Abstract Introduction: While impact factor (IF) remains the "gold standard" metric for journal quality, newer metrics are gaining popularity. These include the H5-index and journal Altmetric Attention Score (AAS). We explored the relationship between the IF, H5-index, and AAS for core general surgery (GS) and subspecialty journals.
Methods: For all GS and subspecialty journals with a Clarivate IF, H5-index (January 1, 2017, to December 31, 2021) and journal AAS were obtained. Journal Twitter presence and activity was sourced from Twitter and the Twitter application programming interface. Spearman's correlations were assessed for numeric variables.
    Results: A total of 105 journals were included, around half (49/105; 46.7%) of which were core GS journals. Median IF was 2.48 and median H5-index 19. Journal IF demonstrated a strong correlation with H5-index overall (r = 0.81), though this ranged from r = 0.95 (P < 0.01) for vascular surgery to r = 0.77 (P < 0.01) for plastic surgery journals. AAS was moderately correlated with the IF and H5-index (r = 0.59 and 0.62, respectively; both P < 0.01). R
Conclusions: Across GS and subspecialty journals, journal IF correlates strongly with the H5-index and moderately with AAS. However, only 35% of variation in AAS and 66% of variation in the H5-index is explained by the IF, indicating that these metrics measure unique aspects of journal quality. The future growth of surgical journals should be geared towards improving across multiple metrics, including both the conventional and the contemporary, while leveraging social media to improve readership and eventual academic impact.

Abstract New attending surgeons should consider hiring a Certified Financial Planner with experience in the surgical profession while also utilizing accessible resources to improve their financial literacy. They should acquire adequate life and disability insurance, devise debt-repayment strategies, and plan for retirement by contributing to tax-advantaged accounts and diversifying investments. New attending surgeons should also be cognizant of the financial implications of important personal life events, such as marriage and childbirth. Having a financial team is important, and this may consist of a financial advisor and surgeon mentors.

Abstract The Hoffmann-Tinel sign is well-known to professionals dealing with nerve lesions and is widely used as a provocative test. It was described by Paul Hoffman and Jules Tinel in the same year (1915), independently. In the present article, a biographical sketch of both authors is presented and the method for eliciting the sign and the sometimes controversial information of its results are discussed.

Abstract Introduction: Inflammation in acute cholecystitis may cause a cholecystectomy to be more challenging. Due to the difficult dissection, conversion to subtotal cholecystectomy via laparoscopic or open procedure may be required. This is done to reduce the risk of bile duct injury and hemorrhage. We sought to describe the incidence and risk factors, safety, morbidity, and outcomes associated with bailout procedures.
Methods: A single academic center, retrospective review of laparoscopic cholecystectomies that resulted in bailout procedures performed between January 2015 and December 2020. Data collected from the chart review included demographics, comorbidities, length of presenting symptoms, vital signs, laboratory and imaging, intraoperative findings, length of surgery, and outcome.
Results: A total of 1892 cholecystectomies were performed with 147 bailout procedures. For bailout 92 (63.4%) were converted to open, with 66% resulting in complete cholecystectomy. Hypertension and diabetes were the most common comorbidities. The median duration of symptoms was 4 days. Difficult anatomy in the hepatocystic triangle (66%) and dense adhesions (31%) were the most common reasons for bailout. The mean duration of surgery was 145.76 (SD 102.94) minutes. There were 2 bile duct injuries, both in open total cholecystectomy subgroup. Bile leak occurred in 23.8% with majority in subtotal cholecystectomy group. There was no difference in hospital length of stay, surgical site infection, or mortality among different bailout procedures.
Conclusions: Subtotal cholecystectomy represents a safe alternative to total cholecystectomy during challenging cases to avoid damaging surrounding structures. The choice of laparoscopic or open subtotal approach is dependent on the surgeons' expertise.
