    Abstract Objective: To analyze misdiagnosis features in clinical cases of “Classified Medical Cases of Famous Physicians” and “Supplement to Classified Case Records of Celebrated Physicians.” Materials and Methods: Two hundred and five ancient misdiagnosed cases were analyzed in aspects of locations (exterior-interior type, qi-blood type and Zang-Fu organs type) and patterns (heat-cold type and deficiency-excess type) by Apriori Algorithm Method. Results: The main types of misdiagnosis in those medical casesare as follows:: Zang-Fu location misjudgment, misjudging the interior as the exterior, misjudging deficiency pattern as excess pattern, and misjudging cold pattern as heat pattern. Among them, the most outstanding type is the misjudgment of deficiency–cold pattern as excess–heat pattern. Conclusions: (1) Accurate judgment of location and differentiation of deficiency and excess patterns are the key points in diagnosing the diseases correctly. The confusion of true deficiency–cold and pseudo-excess–heat pattern should be taken seriously. (2) Data mining on ancient clinical cases offers a new methodology for assisting clinical diagnosis of traditional Chinese medicine.

    Abstract Background: In 2018, 84 615 patients in Taiwan received hemodialysis, of which about 62% to 97% suffered from fatigue. Fatigue caused by hemodialysis may be mental or physical. However, the detailed mechanism remains unclear. This study aimed to evaluate whether far-infrared stimulation of the Neiguan acupoint (P6) could effectively reduce fatigue and improve heart rate variability (HRV) in hemodialysis patients.
    Methods: We conducted a two-arm, randomized trial in a hemodialysis center between March 2015 and March 2016. A total of 73 hemodialysis patients were included and were randomly assigned to an experimental group (n = 37) and a control group (n = 36). The experimental group received far-infrared radiation on Neiguan acupoint during hemodialysis for 12 weeks. The outcomes were fatigue level and HRV.
    Results: All patients showed moderate fatigue ( M = 26.00 ± 13.01, range = 0-78) at baseline. Far-infrared stimulation on Neiguan acupoint significantly reduced overall fatigue ( β = 24, p < 0.001) and improved HRV ( β = 74.36, p < 0.001). Compared to the control group, the experiment group had significantly reduced fatigue levels in all aspects such as reduced energy and motivation ( β = -2.97, p < 0.001), reduced physical strength ( β = -1.28, p < 0.01), reduced mental capability ( β = -2.38, p < 0.001), reduction in daily activities ( β = -1.48, p < 0.01), depressed mood and loss of control ( β = -1.21, p < 0.05) as well as increased autonomous nervous system activity ( β = 14.71, p < 0.01) in the third month of stimulation.
    Conclusion: Far-infrared stimulation of the Neiguan acupoints effectively reduces fatigue and increases autonomic nervous system activity in hemodialysis patients.

    Abstract Atopic dermatitis (AD) is a chronic, recurrent inflammatory skin disease with a wide range of heterogeneity. Accurate biomarkers or predictors are the keys to instructing personalized tailored precise treatment. The development of technology such as transcriptomics, genomics, and proteomics provides novel insights into the possibility to find potential biomarkers. Meanwhile, emerging minimally invasive methods such as tape stripping were used to reveal different profiles of patients’ skin without biopsy. Several potential biomarkers or predictors have been found. In this review, we summarized the current development of potential biomarkers of AD. Nitric oxide synthase 2/inducible nitric oxide synthase (NOS2/iNOS), human beta-defensin-2 (hBD-2), and matrix metalloproteinases 8/9 (MMP8/9) may be the candidate biomarkers for AD diagnosis. Filaggrin (FLG) gene mutation increased the occurrence risk of AD. Fatty-acid-binding protein 5 (FABP5) may serve as an effective biomarker for the atopic march (AM). Squamous cell carcinoma antigen 2 (SCCA2), serum thymus and activation-regulated chemokine (TARC), cutaneous T-cell-attracting chemokine (CTACK), eosinophil-derived neurotoxin (EDN), macrophage-derived chemokine (MDC), lactate dehydrogenase (LDH), and interleukin (IL)-18 can be the candidate biomarkers for disease severity monitoring. IL-17, IL-23, IL-33, and indoleamine 2,3-dioxygenase (IDO1) can be used as predictive biomarkers for AD comorbidities. LDH, TARC, pulmonary and activation-regulated chemokine (PARC), periostin, IL-22, eotaxin-1/3, and IL-8 may be the candidate biomarkers for monitoring treatment effects. There are still unmet needs and a long way to go for more convenient, non-invasive, and effective predictors and biomarkers to better guide personalized precise treatment.

