    Abstract Aim To describe demographic and histomorphological characteristics of 139 patients with epithelial salivary gland tumors in the Southeastern Serbia population. Methods A total number of 139 patients with epithelial tumors arising in major and minor salivary glands in the period 2010-2012 was evaluated. After standard tissue proceeding, the routine haematoxylin-eosin (HE) and histochemical alcian blue-periodic acid-Schiff (AB - PAS) methods were used for histomorphological examination. Results Among 139 patients, 102 (73.38%) had benign, and 37 (26.62%) malignant tumors. The majority of tumors were localized in the parotid gland, in 117 (84.17%) patients. Among benign tumors there were 50 (49.02%) pleomorphic adenoma, 48 (47.06%) Warthin’s tumor, two (1.96%) myoepithelioma, and two (1.96%) oncocytoma. In the group of malignant tumors the most common was mucoepidermoid carcinoma, in 12 (32.43%) patients, carcinoma ex pleomorphic adenoma in six (16.22%), adenoid cystic carcinoma in five (13.51%), and oncocytic carcinoma in three (8.11%) patients. Conclusion Benign tumors were more common than malignant ones, with predominance of pleomorphic adenoma. Malignant tumors are less common than benign in the large salivary glands, and more common in the minor salivary glands. Histochemical AB-PAS method helps in the diagnosis of mucinous salivary gland carcinoma.

    Abstract Eales disease is characterized by three overlapping stages of retinal vasculitis, occlusion, and neovascularization. Diagnosis is mostly clinical, and its etiology appears to be multifactorial. A 26-year-old woman with manifesting vasculitis of the peripheral retina, retinal neovascularization, and vitreous haemorrhage is presented. Depending on the stage of the disease, its management consisted of medical treatment with corticosteroids in the active inflammatory stage and laser photocoagulation in the advanced retinal ischemia and neovascularization stages. Multiple delicate ocular surgeries were performed due to recurrent vitreous haemorrhage and retinal detachment. The experience acquired in this treatment is evidence of the necessity for treatment immediately after the confirmation of Eales disease. The decision to carry out intensive laser treatment and surgery at the right time could improve the prognosis of Eales disease.

    Abstract Melanomas are malignant neoplasms that originate from melanocytes. The most common are on the skin and mucous membranes. Choroidal melanomas are quite different from cutaneous melanomas with regard to presentation, metastases, and treatment. We report two cases of metastatic gastric malignant melanoma of the eye and skin, with reference to the literature. The first patient was a woman aged 23 years, who underwent gastrectomy 22 months after enucleation of the eye due to malignant choroid melanoma. The second patient was a man, 72 years old, who underwent surgery 28 months before because of malignant melanoma of the skin of the forehead. Paraffin sections, 4 μm thick were stained using a classic method, as well as immunohistochemical DAKO APAAP method, using a specific S - 100 antibody and Melan A antibodies. The stomach is considered a rare place for the development of metastases. Metastases in the stomach are often limited to the submucosal as well as the serousmuscular layer, as noted in one of our patients. Metastatic melanoma of the gastrointestinal tract should be suspected in any patient with a history of malignant melanoma and new gastrointestinal symptoms. Because of the similarity between certain common histopathological types of malignant melanoma, primarily achromatic, and types of primary cancers of the stomach, the following immunohistochemical studies are needed: Melan A and S - 100 protein ( markers of malignant melanoma ), as well as mucins: MUC5AC, MUC2 and CDX2 ( markers of different types of primary gastric carcinoma).

    Abstract Dirofilarioses are zoonoses caused by filaria of the genus Dirofilaria, the parasites of domestic and wild animals. People are just random carriers of this parasite. In Europe, human dirofilariosis is caused by two species: Dirofilaria repens ( D. repens, also known as a species of The Old World ), usually with the superficial localization of infection, and D. immitis, which is present throughout the world, and causes, beside superficial, visceral dirofilariosis. So far, based on the data from reference literature, it can be observed that in Serbia about 34 cases of human dirofilariosis have been diagnosed and published. It is assumed that the prevalence of this parasitosis is significantly higher as our country is an endemic area for dirofilariosis in dogs and the region where species of mosquitoes, which are transitory hosts and vectors of Dirofilaria spp., are present. The clinical picture of dirofilariosis depends on the type and location of the parasite in the human body. In our country, patients diagnosed with dirofilariosis had subcutaneous or subconjunctival infection in the majority of cases. Ocular dirofilariosis may affect the orbit and the periorbital region, the skin of the eyelids, the conjunctiva, the Tenon membrane, a retrobulbar space or has an intrabulbar localization. These patients may have a severe disability, and surgery alone can be complicated due to localization. The aim of this review is to highlight the importance of this unexpected important zoonoses, with special emphasis on the importance within the ophthalmic practice.

