    Abstract Objectives: Few studies on ischemic complications and flow changes after a flow diverter covering the anterior cerebral artery. The purpose of the study was to explore the ischemic complications and anatomical alterations associated with the flow diverter after it covers the anterior cerebral artery.
    Materials and methods: In this single-center study, patients treated with FD covering the anterior cerebral artery at the First Affiliated Hospital of Zhengzhou University were retrospectively collected. The primary endpoint was ischemic complications related to the anterior cerebral artery. Secondary endpoints were anatomical changes in the anterior cerebral artery postoperatively and at follow-up.
    Results: A total of 59 patients were included in this study. Four (6.8%) patients presented with ischemic stroke symptoms. Immediately after the procedure, complete occlusion of A1 and decreased blood flow was observed in 13 (22.0%) and 21 patients (35.6%), respectively. At follow-up, A1 artery was occluded in 34 patients (57.6%) and decreased blood flow was observed in 10 patients (16.9%). Symptoms of neurological deficits related to the anterior cerebral artery were not observed in all patients at follow-up.
    Conclusion: Coverage of A1 is safe, with a low incidence of ischemic stroke, when using an FD to treat aneurysms. Risk of reduced perfusion of the anterior cerebral artery postoperatively even if the anterior communicating artery is open. In cases with A1 occlusion, the blood flow in the distal the anterior cerebral artery can be adequately compensated by opening the anterior communicating artery and good vascular anastomoses.

    Abstract We start this article with a discussion of the problematics involved in translating into modern English a (modern) Chinese text concerning a classical Chinese notion, namely qíng 情. Then, we suggest that it is necessary to distinguish between two levels on which a language is used. The first is the manifest level, where one finds family resemblance between, say, Chinese nù 怒 and English “anger”. The second is the generic level, where one finds qíng in classical Chinese, qínggaˇn in modern Chinese, and emotion(s) in English. We argue that the meanings of words at the generic level can only be accessed via the manifest level. It is misleading to directly identify and compare notions at the generic level (in this case, emotion(s) in English and qíng in Chinese). We call the connections at the manifest level “quasi-universals”, and we refer to the notions at the generic level as “embedding conceptual clusters”.

    Abstract Abstract Background Supraspinatus tendon reconstruction (STR) was recently introduced as a new treatment option for irreparable posterosuperior massive rotator cuff tears (IPMRCT). STR was thought to be more advantageous than superior capsule reconstruction (SCR) for restoring supraspinatus (SSP) dynamics. However, there has been no prospective randomized controlled study on the early clinical efficacy of STR. Methods A single-site, prospective, observers and patients double-blinding randomized controlled trial was designed. Fifty-eight patients aged 50–85 years with IPMRCT will be randomized 1:1 to receive either STR or SCR. The clinical outcomes were evaluated using the American Society for Shoulder and Elbow Surgery (ASES) score, range of motion (ROM), visual analogue scale (VAS) for pain, acromiohumeral distance (AHD), Goutlliar grade for fatty infiltration in the SSP, Sugaya grade for the autogenous fascia latas, isokinetic muscle strength testing and surface electromyography (EMG) testing for shoulder abduction muscle strength and complications. Discussion The results of this study will contribute to the treatment algorithm of IPMRCT and assist surgeons in making treatment decisions. This is the first randomized controlled trial to compare the effects of STR and SCR for the treatment of IPMRCT. Trial registration We registered the trial in chictr.org.cn on July 17, 2023 (register number: ChiCTR2300073716). Items from the WHO trial registry were found within the protocol.

    Abstract Abstract The attention mechanism empowers deep learning to a broader range of applications, but the contribution of the attention module is highly controversial. Research on modern Hopfield networks indicates that the attention mechanism can also be used in shallow networks. Its automatic sample filtering facilitates instance extraction in Multiple Instances Learning tasks. Since the attention mechanism has a clear contribution and intuitive performance in shallow networks, this paper further investigates its optimization method based on the recurrent neural network. Through comprehensive comparison, we find that the Synergetic Neural Network has the advantage of more accurate and controllable convergences and revertible converging steps. Therefore, we design the Syn layer based on the Synergetic Neural Network and propose the novel invertible activation function as the forward and backward update formula for attention weights concentration or distraction. Experimental results show that our method outperforms other methods in all Multiple Instances Learning benchmark datasets. Concentration improves the robustness of the results, while distraction expands the instance observing space and yields better results. Codes available at https://github.com/wzh134/Syn .

    Abstract Abstract Deep learning’s performance on the imbalanced small data is substantially degraded by overfitting. Recurrent neural networks retain better performance in such tasks by constructing dynamical systems for robustness. Synergetic neural network (SNN), a synergetic-based recurrent neural network, has superiorities in eliminating recall errors and pseudo memories, but is subject to frequent association errors. Since the cause remains unclear, most subsequent studies use genetic algorithms to adjust parameters for better accuracy, which occupies the parameter optimization space and hinders task-oriented tuning. To solve the problem and promote SNN’s application capability, we propose the modern synergetic neural network (MSNN) model. MSNN solves the association error by correcting the state initialization method in the working process, liberating the parameter optimization space. In addition, MSNN optimizes the attention parameter of the network with the error backpropagation algorithm and the gradient bypass technique to allow the network to be trained jointly with other network layers. The self-learning of the attention parameter empowers the adaptation to the imbalanced sample size, further improving the classification performance. In 75 classification tasks of small UC Irvine Machine Learning Datasets, the average rank of the MSNN achieves the best result compared to 187 neural and non-neural network machine learning methods.

