    Abstract Variations in transmembrane pressure and permeate flux are closely related to membrane fouling. In this study, a laboratory-scale submerged microfiltration system was used to investigate the influence of sodium alginate (SA) concentration and peristaltic pump rotation speed on the fouling under the conditions of (1) the same driving force and non-aerated-PAC, (2) different driving forces and non-aerated-PAC, and (3) different driving forces and aerated-PAC. The results showed that the normalized transmembrane pressure (TMP') increased linearly with decreasing normalized permeate flux (J') during the early microfiltration stage regardless of the operating conditions, indicating that the SA microfiltration process controlled by the peristaltic pump was non-constant-flux and non-constant-pressure. The latter filtration stage was considered constant-pressure filtration when 200-1,200 mg/L of SA was filtrated at the same rotation speed. During filtration of 800 mg/L of SA under the non-aerated-PAC condition, the later filtration stage was considered constant-pressure filtration when the peristaltic pump rotated at slower speeds of 15 and 30 rpm. This approached constant-flux filtration when the peristaltic pump rotated at faster speeds of 60 and 90 rpm, and PAC-aeration scouring was an effective measure for mitigating membrane fouling by SA.

    Abstract A laboratory-scale flat-sheet ceramic microfiltration membrane system was developed to investigate the membrane fouling behaviors and mechanisms of sodium alginate (SA) in the presence of aeration and powdered activated carbon (PAC). When the SA concentration increased from 20 to 500 mg/L, the permeate flux decreased by 81.7%, and the transmembrane pressure (TMP) and resistance increased 1.7 and 24.5 times, respectively. At an SA concentration of 500 mg/L, it was found that the membrane fouling tended to decrease with the increase in the aeration rate, indicating high control of the fouling by air scouring, while PAC-aeration scouring produced a significant improvement in the permeate flux with substantially reduced fouling. In the microfiltration of 500 mg/L SA at an air flow rate of 2.2 L/min and PAC concentrations of 40, 100, and 250 mg/L, the flux increased by 179.3%, 238.0%, and 302.7%, the TMP decreased by 32.6%, 34.8%, and 45.7%, and the cake and pore blocking resistance decreased by 78.0%, 85.1%, and 87.9%, respectively, compared to the corresponding values without PAC-aeration scouring. Intermediate blocking and complete blocking models were confirmed to elucidate the effect of aeration scouring and PAC-aeration scouring on the mitigation of membrane fouling by SA. PRACTITIONER POINTS: Air scouring was effective at mitigating membrane fouling of sodium alginate. The addition of PAC could alleviate membrane fouling of SA. Synergistic scouring by aeration and PAC offers a promising means for more-efficient and cost-effective control of membrane fouling. The fouling mechanisms in various scenarios were elucidated.

Based on Germany’s practical experience, this research offers a strategy idea for other countries with a social health insurance system, aiming to ensure that the health care system does not collapse rapidly due to medical resource shortage in the event of a pandemic. Firstly, this research conducted a documentary analysis to systematically collate the temporary and additional coverage measures provided by the health insurance fund from January to the end of July 2020, which sheds light on the pivotal role of these funds in epidemic prevention and control. Secondly, this research used comparative analysis to examine the time sequence of implementing these different types of coverage measures in the progression of the epidemic to illustrate how the health insurance fund adjusted its response measures. The health insurance fund was actively involved in the development of core strategies for combating the epidemic when it broke out, by taking part in joint multisectoral consultations. By using payment instruments flexibly, the fund led the implementation of epidemic prevention and control measures, as it could allocate health resources quickly and efficiently in emergencies. Furthermore, the health insurance fund played a critical role in transmitting information on the epidemic and guiding the insured to take appropriate protective measures.     Abstract Incremental computation aims to compute more efficiently on changed input by reusing previously computed results. We give a high-level overview of works on incremental computation, and highlight the essence underlying all of them, which we call incrementalization -- the discrete counterpart of differentiation in calculus. We review the gist of a systematic method for incrementalization, and a systematic method centered around it, called Iterate-Incrementalize-Implement, for program design and optimization, as well as algorithm design and optimization. At a meta-level, with historical contexts and for future directions, we stress the power of high-level data, control, and module abstractions in developing new and better algorithms and programs as well as their precise complexities.

    Abstract To reveal the importance of the participation of the health insurance fund in the prevention and control of serious infectious diseases, this research retrospectively analyzed the case of the German statutory health insurance fund in response to the COVID-19 epidemic.     Abstract Natural immunity, the first line for the body to defense against the invasion of pathogen, serves as the body's perception of the presence of pathogens depends on nucleic acid recognition mechanisms. The cyclic GMP-AMP synthase-stimulator of the interferon gene (cGAS-STING) signaling pathway is considered an essential pattern recognition and effector pathway in the natural immune system and is mainly responsible for recognizing DNA molecules present in the cytoplasm and activating downstream signaling pathways to generate type I interferons and some other inflammatory factors. STING, a crucial junction protein in the innate immune system, exerts an essential role in host resistance to external pathogen invasion. Also, STING, with the same character of inflammatory molecules, is inseparable from the body's inflammatory response. In particular, when the expression of STING is upregulated or its related signaling pathways are overactivated, the body may develop serious infectious disorders due to the generation of excessive inflammatory responses, non-infectious diseases, and autoimmune diseases. In recent years, accumulating studies indicated that the abnormal activation of the natural immune cGAS-STING signaling pathway modulated by the nucleic acid receptor cGAS closely associated with the development and occurrence of autoimmune diseases (AID). Thereof, to explore an in-depth role of STING and its related signaling pathways in the diseases associated with inflammation may be helpful to provide new avenues for the treatment of these diseases in the clinic. This article reviews the activation process of the cGAS-STING signaling pathways and its related important roles, and therapeutic drugs in AID, aiming to improve our understanding of AID and achieve better diagnosis and treatment of AID.

    Abstract After 75 years of reformed practice, general practitioners (GPs) in the UK have transformed from health gatekeepers who simply provide medical decision-making such as diagnostic and treatment services and referral services, to health agents who proactively provide more relevant health services such as immunizations, health monitoring and health management, etc. In order to discuss this transformation of the role of the general practitioner and the conditions for the evolution of the role, this study chose the documentary analysis method to provide a comprehensive overview of the legal and normative documents related to the general practitioner. Furthermore, this study uses a comparative analysis method to conclude the definition and role characteristics of GPs as health agents. This study summarises the general pattern of evolution of GPs into health agents. The transformation into a health agent relies on the interpersonal trust and rigorous institutional of society on the general practitioner system. The expansion of GPs' clientele and range of services, together with the motivation to proactively provide services, have combined to push for a "qualitative change" in the GP's role as health agent. The transformation of the role of the general practitioner to a health agent is a historical necessity. It responds to the evolution of society's understanding of health and the need for higher levels of health. Therefore, recognizing the role of GPs as health agents is important for optimizing the use of health care resources and improving the health of society by taking advantage of this role.
