    Abstract Palliative care focuses on improving the quality of life for people with serious illnesses and their loved ones. This article introduces considerations including barriers to care, intersectionality, minority stress, microaggressions, and social safety that may impact the experience and openness of people to receive this care. The authors outline tools to address these challenges including trauma-informed care and how to recognize bias and earn trust. The authors conclude by offering a model for incorporating these assessments and tools with sample scripts to provide patient-centered and holistic palliative care.

    Abstract Desde 2017, el gobierno de Mendoza impulsa la explotación de hidrocarburos no convencionales mediante la técnica de fractura hidráulica (fracking), al centro-oeste de Argentina. El artículo analiza la narrativa dominante que sostiene dicha iniciativa, enfatizando dos características: se trata de una estrategia de desarrollo presentada como el resultado de las fuerzas compulsivas de los hechos (una realidad inexorable); toda resistencia al fracking es denunciada como un obstáculo al desarrollo (pura irracionalidad). Desde el punto de vista metodológico, el trabajo recurre al rastreo en la prensa gráfica y documentos públicos del gobierno y del sector empresario.

    Abstract The aim of this study was to describe the constructive details of a new collector called Mendeźs Trap (MT) and to compare its performance with the Big Spring Number Eight (BSNE) and Modified Wilson and Cook (MWAC). A detailed description of the MT with dimensions, schemes, and photos is included in the paper. To evaluate the performance of the MT, eleven wind erosion events were measured with MT, BSNE, and MWAC installed at 22.5, 43, 63, 112, and 164 cm height on a loamy sand soil without cover and roughness. The material collected was weighed and divided by the inlet area to obtain the horizontal mass flux (HMF). Results showed that the HMF measured with MT is strongly correlated to the HMF measured with BSNE and MWAC. Relative efficiencies (slope of the regression curve between HMF of different samplers) showed that MT is 31.83% more efficient than BSNE and that MWAC is 81.37% more efficient than MT. Different relative efficiencies were measured at different heights, but no relationships between the relative efficiency and height were found. The average amounts of material collected were the highest at 22.5 cm height (1.74 g for MT) and the lowest at 164 cm height (0.03 for MWAC). At all heights, the amounts of sediments collected were ordered in the sequence MT > BSNE > MWAC. Percentages of samples weighing more than 0.1 g, 0.2 g, 0.3 g, 0.4 g, 0.5 g were ordered in the sequence MT > BSNE > MWA. Field evaluation showed that, the HMF measured with MT, BSNE, and MWAC can be compared, and MT can be used in wind erosion studies. MT collects more amounts of material than MWACs and BSNEs, which is especially important at greater heights where little amounts of material are normally collected. The results of this study must be confirmed in soils of other texture classes and the role of the particle size and wind speed in the efficiency of the MT must be measured in wind tunnel.

    Abstract In 1852 the Province of Buenos Aires rejected the agreement of San Nicolas and refused to participate in the constitutional process begun by the Argentine Confederation. Although it was considered part of the same Nation, Buenos Aires initiated an autonomous process of building State which was crystallized in 1854 with the ratification of its own Constitution. Therefore, replacing its political condition of province by that of State, reserving the exercise of its own sovereignty and declaring their territorial boundaries.The article studies the renewed interest of Buenos Aires’ ruling elite for the territory in those years. In particular, it analyses the debate on territorial limits in the Constitution of 1854, emphasizing the use of legal arguments and their relation to the defense of sovereignty and the construction of an autonomous State. In this sense, its hypothesis lies on the fact that the territory claimed by Buenos Aires was used as an element of dispute against the Argentine Confederation and that it expressed uncertainty as to the organization of the Nation.

