    Abstract Alopecia (loss of volume or complete loss of hair) can be a symptom of many diseases. In some cases it is a symptom of a local pathology limited to the scalp, in others it is a dermatological symptom of a systemic pathology. In almost every case, alopecia has a significant impact on the quality of life and may cause limitations in professional, social and private life. The management of alopecia is based on a preceding comprehensive medical differential diagnosis, which significantly exceeds basic dermatological diagnostic procedures. This article presents the basic information on therapeutic methods that can be used in selected diseases associated with non-cicatricial alopecia: androgenetic alopecia, senile alopecia, anagen effluvium, telogen effluvium, alopecia areata, non-cicatricial alopecia secondary to systemic lupus erythematosus, trichotillomania, traction alopecia, pressure alopecia, lipedematous scalp and lipedematous alopecia.

    Abstract Minoxidil is a drug with a hypotensive effect. It was introduced in the 1970s in the treatment of hypertension. This drug is characterized by a multifaceted mechanism of action – including a vasodilatory effect, probably through the activation of the ATP-sensitive potassium channels in vascular smooth muscle cells. Hypertrichosis was observed in patients treated with minoxidil, which prompted research into the possibility of applying this drug to diseases associated with alopecia. The first attempts were unsuccessful due to side effects associated with its hypotensive effect. Introduction of local treatment, and in recent years also methods of oral treatment with low doses of minoxidil, has allowed it to achieve a good therapeutic effect without a significant risk of side effects. In dermatology, the effect of minoxidil associated with stimulation of the onset and prolongation of the anagen phase is used. Minoxidil is available in the form of 2% and 5% solutions, 5% foam and in the form of compound drug for either topical or oral treatment. Side effects are present in 15–20% of patients and in most cases, they do not lead to discontinuation of the treatment. The most common side effects of topical preparations include itching, erythema, and scaling, while in the oral form, hypertrichosis of the facial skin, dizziness, and swelling of the lower limbs are most often observed.

    Abstract Frontal fibrosing alopecia is a variant of lichen planus, leading to permanent hair loss from the frontal hairline. It occurs mainly in postmenopausal women, however, the role of hormonal factors in the pathogenesis of this type of alopecia has not been determined. The coexistence of frontal fibrosing alopecia and autoimmune diseases is observed, e.g., hypothyroidism, rheumatoid arthritis, vitiligo, polymyositis, systemic lupus erythematosus and discoid lupus erythematosus. Considering the possible role of genetic factors, research is underway to identify human leukocyte antigen (HLA) polymorphisms. One of the most studied factors is sunscreen, widely used in cosmetic products. The data available so far in this regard are controversial. Factors raising significant doubts include: the retrospective nature of the research, basing the collected data on the self-assessment of respondents and the lack of specification of the ingredients of these products. In the light of current data, sunscreen components cannot be considered as a factor playing a role in the pathogenesis of frontal fibrosing alopecia. Overestimation of the role of sunscreens in hair loss could lead to unjustified avoidance of their use and contribute to increased incidence of skin cancer.

    Abstract Background: Systemic sclerosis (SSc) is a connective tissue disease manifesting with progressive fibrosis of the skin and internal organs. Its pathogenesis is strictly associated with vascular disfunction and damage. Salusin-α and salusin-β, endogenous peptides regulating secretion of pro-inflammatory cytokines and vascular smooth muscle proliferation, may potentially play a role in SSc pathogenesis. Objectives: The aim of this study was to assess the concentration of salusins in sera of patients with SSc and healthy controls and to evaluate correlations between the salusins levels and selected clinical parameters within the study group. Materials and methods: 48 patients with SSc (44 women; mean age, 56.4, standard deviation, 11.4) and 25 adult healthy volunteers (25 women; mean age, 55.2, standard deviation, 11.2) were enrolled. All patients with SSc were treated with vasodilators and twenty-seven of them (56%) also received immunosuppressive therapy. Results: Circulating salusin-α was significantly elevated in patients with SSc in comparison to healthy controls (U = 350.5, p = 0.004). Patients with SSc receiving immunosuppression had higher serum salusin-α concentrations compared with those without immunosuppressive therapy (U = 176.0, p = 0.026). No correlation was observed between salusins concentrations and skin or internal organ involvement parameters. Conclusions: Salusin-α, a bioactive peptide mitigating the endothelial disfunction, was elevated in patients with systemic sclerosis receiving vasodilators and immunosuppressants. Increased salusin-α concertation may be associated with the initiation of atheroprotective processes in patients with SSc managed pharmacologically, which requires verification in future studies.
