    Abstract The purpose of this study is to investigate problems and barriers of greenhouse growers in Isfahan Province, Iran using the qualitative paradigm. Data were collected with semi-structured interviews and direct observations. Most participants were questioned by phone interview. The response rate to phone calls was 59.2 percent. Data are analyzed by the grounded theory. Structural analysis is utilized to evaluate open-ended questions. The components of the questionnaire including the words, concepts and their correlations in terms of frequency, the number of words, vocabularies, metaphors, and idioms used in the sentences are investigated to discover the existing patterns in their statements. As a result, 849 problems are expressed by greenhouse growers. At the open coding stage, the extracted phrases are formulated in 27 terms of meaningful expressions by integrating the same items and removing repetitive cases. At the axial coding stage, 27 formulated concepts are obtained. Subsequently, the extracted phrases create eight main factors. At the selective coding stage, the greenhouse owners' basic problems are categorized into four categories: economic, infrastructure and strategic, human, and extra-organizational. The study shows that about 50 percent of greenhouse growers’ problems are caused by three factors as follows: extreme price fluctuations, brokerage interval, and high costs of fuel. Finding solutions can improve the greenhouse sector.

    Abstract The decision to perform tracheostomy during the COVID-19 pandemic has been based mainly on practical and empirical standards. Also, the amount of scientific evidence to determine the exact timing of tracheostomy in patients with severe COVID-19 on mechanical ventilation is not significant. We conducted a retrospective cohort study on mechanically ventilated COVID-19 patients from April 25, 2021 to January 25, 2022 in intensive care units of Imam Khomeini Hospital Complex, Tehran, Iran. The 30-day survival of patients was calculated and compared between patients under tracheostomy and those without tracheostomy. A number of 135 COVID-19 cases (75 patients in the intubated group and 60 patients in the tracheostomy group) were included in this study. The mean age of the population was 53.6±12.4. The overall mortality rate was 101 (74.8%). The 30-day mortality rate was significantly higher in the intubated group (90.6%) than in the tracheostomy group (55%) (P< 0.001). The mortality rate was 60% in the early (≤ 7 days) tracheostomy group and 50% in the late (>7 days) group. This difference was not statistically significant (P> 0.05). Tracheostomy is a preferred method in airway management of severe COVID-19 patients under mechanical ventilation; however, early tracheostomy during the first week of intubation may not be superior to late tracheostomy in decreasing the mortality rate.

    Abstract The main purpose of this study was to investigate the role of Knowledge Management (KM) in Organizational Entrepreneurship (OE) among agriculture extension workers at Kermanshah Township, Iran. The statistical population in this study consisted of all agriculture extension workers of Jihad-e-Agriculture management and centers of agricultural services at Kermanshah Township (N=143), of whom 129 were available and provided data for this study. The main instrument in this study was a questionnaire which its validity was confirmed by the panel of experts and its reliability was established by Cronbach's Alpha coefficient (α>0.70). Data was analyzed by SPSSwin21 software in two parts of descriptive (frequency, percentage, mean and standard deviation) and inferential (correlation and regression analysis) statistics. Based on the results of enter multiple regression, KM components were the main predictors of OE among agriculture extension workers. The findings of thisstudy have applicationsforJihad-e-Agriculture management and centers of agricultural services in order to promoting OE among agriculture extension workers in Iran.

    Abstract Background: Laryngoscopes are the potential cause of cross-contamination between patients. A considerable way to prevent this problem is to use of disposable laryngoscope’s blades. This study was designed to evaluate laryngoscopic conditions in pediatrics with disposable and metalic Macintosh blades. Methods: 152 children scheduled for elective surgery aged 3-12 years were enrolled in this randomized clinical trial. After induction of anesthesia, anesthesiologist intubated the patients with one of two kinds of laryngoscope’s blades (disposable or metalic Macintosh). During laryngoscopy and tracheal intubation, glottic view, brightness of laryngoscopic field, time of intubation and satisfaction degree of anesthesiologist was recorded. Findings: All patients were successfully intubated. There was significant difference between disposable and metalic Macintosh blades with respect to time of intubation and brightness of laryngoscopic field. Grade I of laryngoscopy was more frequent in metalic group compared to disposable group (83% vs. 49%) Anesthesiologist satisfaction was significantly improved during laryngoscopy with metalic Macintosh blade. Conclusion: With respect to successful tracheal intubation of all patients in this trial and the usefulness of disposable laryngoscopes to prevent cross-contamination between patients and regardless of significant difference in laryngoscopic conditions between disposable and metalic blades, disposable laryngoscopes can be used under normal intubating conditions in pediatrics.