    Abstract Posterior reversible encephalopathy syndrome (PRES) is an obstetric emergency frequently occurring in a pregnant or puerperal woman, manifested with an acute headache, consciousness impairment, seizures, and visual deficits and is associated with white matter changes predominantly affecting the posterior parietal and occipital lobes of the brain. Apart from the above-described typical location of the changes, the most common atypical location involves the brain stem and basal ganglia. Since magnetic resonance imaging (MRI) is more sensitive and specific imaging technique compared to computerized tomography, establishing the diagnosis and follow-up in patients with PRES is based mainly on MRI findings. It is particularly important not to exclude PRES as a possible diagnosis when we have the appropriate clinical presentation accompanied by the atypical radiological findings, since this clinical-radiological syndrome can often be manifested with an atypical MRI image.

    Abstract Intracranial aneurysm is a focal, abnormal dilation of an artery of the brain. Magnetic resonance angiography (MRA) is a non-invasive technique for vascular imaging and is thus widely used for screening for intracranial vascular lesions. The aim of the study was to show the diagnostic accuracy of 3D Time-of-Flight (3D TOF) MRA in the detection of unruptured cerebral aneurysms with the use of digital subtraction angiography (DSA) as the gold standard. A total of 2.612 consecutive patients underwent 3DTOF MRA. It showed unruptured aneurysms in 94 (3.6%) patients. They included 68 women and 26 men ranging in age from 29 to 76 years (mean, 52.5 years). Twenty-six of them, 20 women and 6 men, underwent DSA. The Mann-Whitney U test was used for the correlation of size. Fisher’s test was used for the correlation of location. The statistical level of significance was set at p<0.05. Most often, the aneurysms were located in the bifurcation of the middle cerebral artery (MCA, n=28, 33.33%) and the internal carotid artery (ICA, n=16, 19%). The mean size of aneurysms was 5.4 mm (range 2 - 15 mm). There was no statistically signifficant difference in the detection and the location (p=0.732) as well as the size (p>0.05) of aneurysms between TOF MRA and DSA. MRA is an accurate and non-invasive method for diagnosis of unruptured intracranial aneurysms. The results of study show the compatibility of MRA findings, the location and the size of an aneurysm in comparison with the “gold standard” – cerebral DSA.

    Abstract Glaucoma is a widely distributed progressive optic neuropathy characterized by progressive damage and loss of retinal ganglion cells of the retina and optic nerve, optic disc excavation increasing and usually, but not always, high intraocular pressure, which eventually results in gradual changes in the visual field. In our study, we tried to determine the potential role and place of electrophysiological researches (PERG and VEP) in the early detection of glaucoma optic neuropathy, comparing the electrophysiological parameters (P100 and A) and numeric parameters in ophthalmic patients studied. The results obtained in our study show that the application of PERG methods detects early damage to the retinal ganglion cells, whereas the same changes were not observed by VEP analysis, suggesting a potential role of predictive PERG analysis in the early diagnosis of pre-perimetric glaucoma.

    Abstract The aim of this research was to examine the relationship between inflammatory parameters and markers of cholestasis in patients with choledocholithiasis. All subjects underwent clinical, laboratory and ultrasound examination at the Department of Internal Medicine, Military Hospital Niš, Serbia. Inflammatory parameters and biochemical markers of cholestasis were measured by standard biochemical methods. Most of cholestasis markers showed no significant correlation with inflammatory parameters in blood, except for a weak significant correlation between the number of monocytes with the activity of aspartate aminotransferase (AST) (r=0.37, p<0.05). Hyperbilirubinemia was significantly correlated with the fibrinogen values (r=0.42, p<0.05), while albumin positive values were positively associated with alanine aminotransferase (ALT) activity (r=0.5, p<0.05), and negatively with the alkaline phosphatase (AP) (r=-0.43, p<0.05). In patients with choledocholithiasis, there was a positive correlation between the number of monocytes and the activities of AST, hyperbilirubinemia, and fibrinogen, albumin and ALT, while a negative correlation between albumin and AP was present.

    Abstract Thelazia callipaeda eyeworm has been frequently reported parasitizing humans in Asia. In Europe, the parasite is endemic in wild and domestic carnivores and only eight cases have been reported in humans so far. We describe the first case of human thelaziosis in Serbia, along with two cases in dogs from the same area. A One Health approach, based on cooperation amongst veterinarians and physicians, is strongly advised for this emerging infection in order to assess the risk for and prevent of the zoonotic infection.

    Abstract We present a 71-year old patient admitted to Plastic and Reconstructive Surgery Clinic, Clinical Center Niš for the right leg conquasation. The patient was injured accidentally by a petrol tiller. On admission, both thigh and lower leg conquasation was observed with large wound on anteromedial thigh and one on medial side of the lower leg. Soft tissue and bones were covered with dirt and pieces of clothing. The wounds were thoroughly rinsed and dirt was removed followed by detailed debridement. After primary surgical treatment of the wound, open proximal lower leg fracture was stabilized with external skeletal fixation using two nails in the proximal and two nails in the distal fragment. Soft tissue defect was treated by plastic surgeon. The patient was administered anti-tetanus protection, antibiotic treatment and anticoagulant prophylaxis of deep vein thrombosis and pulmonary thromboembolism. Physical therapy was initiated subsequent to successful healing of the soft tissue wounds. External skeletal fixation was removed after three months for infection around the proximal nails and the treatment was contuinued using functional Sarmiento’s plaster imobilization leading to full recovery of open lower leg fracture.
