    Abstract Aiming to reveal the relationship between TCM syndrome types and calcium and phosphorus metabolism in maintenance hemodialysis (MHD) sufferers, 50 sufferers diagnosed with MHD who were admitted to Suixi County Hospital in Anhui Province are selected for retrospective decomposition. The data of TCM classification and laboratory indicators of the sufferers are collected. The experimental results indicate that calcium and phosphorus metabolism disorder is common in maintenance hemodialysis patients, especially calcium and phosphorus product and blood phosphorus elevation. The increase of the calcium-phosphorus product is closely related to the mortality and calcification rate of the subcutaneous soft tissue. It can provide a reference for the diagnosis and therapy of abnormal calcium and phosphorus metabolism in sufferers, which is worthy of clinical application.

    Abstract As one of the three main courses from primary school to senior high school, improving the quality of English teaching in and out of class has become the top priority of colleges and universities. English knowledge points are complex, and domestic scholars have studied vocabulary knowledge and grammatical awareness from various perspectives, but there is still a lack of research on the correlation between vocabulary knowledge, grammatical awareness, and cloze test scores of senior high school students. Therefore, this paper carries out empirical research from the depth of English vocabulary, English grammar, reading comprehension, cloze test, and composition, aiming at exploring the relationship between teaching quality and English knowledge points. Classroom teaching quality evaluation is the basic content of education quality evaluation, which not only needs to evaluate the effect of class hours but also needs a student's long-term learning effect. In order to improve the quality of classroom English teaching, enrich the content of classroom English, and ultimately make the quality of students' learning to a higher level, the combination model of the teaching quality evaluation index is established by combining the decision tree with the knowledge point rule association method and the evaluation results are verified by association rule analysis. This paper selects the effective indicators that affect the evaluation of English teaching quality, determines the weight of the indicators by using the analytic hierarchy process, effectively constructs the combination model of the decision tree and rule association method, and establishes the evaluation model of students' English learning ability in the classroom. Taking students as the main object and combining them with the requirements of digital teaching, the evaluation index system is formed, the index weight is determined by using the analytic hierarchy process, and the classroom teaching quality evaluation model based on the decision tree and English knowledge point correlation analysis is constructed to truly reflect the teaching level of teachers, and the correlation analysis is carried out between English teachers' own quality and students' learning effects and knowledge points. By testing the model performance of English vocabulary depth, reading comprehension, grammar, writing, and other knowledge points, we can well evaluate and analyze students' mastery of English learning and the correlation of English knowledge points.

    Abstract The definition of epigenetics refers that molecular modifications on DNA that can regulate gene activity are independent of DNA sequence and mitotically stable. Notably, epigenetics studies have grown exponentially in the past few years. Recent progresses that lead to exciting discoveries and groundbreaking nature of this area demand thorough methodologies and advanced technologies to move epigenetics to the forefront of molecular biology. The most recognized epigenetic regulations are DNA methylation, histone modifications, and non-coding RNAs (ncRNAs). This review will discuss the modern techniques that are available to detect locus-specific and genome-wide changes for all epigenetic codes. Furthermore, updated analysis of technologies, newly developed methods, recent breakthroughs and bioinformatics pipelines in epigenetic analysis will be presented. These methods, as well as many others presented in this specific issue, provide comprehensive guidelines in the area of epigenetics that facilitate further developments in this promising and rapidly developing field.

    Abstract The detection and classification of bone marrow (BM) cells is a critical cornerstone for hematology diagnosis. However, the low accuracy caused by few BM-cell data samples, subtle difference between classes, and small target size, pathologists still need to perform thousands of manual identifications daily. To address the above issues, we propose an improved BM-cell-detection algorithm in this paper, called YOLOv7-CTA. Firstly, to enhance the model's sensitivity to fine-grained features, we design a new module called CoTLAN in the backbone network to enable the model to perform long-term modeling between target feature information. Then, in order to cooperate with the CoTLAN module to pay more attention to the features in the area to be detected, we integrate the coordinate attention (CoordAtt) module between the CoTLAN modules to improve the model's attention to small target features. Finally, we cluster the target boxes of the BM cell dataset based on K-means++ to generate more suitable anchor boxes, which accelerates the convergence of the improved model. In addition, in order to solve the imbalance between positive and negative samples in BM-cell pictures, we use the Focal loss function to replace the multi-class cross entropy. Experimental results demonstrate that the best mean average precision (mAP) of the proposed model reaches 88.6%, which is an improvement of 12.9%, 8.3%, and 6.7% compared with that of the Faster R-CNN model, YOLOv5l model, and YOLOv7 model, respectively. This verifies the effectiveness and superiority of the YOLOv7-CTA model in BM-cell-detection tasks.