    Abstract Background: Several dermoscopic features of juvenile xanthogranuloma (JXG) have been previously described in single cases or small case series and need to be further verified in a large sample.Objective: We aimed to investigate the dermoscopic patterns of JXG in a large case series and the correlations of these with clinical features of different histopathological subtypes of JXG.Methods: Patients who underwent dermoscopic evaluation and had a histopathological diagnosis of JXG were recruited. Histological findings, including stage and Ki67 proliferative index and the dermoscopic features of each lesion were recorded.Results: Forty-one patients with JXG were included. The male to female ratio was 1.28: 1 and the median age of onset was 11 months (range: 0–95 months). Fourteen lesions were histologically categorized in the early stage, 17 in the developed stage, and 10 in the late stage. The “setting sun” pattern was observed in 35 lesions (85.4%) and “clouds” of paler yellow areas in 26 lesions (63.4%). The frequency of the “setting sun” pattern was higher in the early and developed stages (30/31) than in the late stage (5/10) (P = 0.002), while that of “clouds” of paler yellow areas was not significantly different between each stage. Branched linear vessels were detected in the early (11/14) and developed stage (6/17), but not in the late stage. The mean Ki67 index of the lesions with linear vessels was 11.8% (range: 2–40%), which was higher than that of lesions without linear vessels (mean index: 5%, range: 1–30%) (P = 0.005). The pigment network and whitish areas were only detected in 6 and 5 lesions in the late stage, respectively. The whitish areas presented either as streak or stellate shape. The pigment network exhibited either in a centric or a peripheral pattern.Conclusions: The “setting sun” pattern is the characteristic dermoscopic features of JXG in the early and developed stages, while whitish areas and pigment network are the characteristic patterns in the late stage.     Abstract In the practical water resources management, the allowable thresholds of pollutants are not unique. However, the conventional grey water footprint (GWF) model cannot deal with this uncertainty in the controlling threshold. To solve this problem, an improved GWF model and pollution risk evaluation method is designed according to the uncertainty analysis theory and maximum entropy principle. In this model, GWF is defined as the mathematical expectation of virtual water to dilute the pollution load within the allowable threshold, and the pollution risk is deduced by the stochastic probability by which GWF exceeds the local water resources. And then, the improved GWF model is applied in the pollution evaluation of Jiangxi Province, China.     Abstract Lipase-catalyzed transesterification is a promising and sustainable approach to producing biodiesel. To achieve highly efficient conversion of heterogeneous oils, combining the specificities and advantages of different lipases is an attractive strategy. To this end, highly active Thermomyces lanuginosus lipase (1,3-specific) and stable Burkholderia cepacia lipase (non-specific) were covalently co-immobilized on 3-glycidyloxypropyltrimethoxysilane (3-GPTMS) modified Fe 3 O 4 magnetic nanoparticles (co-BCL-TLL@Fe 3 O 4 ). The co-immobilization process was optimized using response surface methodology (RSM). The obtained co-BCL-TLL@Fe 3 O 4 exhibited a significant improvement in activity and reaction rate compared with mono and combined-use lipases, achieving 92.9% yield after 6 h under optimal conditions, while individually immobilized TLL, immobilized BCL and their combinations exhibited yields of 63.3%, 74.2% and 70.6%, respectively. Notably, co-BCL-TLL@Fe 3 O 4 achieved 90–98% biodiesel yields after 12 h using six different feedstocks, demonstrating the perfect synergistic effect of BCL and TLL remarkably motivated in co-immobilization. Furthermore, co-BCL-TLL@Fe 3 O 4 could maintain 77% of initial activity after nine cycles by removing methanol and glycerol from catalyst surface, accomplished by washing with t -butanol. The high catalytic efficiency, wide substrate adaptability and favorable reusability of co-BCL-TLL@Fe 3 O 4 suggest that it will be an economical and effective biocatalyst for further applications.

    Abstract Boron nitride nanosheets (BN NSs) have emerged as promising materials in a wide range of biomedical applications. Despite the extensive studies on these bio-nano interfacial systems, one critical concern is their toxicity, which is affected by a variety of factors, including size. This study aimed at assessing the relationship between BN NSs size and toxicity. Two silkworm strains (qiufeng × baiyu and Nistari 7019) were used as model organisms to investigate the effect of different sizes of BN NSs (BN NSs-1, thickness of 41.5 nm and diameter of 270.7 nm; BN NSs-2, thickness of 48.2 nm and diameter of 562.2 nm) on silkworm mortality, growth, cocoon weight, and tissue microstructure. The findings show that exposure to BN NSs in this work has no lethal adverse effects on silkworm growth or tissue microstructure. BN NSs have a higher effect on the growth rate of qiufeng × baiyu compared to Nistari 7019, demonstrating that the same treatment does not favorably affect the Nistari 7019 strain, as there is no significant increase in cocoon weight. Overall, the study suggests that the sizes of BN NSs employed in this study are relatively safe and have less negative impact on silkworms. This offers significant insights into the effect of BN NSs size, a crucial factor to consider for their safe use in biomedical applications.
