    Abstract Introduction: Transthyretin-related amyloidosis (ATTRv) is a progressive multisystem disorder, predominantly involving the peripheral nerve system (PNS) and heart. Quantification of small fiber damage may help guide treatment decisions, as amyloid deposits frequently affect those fibers early in disease course. Corneal confocal microscopy (CCM) is a promising method to monitor patients with ATTRv, due to similarities between corneal nerves and PNS, as the cornea is innervated by Aδ and C fibers.
    Methods: We compared CCM measures from ATTRv patients to a group of healthy individuals, matched by age and gender. We then investigated the correlations between small fiber tests (SFT): CCM, LDI-Flare and CDT, COMPASS-31 and disability scales (RODS and ONLS) in patients.
    Results: Of 20 patients (6 with V30M), mean age 50.3±15.3Y, 7 female (35%), six (30%) had polyneuropathy and 10 (50%) carpal tunnel syndrome. CDT was abnormal in 9 and LDI-flare in 6 patients. CCM was abnormal in 19 tested patients and significantly reduced when compared to controls (CNFL: 6.31±0.31 vs. 15.21±1.02mm/mm
    Conclusions: In a diverse cohort of ATTRv patients, CCM was the most frequent abnormal measurement. CCM can be a useful test to triage patients in the early disease stages and with few or equivocal symptoms.

    Abstract Objective: To evaluate the DRRiP (Diabetes Related Risk in Pregnancy) score warning system as a tool for predicting neonatal morbidity in gestational diabetes.
    Methods: A retrospective observational cohort study. By applying nine parameters from an antenatal trichotomy of glycemic, ultrasound, and clinical characteristics, DRRiP scores were calculated and assigned to each patient using a checklist tool. Logistic regression models were used to evaluate the association between DRRiP score and adverse fetal outcomes, after adjusting for maternal age and body mass index (calculated as weight in kilograms divided by the square of height in meters).
    Results: In all, 627 women were studied. DRRiP score was an excellent predictor of macrosomia and shoulder dystocia (both areas under the receiver operating characteristics curves [AUROC] = 0.86), and a modest predictor of preterm delivery, hyperbilirubinemia, neonatal intensive care unit admission and a composite of either of the studied events (AUROC range 0.63-0.69). For the composite outcome, the sensitivity of an amber trigger score of 1 was 68.7% (95% confidence interval [CI] 62.27%-74.63%) and specificity was 48.87% (95% CI 43.85%-53.9%). Specificity at a red trigger score of 3 (89.7%) and a graded increase in post-test probability (90.7% risk at a score of 5) were highly encouraging.
    Conclusion: DRRiP score offers reasonable discriminative performance that could be clinically useful for meaningful risk stratification when making delivery plans.

    Abstract Despite their successes in NLP, Transformer-based language models still require extensive computing resources and suffer in low-resource or low-compute settings. In this paper, we present AxomiyaBERTa, a novel BERT model for Assamese, a morphologically-rich low-resource language (LRL) of Eastern India. AxomiyaBERTa is trained only on the masked language modeling (MLM) task, without the typical additional next sentence prediction (NSP) objective, and our results show that in resource-scarce settings for very low-resource languages like Assamese, MLM alone can be successfully leveraged for a range of tasks. AxomiyaBERTa achieves SOTA on token-level tasks like Named Entity Recognition and also performs well on "longer-context" tasks like Cloze-style QA and Wiki Title Prediction, with the assistance of a novel embedding disperser and phonological signals respectively. Moreover, we show that AxomiyaBERTa can leverage phonological signals for even more challenging tasks, such as a novel cross-document coreference task on a translated version of the ECB+ corpus, where we present a new SOTA result for an LRL.     Abstract Sacrum is an important bone for identification of sex in human skeletal system. Being a component of axial skeleton it contributes to formation of pelvic girdle. This cross sectional and descriptive study was done among 150 (59 male and 91 female) fully ossified dry human sacrum in Mymensingh Medical College, Mymensingh, Bangladesh from January 2017 to December 2017. Sample collection was done by non-random purposive sampling technique from Anatomy department of Mymensingh Medical College and Community Based Medical College, Bangladesh, Mymensingh. The parameters included the straight length of sacrum and width of sacrum which was measured by digital vernier slide caliper and was expressed in millimeter (mm) and sacral index was calculated from these measurements. In present study the mean straight length of sacrum in male and in female was 104.27±5.76mm and 92.82±7.59mm respectively. The mean width of sacrum in male and in female was 99.51±5.80mm and 102.98±6.69mm respectively. Mean sacral index were 95.42±3.14 and 111.27±7.66 in male and female respectively. Comparison of straight length and width of sacrum and sacral index were done between male and female by unpaired student 't' test which was statistically significant.

    Abstract BACKGROUND Dialister pneumosintes is a suspected periodontal pathogen. It can affect different parts of the body either by hematogenous transmission or regional spread. Here, we report a case of 30-year-old previously healthy woman diagnosed with mediastinal and neck abscess caused by this pathogen. CASE REPORT A 30-year-old woman presented with a 1-day history of fever, vomiting, and diarrhea. She was on her last dose of a 2-week course of oral antibiotic for suspected dental abscess. On admission, parenteral broad-spectrum antibiotic was started for sepsis of unknown source. Because of intermittent spike of high temperature despite being on an antibiotic, cross-sectional imaging was performed, which revealed a superior mediastinal abscess with extension in the neck. She was referred to the ENT surgeon for incision and drainage of the collection. However, the procedure was complicated by injury to the right internal jugular vein. Her postoperative period was also convoluted with the development of pulmonary embolism, followed by deep vein thrombosis of the right upper limb. Her pus polymerase chain reaction test detected 16s rRNA gene, suggestive of gram-negative anaerobic bacilli, and anaerobic blood culture grew Dialister pneumosintes. After a prolonged course of illness and antibiotic treatment, she recovered well, and now is back to her normal activities. CONCLUSIONS Potential life-threatening complications may develop from periodontal infection by this microorganism. In patients being treated for sepsis of unknown origin, not responding to antibiotic treatment, and with a history of recent periodontal infection, a deep-seated abscess needs to be considered.
