Abstract Background: Although, sickle cell trait (SCT) was considered a benign condition with most patients being asymptomatic, however, there is an impressive increase in the number of sudden deaths in the military recruits and athletes, which has led to SCT as a rapidly emerging medical issue. Genetic factors have been extensively investigated in the etiopathogenesis of SCT but, environmental factors have not been studied in depth.
Aims: The main aim of this study was to investigate the effect of high temperature on the red blood cells (RBCs) in those with SCT and compare this to a control group lacking SCT.
Patients and methods: Heat stress to RBCs was induced by in vitro incubation of freshly drawn blood at high temperatures (45°C for 35 min). Additional information such as hemoglobin (Hb) level, RBC count, mean corpuscular volume (MCV), and hemoglobin S level was obtained from the medical record of the case and control groups. Data were entered in Statistical Package for Social Sciences version 22.0 (IBM Corp, Armonk, NY, United States) and analyzed to examine the research hypothesis.
Results: A total of 17 blood samples from SCT (HbAS subjects) labeled as cases and 16 samples from controls (HbAA subjects) were included in this study. The results of this study showed no significant change in sickled erythrocytes in SCT in response to in vitro heat stress.
Conclusion: This study's findings appear to suggest that hyperthermia could be excluded as one of the major factors inducing sickling complications during exhausting exercise. Long-term studies in the future are recommended in this area, particularly to assess the effect of high temperature and sudden death in SCT.

Abstract Aim: To evaluate the use of computed tomography (CT) and low-dose CT in the detection of latent tuberculosis (TB).
Materials and methods: A systematic search of literature in adherence with the PRISMA guidelines was carried out. Quality assessment of the included studies was conducted.
Results: The search strategy identified a total of 4,621 studies. Sixteen studies were considered eligible and included in the review. There was high heterogeneity among all studies. CT was identified as much more sensitive for the detection of latent TB in all studies despite chest radiography often being recommended in guidelines to assess patients for latent TB. Low-dose CT showed promising results in four of the studies; however, these results were limited due to small sample sizes.
Conclusion: CT is much superior to chest radiography consistently identifying additional cases of latent TB. There are limited high-quality publications available using low-dose CT but findings thus far suggest low-dose CT could be used as an alternative to standard-dose CT for the detection of latent TB. It is recommended that a randomised controlled trial investigating low-dose CT should be carried out.

Abstract The COVID-19 pandemic has had a profound global impact, affecting people's physical and mental health, and their social and economic circumstances. Mitigation measures have disproportionately affected women. Studies have reported menstrual cycle and psychological disturbance associated with the pandemic. Pregnancy is a risk factor for severe COVID-19 disease. Reports have also demonstrated associations between COVID-19 infection, vaccination and Long COVID syndrome and reproductive health disturbance. However, studies are limited and there may be significant geographical variation. Also there is bias amongst published studies, and menstrual cycle data was not included in COVID-19 and vaccine trials. Longitudinal population based studies are required. In this review we discuss existing data, along with recommendations for further research required in this area. We also discuss a pragmatic approach to women presenting with reproductive health disturbance in the era of the pandemic, encompassing a multi-system assessment of psychological, reproductive health and lifestyle.

Abstract Aim: The study aimed to identify risk factors associated with sleep-related deaths of infants (0-24 months) in the province of Manitoba, Canada, between January 2009 and December 2018.
Methods: A systematic retrospective case review of autopsies and administrative records in Manitoba between 2009 and 2018.
Results: A total of 145 infants died in cases where unsafe sleep environments were known to have contributed to or resulted in their death and where no explained medical causes were identified. Where data complete, all infants had at least one known risk factor for sleep-related deaths, and 96% had multiple. The most common risk factors increased over time and included objects in the sleeping environment (90% of cases), not approved sleep surfaces (77%) and bedsharing (50%). Indigenous infants, infants of young mothers and infants in low-income neighbourhoods are overrepresented. Risk factors for Indigenous infants differed from cases involving non-Indigenous infants.
Conclusion: A high proportion of sleep-related infant deaths were associated with not approved sleep surfaces and bedsharing, especially for infants under one year. Families in low-income neighbourhoods, Indigenous families and families with young mothers were disproportionately affected by sleep-related infant deaths. There is a need to enhance messaging and smoking cessation messaging in Indigenous communities to prevent sleep-related deaths.

Abstract Wrist arthroplasty is becoming more commonplace, with various implant choices available. The Motec (Swemac Orthopaedics AB, Linköping, Sweden) cementless ball-and-socket system is being increasingly utilised and is designed for the distal component to be implanted into the third metacarpal. However, distal component failure is a recognised complication. We outline our experience with the revision of this component into the second metacarpal. This technical note is presented through our experience of two patients who underwent revision arthroplasty for the management of peri-prosthetic fracture of the third metacarpal. This technique has demonstrated a safe and viable solution to this complication, achieving good anatomical centre of rotation, function, range of movement and patient satisfaction.

Abstract Abstract: Lymphedematous fibroepithelial polyps are rare proliferations involving the glans penis typically associated with chronic condom catheter usage. To the best of our knowledge, less than 20 cases have been reported in the literature with relatively similar clinical presentation. Herein, we report a case of lymphedematous fibroepithelial polyp not associated with condom usage in a 74-year-old man treated with surgical excision. On histopathological examination, the lesion displayed a hypocellular proliferation of spindled and stellate fibroblasts with intermixed Touton-like giant cells embedded within an edematous stroma with thickened vasculature. Immunohistochemical profiling exhibits strong immunoreactivity for smooth muscle actin, weak positivity for factor XIIIa, and no reactivity for CD34 or desmin. We also conducted a review of the reported range of clinical and histopathological features of this entity in addition to the various hypotheses regarding its pathogenesis. We hope to increase awareness and accurate diagnosis of this entity, which could affect clinical management.
