Abstract China's healthcare system has made great achievements in the management of medical services and public health challenges for the Chinese people. However, the issue of an aging population in Chinese society is becoming more and more salient. The gap between demand and supply of healthcare is expanding. China's healthcare system is facing unprecedented challenges. These problems include an insufficient medical insurance fund, nonuniform insurance reimbursement policies, a poor integrity system, and a lack of supervision in the management of the medical insurance fund. To address these challenges, some practical solutions are worth considering. A national medical insurance supervision platform should be strengthened. Besides, blacklists for illegal medical institutions and individuals engaged in malicious medical disturbances should be created. The country should also introduce policies to narrow the differences in regional medical insurance policies and balance the reimbursement levels of residents in different regions. Big data and artificial intelligence can be used to monitor the entire process of medical insurance fund utilization. The government needs to establish relevant laws and regulations to optimize the medical insurance system and ensure the safe and effective operation of the medical insurance fund.

Abstract The existing real-time monitoring system of soil temperature in underground buildings has the defect of poor monitoring accuracy. In order to solve the above problems, the real-time monitoring system of soil temperature in underground buildings is designed based on arm. The hardware of the real-time monitoring system for the soil temperature of underground building reclamation includes the temperature acquisition unit, arm processing unit and wireless transmission unit, and the software includes the upper computer monitoring module and the lower computer program module. Through the design of the hardware and software of the system, the operation of the real-time monitoring system for the soil temperature of underground building reclamation based on arm is realized. The experimental results show that compared with the existing real-time monitoring system, the designed real-time monitoring system based on ARM greatly improves the monitoring accuracy, which fully shows that the proposed real-time monitoring system based on arm has better monitoring performance.

Abstract Different from the linear counterpart, elastic wave propagation in nonlinear elastic metamaterials with damping (EMD) systems has much more novel behaviors. It is little work that reveals the nonlinear wave propagation mechanism in the nonlinear EMD in the literature. In this paper, nonlinear EMD systems with different configurations for the nonlinear elements are constructed to study the nonlinear wave propagation characteristics based on the analytical formulation. Nonlinear wave propagation attenuation characteristics in nonlinear EMD systems are studied in the frequency-domain. Spectro-spatial characteristics of nonlinear traveling wave packet in space- and time-domain are also analyzed to reveal space frequency-domain characteristics. Influences of different configurations for the nonlinear elements on wave propagation in nonlinear EMD systems are discussed. Results shown that locations of band structures can be tuned by changing intensities of nonlinearities in the nonlinear EMD systems, which are not found in the linear system. Moreover, different configurations for the nonlinear elements can lead to localization feature emerged in a nonlinear EMD system, which indicates solitary waves causing from the interaction between dispersion and nonlinearity exist in the nonlinear systems. This interesting wave propagating properties can be used to design new devices for acoustic switches, energy harvesting, and broadband vibration control.

Abstract Objective: Aptamers are increasingly applied in cancer research. Here, we have performed the first bibliometric analysis to demonstrate the evolution of aptamers in cancer research over the past decade and to reveal future trends.
Method: Original articles and reviews on aptamers in cancer research published from 2013 to 2022 were retrieved from the Web of Science Core Collection database. VOSviewer, CiteSpace, and R software were used for bibliometric analysis of the literature and visualization of the results.
Results: A total of 1627 eligible publications were analyzed. Annual and cumulative publications have been found to be steadily increased. China was the most productive country (884 publications) and Hunan University was the most productive institution (97 publications). The United States had the highest level of international collaboration (betweenness centrality = 0.55). Wei-Hong Tan was the most productive author (68 publications) and Craig Tuerk was the most cited author (387 citations). Analytical Chemistry and Biosensors and Bioelectronics were the most influential journals in this field. Three major themes were identified: aptamer selection techniques, aptamer-targeted drug delivery, and aptasensors for cancer detection. The research hotspots have shifted from aptamer selection, targeted drug delivery, molecular imaging, and biomarker detection to electrochemical aptasensors and therapeutic applications. The future may focus on high specificity and affinity in aptamer selection, aptasensor fabrication, aptamer- targeted drug delivery, and therapeutic aptamer development.
Conclusion: The field of aptamers in cancer research has been steadily developing over the past decade, and future research may focus on aptamer application in cancer detection and therapy and the improvement of aptamer selection.

Abstract Cryptococcal meningitis (CM) is a fatal fungal central nervous system (CNS) infection caused by Cryptococcus infecting the meninges and/or brain parenchyma, with fever, headache, neck stiffness, and visual disturbances as the primary clinical manifestations. Immunocompromised individuals with human immunodeficiency virus (HIV) infection or who have undergone organ transplantation, as well as immunocompetent people can both be susceptible to CM. Without treatment, patients with CM may have a mortality rate of up to 100% after hospital admission. Even after receiving therapy, CM patients may still suffer from problems such as difficulty to cure, poor prognosis, and high mortality. Therefore, timely and effective treatment is essential to improve the mortality and prognosis of CM patients. Currently, the clinical outcomes of CM are frequently unsatisfactory due to limited drug choices, severe adverse reactions, drug resistance, etc. Here, we review the research progress of CM treatment strategies and discuss the suitable options for managing CM, hoping to provide a reference for physicians to select the most appropriate treatment regimens for CM patients.

Title translation Cambios dinámicos y significado clínico de la HMGB1 sérica en pacientes con artroplastia total de cadera.
Abstract Background: Deeper understanding on the risk factors and seeking potential predicted biomarkers for prognosis of total hip arthroplasty (THA) patients are of great significance. Limited researches focused the correlation between high mobility group box protein-1 (HMGB1) and the prognosis of THA patients.
Objective: The objective of this study was to investigate the role of HMGB1 and inflammatory factors in patients underwent total hip arthroplasty (THA).
Methods: The present prospective study enrolled 208 THA patients who went to our hospital during January 2020 to January 2022. Serum levels of HMGB1, C-reactive protein (CRP), interleukin-1b (IL-1β), and IL-6 were detected at the admission, 1 day, 3 days, 7 days, 30 days, and 90 days after surgery. The levels of Harris score, Fugl-Meyer, 36-item short-form health survey (SF-36), and Pittsburgh sleep quality index (PSQI) were detected on 90 days after surgery in two groups. Receiver operating characteristic curve (ROC) was performed for analyzing the diagnostic value of HMGB1 and logistic regression model was used for identifying the risk factor for poor prognosis of THA patients.
Results: Serum levels of HMGB1 and inflammatory factors increased after surgery compared with the baselines. A positive correlation was found between HMGB1 and CRP on 1 day after surgery, and positive correlations were found among HMGB1, IL-1β, and IL-6 on 3 day after surgery. Besides, low HMGB1 reduced the incidence of post-operative complications and improved prognosis of THA patients.
Conclusion: Serum HMGB1 was correlated with inflammatory factors and the prognosis of THA patients.