    Abstract Hepatocellular carcinoma (HCC) is one of the leading causes of cancer-related deaths in the world. Metabolic reprogramming is considered a new hallmark of cancer, but it remains unclearly described in HCC. The dysregulation of the PI3K/AKT/mTOR signaling pathway is common in HCC and is, therefore, a topic of further research and the concern of developing a novel target for liver cancer therapy. In this review, we illustrate mechanisms by which this signaling network is accountable for regulating HCC cellular metabolism, including glucose metabolism, lipid metabolism, amino acid metabolism, pyrimidine metabolism, and oxidative metabolism, and summarize the ongoing clinical trials based on the inhibition of the PI3K/AKT/mTOR pathway in HCC.

    Abstract The prediction of pause fillers plays a crucial role in enhancing the naturalness of synthesized speech. In recent years, neural networks, including LSTM, BERT, and XLNet, have been employed for pause fillers prediction modules. However, these methods have exhibited relatively lower accuracy in predicting pause fillers. This paper introduces the utilization of the RoBERTa model for predicting Chinese pause fillers and presents a novel approach to training the RoBERTa model, effectively enhancing the accuracy of Chinese pause fillers prediction. Our proposed approach involves categorizing text from different speakers into four distinct style groups based on the frequency and position of Chinese pause fillers. The RoBERTa model is trained on these four groups of data, which incorporate different styles of fillers, thereby ensuring a more natural synthesis of speech. The Chinese pause fillers prediction module is evaluated on systems such as Parallel Tacotron2, FastPitch, and Deep Voice3, achieving a notable 26.7% improvement in word-level prediction accuracy compared to the BERT model, along with a 14% enhancement in position-level prediction accuracy. This substantial improvement results in a significant enhancement of the naturalness of the generated speech.

    Abstract Background: There are many different approaches to umbilical cord care in clinical practice. Dry care is recommended by the WHO based on its ease of implementation and relatively low cost. However, the effect of dry care on the time of umbilical cord separation and related complications require further study.
    Purpose: The aim of this study was to conduct a systematic review and meta-analysis of randomized controlled trials to evaluate the effect of dry care on the time of umbilical cord separation and related adverse events in newborns.
    Methods: A literature search was conducted of articles published before December 2021 in the Cochrane Library, PubMed, Medline, CINAHL, Embase, Airiti Library, and National Digital Library of Theses in Taiwan databases. The research quality of the identified articles was assessed using the Modified Jadad Scale. Statistical analyses were performed using Review Manager, version 5.3, with statistics reported as mean differences with 95% confidence intervals.
    Results: Seven randomized controlled trials covering a total of 27,037 newborns were identified for analysis. Umbilical cord care in these studies included the following approaches: dry care, alcohol, chlorhexidine (CHX), mother's milk, and salicylic sugar powder (SSP). The results of the meta-analysis showed that umbilical cord separation time was significantly lower for the dry care approach compared to the alcohol approach (MD: -1.78 days, 95% CI: -2.4 to -1.16, three studies, n = 2,102) but not significantly different than the CHX approach (MD: -0.15 days, 95% CI [-1.99, 1.69], two studies, n = 10,519). Notably, the umbilical cord separation time for both mother's milk (MD: 1.19 days, 95% CI [0.82, 1.56], three studies, n = 730) and SSP (MD: 4.9 days, 95% CI [3.71, 6.09], one study, n = 92) approaches were reported as significantly less than the dry care approach. In addition, the CHX approach was associated with significantly fewer cord-related adverse events, while the alcohol, mother's milk, and SSP approaches were associated with rates of cord-related adverse advents similar to dry care.
    Conclusions / implications for practice: Because using dry care for umbilical cord care shortens the cord separation time compared to the alcohol approach and does not increase the incidence of omphalitis, this approach should be the first choice for cord care. Both mother's milk and SSP approaches are also associated with shorter umbilical cord separation times without adverse effects, and may be used as alternatives to dry care in umbilical cord care.