    Abstract This chapter surveys the range of different orientations toward decision-making, common clinical scenarios, and considerations to bear in mind when caring for culturally diverse patients at the end of life. While this chapter draws on the cultural competency literature, its primary goal is to articulate an approach to end-of-life care that is rooted in cultural humility and structural competency. Medical providers, as representatives of the social institution of medicine, have their own cultural values that often come into conflict with patients' cultural values, especially when patients and providers have different unspoken visions of the "good death," or when patients wish to receive interventions that their providers deem futile. In the final section of the chapter, we seek to move away from this confrontational paradigm by analyzing two case studies of decision-making across cultures in order to empower providers to engage in value-based shared decision-making and thereby achieve goal-concordant care.

    Abstract COVID-19 is a respiratory disease that has been shown to have extrapulmonary manifestations. One association with COVID-19 is rhabdomyolysis, which is defined as the breakdown of skeletal muscles. There have been increasing reports of rhabdomyolysis in obese, middle-aged male COVID-19 patients, but limited published cases affecting young adult females. This case discusses the early presentation of rhabdomyolysis in a young, morbidly obese, asthmatic woman with COVID-19. A 28-year-old, unvaccinated, African American female with past medical history of asthma, tobacco abuse, and a BMI of 46 initially presented to the emergency department with a complaint of fever, cough, and shortness of breath for two days. She was initially diagnosed with an asthma exacerbation and was treated symptomatically, but her symptoms persisted despite treatment. She began to experience myalgias the next day, followed by bilateral lower extremity weakness and dark urine two days later. Urinalysis revealed gross hematuria, 2-4 red blood cells per high-power field, 100 mg/dL protein, >8.0 mg/dL urobilinogen, and 0-2 hyaline casts. Alanine aminotransferase (ALT), aspartate aminotransferase (AST), and creatine kinase (CK) levels were noted to be elevated. Her subsequent COVID-19 test was positive, and both blood and respiratory cultures were negative. She was diagnosed with rhabdomyolysis which was likely secondary to COVID-19. Her CK, ALT, and AST levels normalized after two weeks with the resolution of rhabdomyolysis, but she continued to have persistent COVID-19 infection and deteriorating respiratory status. She eventually required mechanical ventilation on day 20 and passed away on day 59 of hospitalization. Rhabdomyolysis is an infrequent finding that can be associated with COVID-19. It has been increasingly reported in middle-aged obese male patients but is far less common in younger females. The presence of elevated CK has been associated with higher mortality among COVID-19 patients, but current literature demonstrates that the majority of these patients are older males. It is imperative to recognize and treat rhabdomyolysis in all patients, particularly younger females, to help mitigate the comorbidities of COVID-19.

    Abstract The current Achilles heel of cancer drug discovery is the inability to forge precise and predictive connections among mechanistic drivers of the cancer cell state, therapeutically significant molecular targets, effective drugs, and responsive patient subgroups. Although advances in molecular biology have helped identify molecular markers and stratify patients into molecular subtypes, these associational strategies typically fail to provide a mechanistic rationale to identify cancer vulnerabilities. Recently, integrative systems biology methodologies have been used to reverse engineer cellular networks and identify master regulators (MRs), proteins whose activity is both necessary and sufficient to implement phenotypic states under physiological and pathological conditions, which are organized into highly interconnected regulatory modules called tumor checkpoints. Because of their functional relevance, MRs represent ideal pharmacological targets and biomarkers. Here, we present a six-step patient-to-model-to-patient protocol that employs computational and experimental methodologies to reconstruct and interrogate the regulatory logic of human cancer cells for identifying and therapeutically targeting the tumor checkpoint with novel as well as existing pharmacological agents. This protocol systematically identifies, from specific patient tumor samples, the MRs that comprise the tumor checkpoint. Then, it identifies in vitro and in vivo models that, by recapitulating the patient's tumor checkpoint, constitute the appropriate cell lines and xenografts to further elucidate the tissue context-specific drug mechanism of action (MOA) and permit precise, biomarker-based preclinical validations of drug efficacy. The combination of determination of a drug's context-specific MOA and precise identification of patients' tumor checkpoints provides a personalized, mechanism-based biomarker to enrich prospective clinical trials with patients likely to respond. © 2022 The Authors.
