    Abstract Using the multiple versions of Doctor Faustus's fraudulent leg removal presented in texts A and B of Christopher Marlowe's tragedy, along with The English Faust Book (a source text for Marlowe), and examining an extensive number of early modern surgical manuals, this essay discusses leg amputation in the early modern period. As well as attempting to understand the circumstances that would cause a surgeon to proceed with such an extreme course of action, the essay also explores the operation itself, its evolution through the early modern period, the instruments used, the life prospects of an amputee in terms of mobility and prosthetics, and finally the social implications and moral responsibilities of removing a limb in the context of a society that placed great importance in the idea of corporeal integrity in the afterlife.

    Abstract Children exposed to parental unemployment have been found to lag behind in school, but research has struggled to pin down the underlying explanation. One hypothesis is that parental unemployment may dampen children's aspirations to do well and go far in school. Yet, few studies on parental unemployment have relied on actual measures of children's aspirations or devised a formal analysis of this mechanism. Using the UK Household Longitudinal Study (Waves 1-12, N = 1067), I investigate the role of educational aspirations in children's General Certificate of Secondary Education (GCSE) attainment. I compare adolescents exposed to parental unemployment before or only after the typical age at which GCSE exams are taken. In adjusted models, children exposed to parental unemployment before their GCSEs are around 6 percentage points less likely to attain any GCSE qualification by age 17. On average, children have high educational aspirations, although intentions to enrol in college or university are relatively lower among children exposed to an early spell of parental unemployment. Nevertheless, a hypothetical intervention setting these aspirations to the same level for all children only accounts for a modest portion of the educational penalty tied to an early spell of parental unemployment. Several sensitivity and robustness tests support this conclusion. This note seeks to stimulate more research on the mechanisms underpinning the intergenerational effects of unemployment. Findings cast doubts on the idea that children's aspirations, the target of broader policy discourse and interventions, are a crucial part of the equation.

De esta sociedad ha pervivido, excepcionalmente y en perfecto estado de conservación, una parte de sus registros contables, once libros correspondientes a la gestión de Joan de Torralba, los cuales representan, sin embargo, una parte ínfima de los que debieron de generar los movimientos económicos de las dos sedes principales (Barcelona y Zaragoza), la delegación de Tortosa y las plazas de Génova, Pisa y Venecia.     Abstract This study examines family expenditures and how they respond to the provision of family cash transfers, particularly among higher-income families. Naming cash benefits with explicit reference to 'families' or 'children' can nudge households into labelling the extra cash for financial investments in children. Labelling has mainly been assessed among lower-income families. Yet if also higher-income families engage in labelling, there could be unintended consequences on the often stark disparities in child-related investments across the socio-economic divide. Drawing on 2006-2019 data from Household, Income, and Labour Dynamics in Australia (HILDA), the study relies on reforms to Australia's Family Tax Benefit to 'reveal' expenditure responses among higher-income families via an instrumented difference-in-differences design. Higher-income households seem to earmark a family cash transfer for children's clothing but not for children's education fees, while they also assign money to adult clothing. Lower-income households, differently, seem to engage in more clear-cut, child-oriented labelling, at the expense of adult-assignable goods. Family cash transfers can nudge households into spending more money on their children across the socioeconomic divide, but not necessarily homogeneously so. Providing more well-off families with modest transfers might thus have limited perverse effects on inequality in family expenditures.

    Abstract The study examines whether and why parental job loss may stifle early child development, relying on cohort data from the population of children born in Ireland in 2007-2008 (N = 6,303) and followed around the time of the Great Recession (2008-2013). A novel approach to mediation analysis is deployed, testing expectations from models of family investment and family stress. Parental job loss exacerbates problem behavior at ages 3 and 5 (.05-.08 SDs), via the channels of parental income and maternal negative parenting. By depressing parental income, job loss also hampers children's verbal ability at age 3 (.03 SDs). This is tied to reduced affordability of formal childcare, highlighting a policy lever that might tame the intergenerational toll of job loss.

