    Abstract The standard care for burns is tangential surgical debridement and subsequent covering, but recently enzymatic debridement has appeared as an alternative. The objective of this study, using an individualised cost-per-patient information system, is to compare the cost per patient of these two alternatives and identify their main determining factors. A non-randomised, retrospective, observational study was carried out with 79 patients, 39 of whom were treated with surgical debridement. The average cost per patient for enzymatic debridement is lower, particularly due to a shorter length of stay of critical hospitalisation (13.7 vs. 18.9 days; €26,101 vs. €33,919), a decreased need for surgical procedures (0.45 vs. 1.28) and a shorter use of operating theatres (53 vs. 202 minutes; €904 vs. €3,000). Age, aetiology, evolution length and percentage TBSA are robust determinants of the cost of care for burn patients. The type of procedure does not appear to significantly affect the cost per patient.

    Abstract The fingertip is mankind's tactile interface with the physical world, from reading braille, to using touchscreens, to wielding power tools. Its special tissue architecture demands astute evaluation and meticulous surgical or nonsurgical care after injury to return patients to their preinjury level of function. Attentive deliberation of physiologic, vocational, and psychosocial factors could improve the odds of achieving satisfactory results. In this article, we explore these aspects of fingertip injury to provoke readers to examine their practices and philosophies.

    Abstract Since the first cases were detected in China in December 2019, the COVID-19 pandemic has spread rapidly, collapsing many healthcare systems, and forcing them to adapt. Hand surgery has been indirectly affected by this scenario. This article aims to provide an overview on how Spanish hand surgeons have modified their daily practice. Based on a survey conducted nationwide, we observed a decrease in the number of emergency cases and cancellation of elective cases, shift to a more conservative treatment approach, use of personal protective equipment, and decrease in the number of outpatient visits and tests. Without definitive evidence at this point, knowing how we have dealt with the situation so far will help us adopt the needed measures to ensure both the patient's and surgeon's safety and manage available resources in future pandemics.

    Abstract Painful neuromas are not uncommon following nerve injury, and are especially bothersome in the hand, with severe cases resulting in significant disability. Outcomes of neuroma surgery are unpredictable regardless of technique. It is recognized that optimal soft tissue environment influences nerve healing more than the specific nerve graft technique, and it is in this context, we present a novel technique of transferring a posterior interosseous nerve graft along with vascularized synovial and fat tissue based on a branch of the posterior interosseous artery to provide healthy soft tissue for nerve healing of neuroma in continuity about the hand and digits.

    Abstract Reconstruction of bilateral soft tissue defects in hand and distal third of the forearm, is a challenge for any reconstructive surgeon. When there is circumferential skin loss affecting the whole hand and fingers as in major degloving injuries, the extent of tissue required for reconstruction narrows down the choice of flaps. When the injury affects both hands the magnitude of the problem becomes compounded. There is no report in the literature of free skin flaps to cover circumferential degloving injuries in both hands. We are presenting the technical considerations and outcome of pedicled abdominal flaps used for immediate coverage of circumferential degloving injuries of both hands.

    Abstract Deep and extensive burns of lower extremities present a difficult challenge to healthcare professionals. After debridement, bones, tendons or joints are frequently exposed and cannot be covered by simple autografts. Moreover, in the case of major burns, damage to the surrounding areas of skin and the severity of the patient's overall condition, often count against using pedicled or microsurgical flaps. In dealing with such complex wounds, which are difficult to treat, several authors have recommended the combined use of Integra(®) and negative pressure wound therapy (NPWT). They emphasize that NPWT eliminates wound exudate, promotes neovascularisation and cell migration through the Integra(®) matrix while increasing its stability and adherence to the wound bed, as well as decreasing the time needed for its total integration. The case presented here is of a patient with major third-degree flame burns to the lower extremities. After debridement, the external and internal malleolus bilaterally became exposed as well as the partially debrided tendons (Achilles, extensor digitorum longus, long and short peroneus, anterior and posterior tibialis). After ruling out the use of local or microsurgical flaps due to the patient's poor general condition and the presence of burns debrided to the fascia over both lower extremities, we elected to manage the patient with a combined treatment using Integra(®) and NPWT. After three weeks of treatment, the surface layer of the Integra(®) matrix was replaced with autografts. Due to partial loss of the skin grafts, a second autograft was needed. At present the patient is completely healed; he can walk with full flexion-extension of both ankles.