    Abstract Purpose: The study examined the use of percent grammatical utterances (PGUs) for assessing grammatical skills in Mandarin-speaking 3-year-old children.
    Method: Participants were 30 Mandarin-speaking 3-year-olds with typical development. Language samples were collected in two visits for each child using a picture description task. Children were asked to talk about 16 pictures in response to questions and prompts at each visit. Pictures for the language sample collection were identical across the visits. PGUs were computed, and the grammatical errors that children produced in the task were coded and tallied for error types at each visit. Test-retest reliability, split-half reliability, and concurrent criterion validity of PGUs were evaluated.
    Results: The mean PGU level was approximately 78% at Visit 1 and 81% at Visit 2, both of which were significantly below the mastery level (i.e., 90%). The correlation coefficient for test-retest reliability of PGU was large (
    Conclusion: The initial evidence from psychometric properties suggests that PGU computed from the picture description task is a reliable and valid measure for evaluating grammatical skills in Mandarin-speaking 3-year-old children.

    Abstract To observe the effect of exercise intervention and nutrition health education on body composition and fitness of female college students with invisible obesity. 60 female college students with invisible obesity were randomly divided into three groups: control group, exercise intervention group and exercise + nutrition health education group. The control group had normal study and life; the exercise intervention group used aerobic exercise and resistance exercise to carry out collective exercise prescription exercise and individual contract exercise; the exercise intervention + nutrition health education group carried out nutrition health education on the basis of exercise intervention. Before the experiment, the three groups of female college students were homogenous (P > 0.05); after 16 weeks of the experiment, the body composition and fitness of the exercise intervention group and the exercise intervention + nutrition and health education group were significantly higher than that before the experiment, among which there were significant differences in BMI, BF%, VFI, vital capacity, 800m, sit forward flexion and sit up indexes (P < 0.05); after 8 weeks of training, the exercise intervention group was significantly higher than that before the experiment (P < 0.05) The BMI, BF% and VFI of the exercise intervention + nutrition health education group continued to decrease (P < 0.05).     Abstract To explore the effect of prescription of traditional sports regimen with different amounts of exercise on improving depression and sleep quality of college students. Depression Screening Scale (PHQ-9) and Zung’s Self-rating Depression Scale (SDS) were used to screen students with mild depression, who were randomly divided into control group, experimental group 1 (moderate exercise intensity, 30min each time, 5 times a week,) and experimental group 2 (moderate exercise intensity, 60min each time, 5 times a week). The exercise prescription intervention with different exercise amount was performed for 16 weeks. Compared with before exercise, the total scores of SDS and PSQI of the two experimental groups decreased significantly after 8 weeks of intervention; after 16 weeks of intervention, the total scores of SDS and PSQI of the experimental groups were obviously different (P < 0.05) compared with 8 weeks of intervention; after 8 weeks of training cessation, the total scores of SDS and PSQI of the experimental groups were increased, but however, significant decrease in the total scores still can be seen compared with those before the experiment(P<0.05). Compared with the control group, after 8 weeks of intervention, 16 weeks of intervention and 8 weeks of training cessation, the total scores of SDS and PSQI of the two experimental groups were significantly reduced and the effect of experimental group 2 on improving PSQI score of college students was significantly better than that of experimental group 1.Traditional sports regimen intervention can improve college students’ depression and improve their sleep quality, and it can still maintain the effect after stopping training for 8 weeks; exercise intervention of 60 minutes each time, 5 times a week, has better effect on improving college students’ sleep quality than that of 30 minutes each time, 5 times a week.