    Abstract Background: Infectious mononucleosis (IM) is characterized by pharyngitis, cervical lymphadenopathy, fatigue and fever. IM is most commonly seen in primary Epstein-Barr virus (EBV) infection, with higher occurrence in children.
    Objective: To explore the value of gamma globulin combined with acyclovir for IM children and their impact on immune function.
    Methods: This prospective randomized controlled study recruited 111 children under 14 years old with IM from Anhui Provincial Children's Hospital during March 2019 and March 2022. Among them, 11 children dropped out, and 100 eligible children were randomized 1:1 into a control group and a study group. The control group received acyclovir, and the study group received additional gamma globulin. The baseline data, clinical efficacy, immune function, and adverse reactions were collected and compared.
    Results: The study group had a shorter antipyretic time, lymph node reduction time, pharyngitis improvement time, and hospital stay compared to the control group (P < 0.05). The study group yielded lower levels of total white blood cell count, alanine aminotransferase, and creatine kinase-MB than the control group (P < 0.05). After treatment, the levels of CD3+ and CD8+ were lower, and the levels of CD4+, CD4+/CD8+, IgA, and IgG were higher in the study group than those in the control group (all P < 0.05). The incidence of adverse reactions between the two groups was comparable (14.00% vs. 24.00%). The positive rates of EBV-specific antibody and nuclear antigen in the study group were lower than those in the control group (P < 0.05).
    Conclusion: The combined treatment of gamma globulin and acyclovir is a promising alternative for patients with IM compared to acyclovir alone. This combined regimen shortens the duration of clinical manifestations in children, promotes the recovery of laboratory indices, improves clinical efficacy, and enhances immune function. Furthermore, its safety profile is acceptable, warranting its further promotion.

    Abstract Objective: To explore the application value of intravenous acyclovir in children with infectious mononucleosis (IM) and its effects on immune function.
    Methods: The data of 136 children with IM treated in Anhui Provincial Children's Hospital from March 2019 to March 2022 were retrospectively analyzed. According to the inclusion and exclusion criteria, 98 children were selected. Among them, 45 children treated with routine ribavirin were assigned to the control group, and the other 53 children treated with intravenous acyclovir were enrolled into the observation group. The two groups were compared in terms of efficacy, incidence of adverse reactions, recovery time of clinical symptoms, and immune function indexes, IgG, IgA, IgM, white blood cell (WBC) count and lymphocyte proportion, before and 10 days after the treatment. Independent risk factors affecting efficacy were analyzed by multivariate logistic regression analysis.
    Results: The observation group showed a significantly higher overall response rate than the control group (P=0.025). The control group experienced significantly longer recovery time of body temperature returning to normal, cure time of isthmitis, time for lymph node reduction, and alleviation time of hepatomegaly than the observation group (P<0.05). Additionally, the control group presented with a significantly higher incidence of adverse reactions than the observation group (P=0.028). After treatment, the observation group showed significantly lower levels of IgG, IgA, IgM, WBC count and lymphocyte proportion than the control group (all P<0.010). Longer average course of disease (OR: 1.449, 95% CI: 1.095-1.918), higher admission temperature (OR: 6.996, 95% CI: 1.350-36.257), higher admission IgA level (OR: 4.735, 95% CI: 1.357-16.520) and higher admission IgG level (OR: 1.470, 95% CI: 1.012-2.134) were independent risk factors for ineffective efficacy, while acyclovir (OR: 0.058, 95% CI: 0.005-0.729) was an independent protective factor.
    Conclusion: In the treatment of IM, intravenous acyclovir can substantially improve the overall clinical response rate for patients, with less adverse reactions, and can greatly alleviate various clinical symptoms and signs including fever, isthmitis, cervical lymph node enlargement, and hepatosplenomegaly, with obvious regulating effects on the immune function, so it is worth popularizing and applying in clinical practice.

    Abstract Central nervous system (CNS) diseases are the leading cause of death worldwide. By performing compensatory functions and improving the inflammatory microenvironment, the transplantation of neural stem cells (NSCs) can promote functional recovery from brain injury, aging, brain tumours, and other diseases. However, the ability of NSCs to differentiate into neurons is limited, and they are associated with a risk of tumourigenicity. NSC-derived extracellular vesicles (NSC-EVs) can modulate the local microenvironment of the nervous system as well as distant neuronal functions. Thus, cell-free therapy may be a novel remedy for CNS disorders. This article reviews the characteristics, contents, and mechanisms of action of NSC-EVs as well as their roles and application prospects in various CNS diseases.