    Abstract Background: In recent years, perinatal viability has shifted from 24 to 22 weeks of gestation at many institutions after improvements in survival in neonates delivered at the limit of viability. Monitoring these fetuses is essential because antenatal interventions with resuscitation efforts are available for patients at risk of delivery at the limit of viability. However, fetal monitoring using biophysical profiles has not been extensively studied in very preterm pregnancies, particularly in the periviable period (20 weeks 0 days to 23 weeks 6 days).
    Objective: This study aimed to (1) investigate whether the completion of biophysical profiles within 30 minutes is feasible in very preterm pregnancies, and (2) determine the average observation time required to achieve a score of 8 out of 8 in very preterm pregnancies from 20 weeks 0 days to 31 weeks 6 days.
    Study design: This study prospectively evaluated biophysical scores in singleton pregnancies undergoing routine ultrasonography at or near viability from 20 weeks 0 days to 23 weeks 6 days (periviable or group I), 24 weeks 0 days to 27 weeks 6 days (group II), and 28 weeks 0 days to 31 weeks 6 days (group III). The results and duration of biophysical profiles were compared with those of a control group (32 weeks 0 days to 35 weeks 6 days) undergoing indicated fetal surveillance. Biophysical profiles were performed for all studied pregnancies until a score of 8 out of 8 was obtained. When >1 biophysical profile was obtained during pregnancy, each was analyzed individually. Pregnancies with fetal anomalies or obstetrical/medical indications for fetal well-being surveillance were excluded. Analysis of variance and post hoc Tukey tests were used for comparisons.
    Results: Data were collected for 123 participants, yielding 79, 75, and 72 studies for groups I, II, and III, respectively. The control group included 42 patients, yielding 140 studies. At 30 minutes, 80% (63/79) of the studies in the periviable group had a score of 8 out of 8, as opposed to 100% (140/140) in the control group (P<.001). The mean±standard deviation time in minutes to achieve a biophysical score of 8 out of 8 was 23.3±10.1 in the periviable group, as opposed to 9.4±6.5 in controls (P<.001). Extending the study to +2 standard deviations (43.6 minutes) in the periviable group resulted in 97% (77/79) of the scans scoring 8 out of 8 in the absence of adverse outcomes. In the other groups, a biophysical score of 8 out of 8 within 30 minutes was obtained in 97% (73/75) and 100% (72/72) in groups II and III, respectively; the mean±standard deviation times were 17.1±8.4 minutes (group II) and 13.1±7.3 minutes (group III). No adverse outcomes developed during the study participation in groups I to III.
    Conclusion: Biophysical scores of 8 out of 8 can be successfully achieved in low-risk periviable pregnancies (20 weeks 0 days to 23 weeks 6 days) within an observation time longer than the standard 30-minute duration. The time required to reach a score of 8 out of 8 decreases as gestation progresses. We suggest adjusting the observation time for biophysical profile completion according to the gestational age.

    Abstract The application of new robotic platforms in colorectal surgery has increased greatly in the last 10 years. New systems have been released and entered the surgical panorama, broadening the technological offer. Robotic surgery applied to colorectal oncological surgery has been widely described. Hybrid robotic surgery in right sided colonic cancer has been previously reported. According to the site and local extension of a right-sided colon cancer, a different lymphadenectomy could be required. For more distant and locally advanced tumors a complete mesocolic excision (CME) is indicated. CME for right colon cancer is a complex operation compared to standard right hemicolectomy. Therefore a hybrid robotic system may be effectively applied to CME during a minimally-invasive right hemicolectomy to improve the dissection accuracy. Here we report a step-by-step hybrid laparoscopic/robotic right hemicolectomy with CME performed with the Versius Surgical System, a tele-operated surgical robotic system intended for the use of robotic assisted surgery.
