hole versus craniotomy drainage of subdural empyema suggest the efficacy of these more limited procedures. Local instillation of antibiotics is a potentially effective adjunct for refractory brain abscesses. Recent studies suggest that the most significant predictors of poor outcome are the patient's level of consciousness and the rapidity of disease progression prior to the initiation of treatment.

    Abstract The theory of dynamical systems (sometimes called chaos theory) has emerged in the past two decades as a powerful tool for understanding the evolution of complex systems. Attempts to develop psychoanalysis along the lines of nineteenth century physical science have proven unsatisfactory. The theory of dynamical systems provides another route for development. It suggests that prediction should aim at describing the overall evolution of systems and that the possibilities for such evolution are broader than classical theory suggested. It also shows that complex systems often involve structures that repeat basic features on several different levels of observation. This suggests a method for systematically exploring the overly rich data of psychoanalysis.
