    Abstract In this work, a novel direct speech-to-speech methodology for translation is proposed; it is based on an LSTMneural network structure which has proven useful for translation in the classical way, i.e., the one consistingof three stages: speech-to-text conversion, text-to-text translation, and text-to-speech synthesis. In contrastwith traditional approaches, the one in this work belongs to the recently appeared idea of direct translationwithout text representation, as this sort of training better corresponds to the way oral language learning takesplace in humans. As a proof of concept digits are translated from an audio source in Spanish and pronouncedas an audio signal in English. Advantages and disadvantages of the proposal when compared with traditionalmethodologies are discussed.

    Abstract Background: The social determinants of health (SDOH) of patients with COVID-19-related thrombosis have been scarcely explored. Our objective was to investigate the cases of thrombosis in a group of socially disadvantaged populations with COVID-19. Methods: We investigated the thrombotic events that occurred in a cohort of migrant and Spanish patients with COVID-19 that were admitted to a medicalized hotel in Madrid. Demographic data, past medical history, and socio-economic backgrounds, such as monthly household income, level of education, and living conditions, were explored to determine the factors related to thrombosis. Results: A cohort of 383 subjects (mean age 55.4 ± 14.6 years old, 69% male), of which 58% were migrants, was studied. Fourteen (3.6%) cases of thrombosis were reported. Thrombosis was more frequent in Spanish than in migrant individuals (OR 5.3, 95%CI 1.4–19.5, p = 0.005). Neither a low monthly household income nor a low education level showed a statistical association with thrombosis ( p ≥ 0.05). History of venous thromboembolism (OR 8.1, 95%CI 2.2–28.6) and being a current smoker (OR 4.7, 95%CI 1.3–16.0) were factors associated with thrombosis. Conclusions: The SDOH studied were not associated with thrombosis; however, further investigation must be performed to investigate the socio-economic conditions of subjects with COVID-19 with adverse outcomes such as thrombotic events.

    Abstract Resumo: Propõe-se uma reflexão sobre o preparo do profissional de Medicina para lidar com a morte. Destaca-se que é através de um mecanismo de negação que esse profissional se defende da angústia produzida pelo falecimento de um paciente. Tal procedimento acarretaria a diminuição gradativa da comunicação com os doentes considerados sem perspectivas de recuperação. Analisa-se como esse comportamento estão relacionados a processos de idealização da figura do médico tanto por parte do paciente como do familiar. Também se alerta sobre a relação que essas dificuldades do relacionamento médico-paciente têm com o currículo e com a estrutura dos cursos de Medicina, assinalando-se possíveis estratégias para fugir do impasse de lidar com a realidade da morte sem os instrumentos adequados.

    Abstract Abstract Remote ischemic conditioning (RIC) is a promising therapeutic approach for ischemic stroke patients. It has been proven that RIC reduces infarct size and improves functional outcomes. RIC can be applied either before ischemia (pre-conditioning; RIPreC), during ischemia (per-conditioning; RIPerC) or after ischemia (post-conditioning; RIPostC). Our aim was to systematically determine the efficacy of RIC in reducing infarct volumes and define the cellular pathways involved in preclinical animal models of ischemic stroke. A systematic search in three databases yielded 50 peer-review articles. Data were analyzed using random effects models and results expressed as percentage of reduction in infarct size (95% CI). A meta-regression was also performed to evaluate the effects of covariates on the pooled effect-size. 95.3% of analyzed experiments were carried out in rodents. Thirty-nine out of the 64 experiments studied RIPostC (61%), sixteen examined RIPreC (25%) and nine tested RIPerC (14%). In all studies, RIC was shown to reduce infarct volume (− 38.36%; CI − 42.09 to − 34.62%) when compared to controls. There was a significant interaction caused by species. Short cycles in mice significantly reduces infarct volume while in rats the opposite occurs. RIPreC was shown to be the most effective strategy in mice. The present meta-analysis suggests that RIC is more efficient in transient ischemia, using a smaller number of RIC cycles, applying larger length of limb occlusion, and employing barbiturates anesthetics. There is a preclinical evidence for RIC, it is safe and effective. However, the exact cellular pathways and underlying mechanisms are still not fully determined, and its definition will be crucial for the understanding of RIC mechanism of action.

    Abstract Los homicidios de pareja, especialmente los cometidos por hombres, han recibido especial atención tanto desde el ámbito social como académico. Pese a esto, son pocos los estudios que han abordado el estudio de las diferencias existentes entre los homicidios de parejas cometidos por hombres y mujeres, habiendo encontrado las investigaciones en este ámbito que existen diferencias importantes. En este trabajo se analizan 149 casos de homicidio de pareja cometidos por hombres (n = 134) y por mujeres (n = 15), registrados y esclarecidos por la Guardia Civil entre los años 2013 y 2018. Los resultados muestran que existen diferencias significativas entre ambos tipos de homicidios y que las explicaciones reduccionistas no tienen cabida a la hora de explicar fenómenos tan complejos y multicausados como los homicidios de pareja, independientemente del sexo del autor.

