    Abstract In the United Kingdom, local government includes authorities and subordinate agencies that are established according to the law, under the direction of a locally elected council to provide services for their local neighborhoods and represent their concerns and interests. The United Kingdom does not have a federal government, like the United States; there is no division of powers between the central government and local units. The central government has all governmental powers and is dominantly responsible for public policy making. The research will clarify the function of local government and identify the relationship between the central government and local authorities. It will also analyze modern models of policy networks in the UK and demonstrate their difference from the federal system.

    Abstract Over the years, the number of patients with end-stage renal disease (ESRD) have been steadily increasing. Cardiovascular causes account for 55.2% of all deaths among ESRD patients. In particular, there is an increased risk of atrial fibrillation (AF) & venous thromboembolism (VTE). AF is more prevalent in hemodialysis (HD) (21.3%) than in peritoneal dialysis (PD) (15.5%) or in patients with kidney transplant (13.7%). In addition to AF, the risk of stroke (event rate of 5.61 per 100 person years) in ESRD patients is greatly increased compared to those without renal impairment (event rate of 3.61 per 100 person years), which is very concerning due to the already increased cardiovascular mortality among these patients. ESRD is a complex condition in which usual interventions do not often provide the desired outcome seen in other comorbidities. The inherent paradox of hypercoagulability and increased risk of bleeding along with the lack of risk tratification models further complicate the selection of anticoagulants in patients with ESRD and AF. Due to the lack of controlled trials and the contradictory results and outcomes, it is not possible to argue for or against the use of any particular therapeutic agent. Data regarding risks and benefits of antiplatelet agents use among ESRD patients with AF has been conflicting and there is no consensus or guideline regarding their routine use.

    Abstract Whether autoimmune or rheumatic disease can be precipitated after vaccination is controversial. Only epidemiological studies or clinical trials with an extremely large sample size can allow for a consistent assessment of the relative risk of vaccine-related incidence. Studies with such large sample sizes are complex, difficult to perform, and costly, which limit their administration. There exists no minimum criteria for diagnosing vaccine-related autoimmune diseases, this question must be analyzed on a case-by-case basis. Although our report does not prove a causal association between vaccination and vasculitis, it seems possible that in rare cases, vaccination might induce vasculitic disease. While the possibility of autoimmune disease is very real, without vaccinations the SARS-CoV-2 pandemic will spread unchecked, bringing with it a slew of multisystem disorders, including autoimmune diseases both in the present and future ('Table 1, Figures 1, 2').

    Abstract Background A workplace-based assessment (WBA) is used to assess learners’ competencies in their workplaces. Many workplace assessment tools are available and validated to assess various constructs. The implementation of workplace-based assessment requires proper training of the staff.Objective This study aimed to explore the impact of staff training on WBA practices and evaluate the inter-rater reliability of these practices while using entrustment scales, performance descriptors, and personal judgment.Design A quasi-experimental study, in which the staff members of the orthopedic department were invited to participate in a training program on the use of entrustment scales and assessment descriptors within the WBA tools. As a response to the training, subjective judgment was replaced by entrustment scales and performance descriptors in a trauma course offered by the orthopedic department. The inter-rater reliability of the WBA was evaluated using various rating scales.Results The entrustment scales had higher inter-rater reliability of the assessment tools than performance descriptors and the personal judgment.Conclusion The inter-rater reliability was highest when using entrustment scales for WBAs, which could indicate that the entrustment scales achieve good psychometric properties as regards consistency among different raters. Thus, they decrease the confounding effect of differences in assessors. They may also give a clearer image of the actual academic level of the learners.

