    Main description: Was ist politische Kultur und in welchem Zusammenhang steht sie mit dem Neoliberalismus? Aus einer an Antonio Gramscis Theorie informierten Untersuchungsperspektive zeigt Angelo Maiolino die politische Kultur als eine hegemoniale Konstellation, deren Werte, Deutungsmuster des Politischen und Sozialen sowie Selbst- und Weltverständnisse mittels Konsens und Zwang stabilisiert und reproduziert werden. Dabei spielen die unter dem diffusen Titel des Neoliberalismus stattfindenden Wirtschafts- und Gesellschaftspolitiken eine entscheidende Rolle: Sie verlangen nicht nur neue marktkonforme politische und zivilgesellschaftliche Institutionen, sondern auch entsprechende individuelle Verhaltensweisen, aus denen wiederum kollektiv geteilte Werte und Deutungen entstehen, die in ihrer schieren alltäglichen Präsenz unhinterfragt übernommen werden und so in die politische Kultur sedimentieren. Der Neoliberalismus entpuppt sich so als ein wirtschaftliches und politisches Projekt, das auf eine ihn stützende und reproduzierende politische Kultur angewiesen ist.

    Abstract Compliance with legal deadlines for the assessment and incorporation of technologies in Brazil's Unified Health System (SUS) is essential to ensure public access to essential medicines. The scope of this paper was to analyze the compliance with legal deadlines for incorporation and availability of medicines in the SUS, comparing Oncology and the Specialized Component of Pharmaceutical Assistance (SCPA). A comparison was made of the drugs incorporated that were submitted to Conitec in the period from January 1, 2017, to April 30, 2020. A total of 85 drugs were recommended for incorporation by Conitec, of which 15 (17.64%) were for Oncology and 70 (82.36%) were for SCPA. The time between analysis and recommendation by Conitec until the publication of the decision by the Ministry of Health was, on average, 86 days longer for oncological drugs and the availability timeframe of technologies incorporated in the oncology area was, on average, 389 days longer than for SCPA. The major progress achieved with the creation of Conitec in Brazil is acknowledged, but the results of this study point to a pressing need to improve the process of making available technologies incorporated into the SUS, especially in oncology.

    Abstract Introduction: Chimeric antigen receptor T (CAR-T) cell therapy is an emerging treatment option for relapsed/refractory multiple myeloma (RRMM) that is a multi-step process involving various stakeholders. Appropriate education on the practical logistics is therefore paramount to ensure treatment success.
    Methods: A group of key opinion leaders met to explore the key elements of setting up and running a CAR-T center in Brazil. For each step in the CAR-T cell therapy process, the experts agreed on basic requirements, gave their key recommendations from practical experience, and considered any remaining unanswered questions.
    Results: This paper presents best-practice recommendations and advice on how to overcome common challenges for each step in the CAR-T cell therapy process, with a focus on the current situation in Brazil. Key themes throughout the process are collaboration within the multidisciplinary team and with the referring physician, along with communication and education for patients and their caregivers.
    Conclusion: We believe that the expert insights presented in this paper, in particular on optimal patient selection and timing of CAR-T cell therapy, will deepen understanding of the CAR-T process and aid implementation of this novel therapy for patients with RRMM in Brazil.

    Abstract Multiple myeloma is a malignant clonal plasma cell disorder that accounts for about 10% of all hematological malignancies. Conventional melphalan-based chemotherapy produces less than 5% of complete remission rates, with less than 5% of patients surviving 10 years or more. The VAD regimen is very effective to induce cytoreduction, but does not prolong event free survival or overall survival. High-dose therapy with autologous bone marrow or peripheral blood stem cell support induces complete remission in 30%-50% of patients, with a very low transplant related mortality (<5%). Both event free survival and overall survival are prolonged with High-dose therapy, but relapses continue to be a major problem. A subset of patients (chromosome 13 abnormalities and high level of beta2-microglobulin at diagnosis) at high risk of relapse has been identified. Double transplants and post-transplant strategies, with the use of alpha interferon, thalidomide, or combined chemotherapy, are additional measures used to reduce the incidence of relapse. Allogeneic transplants can induce durable molecular remissions. However, the very high (30%-50%) transplant-related mortality limits the application of this procedure. The emerging concept of non-myeloablative transplants may be a very attractive alternative to improve these results. This strategy may be associated with lower transplant related mortality, and yet provide a graft versus myeloma effect, potentially increasing event free survival and overall survival.