    Abstract Foundation: diversity of activities carried out by the family doctor, heterogeneity of teachers who evaluate them and different phases in which this doctor can be found, among other circumstances, cause his evaluation in the healthcare context to be riddled by innumerable subjective criteria and that there is no uniformity. Objective: to design an evaluation guide, for direct observation of clinical skills development of the Comprehensive General Medicine resident in gynecobstetric care. Methods: this research was carried out by using theoretical and empirical methods. Clinical skills prioritization was developed on the basis of the identified problems. Direct observation of the resident's behavior was done in their actual outpatient appointments. Professors of Comprehensive General Medicine observed the performance while patient´s assistance. Based on the main problems identified in the resident's performance, the guide was prepared and submitted to expert judgment for approval. Results: the guide includes the evaluation of the following clinical skills: physical obstetric examination, auscultation of the fetal heart rate, physical examination of the postpartum woman, physical examination of the breasts, placement and removal of intrauterine device, cytological test, canal brushing and Schiller test. Conclusion: the guide is easy to apply and allows identifying the resident´s difficulties during the execution of the procedure. Its systematic implementation contributes to the diagnosis of learning needs.

    Abstract El ensayo clínico constituye el método empleado para evaluar un producto, sustancia, medicamento, técnica diagnóstica o terapéutica. La industria biotecnológica emplea sistemas de gestión de ensayos clínicos para lograr un manejo eficiente de los datos recopilados durante el proceso. El Centro de Inmunología Molecular (CIM) emplea el Sistema de Gestión de Ensayos Clínicos XAVIA Clínicas desarrollado por la UCI, para soportar el 100% de los ensayos clínicos desde el 2012. XAVIA Clínicas posee limitantes en sus funcionalidades; además, a partir de la experiencia obtenida por su uso, se identificaron mejoras y nuevas funcionalidades que constituyen el incentivo para desarrollar un nuevo sistema. El objetivo del presente artículo es presentar el Sistema para el manejo de datos de Ensayos Clínicos XAVIA SIDEC y su generalización en las instituciones de BioCubaFarma. Se estudió la bibliografía referente al manejo de datos de ensayos clínicos y cuadernos de recogida de datos electrónicos y se analizaron sistemas homólogos disponibles en el mercado internacional. La metodología de desarrollo empleada fue AUP-UCI. Se utilizó Eclipse como entorno integrado de desarrollo, Java como lenguaje de programación, JBoss como servidor de aplicaciones y PostgreSQL como sistema de gestión de bases de datos. El Sistema para el manejo de datos de Ensayos Clínicos XAVIA SIDEC facilita y flexibiliza el diseño y aprobación de los Cuadernos de recogida de información (CRD), estandariza la información manejada, valida los datos recopilados a través de reglas, mejora el flujo de información entre las entidades que conducen el ensayo y optimiza el proceso de monitoreo.     Abstract IntroductionCoagulopathy associated with isolated traumatic brain injury (C-iTBI) is a frequent complication associated with poor outcomes, primarily due to its role in the development or progression of haemorrhagic brain lesions. The independent risk factors for its onset are age, severity of traumatic brain injury (TBI), volume of fluids administered during resuscitation, and pre-injury use of antithrombotic drugs. Although the pathophysiology of C-iTBI has not been fully elucidated, two distinct stages have been identified: an initial hypocoagulable phase that begins within the first 24 h, dominated by platelet dysfunction and hyperfibrinolysis, followed by a hypercoagulable state that generally starts 72 h after the trauma. The aim of this study was to design an acronym as a mnemonic device to provide clinicians with an auxiliary tool in the treatment of this complication.MethodsA narrative analysis was performed in which intensive care physicians were asked to list the key factors related to C-iTBI. The initial sample was comprised of 33 respondents. Respondents who were not physicians, not currently working in or with experience in coagulopathy were excluded. Interviews were conducted for a month until the sample was saturated. Each participant was asked a single question: Can you identify a factor associated with coagulopathy in patients with TBI? Factors identified by respondents were then submitted to a quality check based on published studies and proven evidence. Because all the factors identified had strong support in the literature, none was eliminated.     Abstract Background and objective: Stress in surgical settings has subtle psychological and physiological repercussions in children. The objective is to evaluate whether hypnosedation is effective in reducing the doses of sedation and analgesia required during the periprocedural period in children undergoing dermatological surgery, without negatively affecting pain and satisfaction.
    Patients and methods: A prospective, longitudinal, observational study where paediatric patients (aged 5-16 years) scheduled for dermatological surgery were analysed according to whether they received hypnosis or distraction during surgery (both common procedures at the centre). As outcome measurements we used sedation doses (propofol) during surgery and the need for analgesia; pain assessment post-surgery and at 24 h using a visual analogue scale (VAS) or revised face pain scale (FPS-r) (both 0-10) depending on age, as well as patient and guardian satisfaction (on a scale of 0-10).
    Results: Of the 68 patients eligible during the follow-up period, 65 were included. Of these, 33 were treated with hypnosis and 32 with distraction. Children who underwent hypnosis required less total propofol (45.5 ± 11.8 mg vs. 69.3 ± 16.8 mg;
    Conclusions: Hypnoanalgesia in children undergoing dermatological outpatient surgery could not only reduce sedation and analgesia requirements, but also improve child and guardian(s) satisfaction.