    Abstract The concomitant occurrence of tuberculous adenitis and Hodgkin lymphoma is rare, posing a diagnostic dilemma since both have similar symptoms, such as lymphadenopathy, weight loss, fever, and night sweats. We reported such a case in a 15-year-old girl who presented with fever and neck swelling and was found to have lymphadenopathy. A biopsy of the right supraclavicular lymph node showed Reed-Sternberg cells and stained positive for acid-fast bacilli and tuberculosis culture. The patient was diagnosed with tuberculous adenitis with concurrent Hodgkin lymphoma in the same lymph node. She was started on anti-tubercular medications and chemotherapy and showed clinical improvement. This case highlights the need for suspicion in order to identify these two disorders in the same patient, since missing one of them is possible and may lead to fatal complications.

    Abstract Introduction: Fasting in patients with type 2 diabetes mellitus (T2DM) is associated with high risk of hypoglycemia. The aim of this study was to compare the effectiveness and safety of vildagliptin in T2DM patients fasting during Ramadan in a real-life setting in Egypt.
    Methods: In this 16-week prospective and noninterventional study, data were collected up to 6 weeks before and after Ramadan fasting. Patients who had received vildagliptin or sulfonylurea (SU) either as dual therapy with metformin or as monotherapy were enrolled into the study.
    Results: Two hundred fifty four patients were enrolled in the study, out of which 246 [121 (97.6%) treated with vildagliptin and 125 (99.2%) with SU] were included in the safety analysis set. A significantly lower proportion of patients experienced ≥1 hypoglycemic event (HE) with vildagliptin as compared to those receiving SUs (1.7% vs. 19.2%, respectively; p < 0.001). No patient in either group reported a grade 2 HE. At week 16, mean change in HbA1c from baseline for vildagliptin and SU were -0.1% and +0.3%, respectively, with a between-treatment difference of -0.4% (p < 0.001). Mean change in body weight from baseline for vildagliptin and SU were -0.8 and -0.1 kg, with a between-treatment difference of -0.7 kg (p = 0.011). A higher proportion of SU-treated patients experienced adverse events compared to those treated with vildagliptin (23.2% vs. 5.8%, respectively), the primary reason being the high incidence of hypoglycemia in the SU group (n = 24, 19.2%).
    Conclusions: Treatment with vildagliptin was associated with lower incidence of hypoglycemia compared with SU and showed good glycemic and weight control in patients with T2DM fasting during Ramadan in a real-life setting in Egypt.

    Abstract Background: The pathogenesis of diffuse sinonasal polyposis is still not completely established, possible explanations are osteitis, aeroallergens, fungal sinusitis and biofilms. There are no reports in Egypt about osteitis and biofilms in those patients. Purpose: To study the incidence and impact of osteitis and biofilms in Egyptian patients on diffuse sinonasal polyposis patients. Patients and methods: Fifty patients (22 males, mean age of 30.68 ± 7.24 years) submitted to surgery for diffuse sinonasal polyposis. Computerized scan on sinuses ordered and scored by Lund–Mackay staging protocol, severity of Osteitis using the Global Osteitis Scoring Scale. Tissue samples were taken from diseased sinuses to be analyzed histopathologically for osteitis, and with scanning electron microscopy to detect bacterial biofilms. Another ten patients as a control scheduled for septoplasty or turbinectomy with no evidence of sinusitis, and tissue specimens were obtained 1 cm behind the anterior end of inferior turbinate and processed in the same manner for biofilm comparison. Study design: Contemporary prospective cross-sectional cohort study. Results: In 70% (35/50) of the polyposis patients, histopathology was positive for osteitis. Biofilms were detected by electron microscope in 39 (78%). Two of controls (20%) were biofilm positive with a significant difference (p = 0.035). The mean Lund–Mackay was 19.08 ± 3.67 and mean osteitis score was 18.68 ± 11.99. There was a significant correlation between Lund–Mackay and osteitis score (p < 0.001) and between both and histopathologically proven osteitis (p = 0.049), biofilms (p = 0.005) and postoperative endoscopic healing (p = 0.046) where increased soft tissue disease and osteitis and biofilm were associated with bad healing and vice versa. Conclusion: Osteitis and bacterial biofilms underlie the majority of Polypoidal chronic rhinosinusitis and both correlated significantly.
