    Abstract This paper introduces an intelligent index that numerically assesses high-heeled gait deviations. Experiments were conducted on 14 young female volunteers, and the spatial-temporal gait parameters were calculated at each heel height. A type-2 fuzzy system index was built using the baseline case (barefoot). The index showed sensitivity to heel height changes. Moreover, its values divided the heel heights used in this study into three groups, depending on their effect on the gait parameters. A high correlation between the proposed index and the gait profile score (GPS) was found, this supports the index validity to evaluate different human gait deviations.

    Abstract Kombucha is a unique fermented beverage made from a symbiotic culture of yeast and bacteria. Kombucha is normally based on black tea added to water, then sugar is added as a substrate for fermentation in this beverage. This unique beverage is composed of amino acids, flavonoids, vitamins, and some active enzymes. Several beneficial health effects such as antioxidant, antimicrobial effects have been reported as a result of probiotics and prebiotics presence. These health effects of kombucha are attributed to its bioactive chemical and biological agents of probiotics bacteria e.g., Gluconobacter, Acetobacter and yeasts like Saccharomyces sps., along with glucuronic acid as the main sources of the health protection. This review focuses on the beneficial effects of Kombucha including antimicrobial, antioxidant, anti-cancer antidiabetic properties, as well as liver protection, treat of gastrointestinal problems, AIDS, gastric ulcers, obesity (and energy production), detoxification, and skin health.

    Abstract Heavy metals and mycotoxins in foodstuffs are one of the major concerns of our world nowadays. Food decontamination with the help of microbial biomass is a cheap, easy, efficient and green method known as bioremoval. Probiotics are able to reduce the availability of heavy metals and toxins in food products. The purpose of this review is to summarize the probiotics and potential probiotics' interesting role in food bio-decontamination. After a brief glance at the definition of potential probiotic strains with bioremoval ability, LABs (lactic acid bacteria) are described as they are the most important groups of probiotics. After that, the role of the main probiotic and potential probiotic strains (Bacillus, Lactobacillus, Lactococcus, Enterococcus, Bifidobacterium, Pediococcus, Propionibacterium, Streptococcus and Saccharomyces cerevisiae) for heavy metals and mycotoxins bioremoval are described. Additionally, the bioremoval mechanism and the effect of some factors in bioremoval efficiency are explained. Finally, the investigations about probiotic and contaminant stability are mentioned. It is worth mentioning that this review article can be exerted in different food and beverage industries to eliminate the heavy metals and mycotoxins in foodstuffs.

    Abstract Objective: Allogeneic stem cell transplantation (allo-SCT) may have a curative potential due to the graft versus lymphoma effect. In this study, we aimed to compare transplant outcomes between refractory-T-NHL (ref-NHL) and Chemosensitive-T-NHL (CS-T-NHL).
    Materials and methods: We retrospectively reviewed the records of 26 ref-NHL and 29 CS-T-NHL consecutive patients who underwent allo-SCT at our center and compared the transplant outcomes between the groups.
    Results: All patients were heavily pretreated with 27% of patients relapsing post-auto-SCT and two patients in the ref-T-NHL post-allo-SCT. Patients were transplanted mainly from unrelated donors. There were no differences in leucocytes and platelet engraftment between the two groups. At 3 years, the relapse incidence was 34% in Ref-TNHL and 19% in CS-TNHL (p = .33), with non-relapse mortality rates of 28% and 22%, respectively (p = .52). Female patients and those with a previous auto-SCT had lower relapse incidence (p = .045, p = .003). The 3-year overall survival was 39% in Ref-TNHL and 56% in CS-TNHL (p = .15). Trends for improved progression-free survival (PFS) and graft-versus-host disease relapse-free survival (GRFS) were observed in the CS-TNHL group (PFS: 60% vs. 30%, p = .075; GRFS: 38% vs. 21%, p = .1).
    Conclusion: Acknowledging the retrospective nature of our study, our results indicate that allo-SCT has a curative potential in patients with T-NHL even in refractory status.

    Abstract We aimed to develop a concise objectifiable risk evaluation (CORE) tool for predicting non-relapse mortality (NRM) and overall survival (OS) after allogeneic hematopoietic stem cell transplantation (allo-HCT). A total of 1120 adult patients who had undergone allo-HCT at our center between 2013 and 2020 were divided into training, first, and second validation cohorts. Objectifiable, patient-related factors impacting NRM in univariate and multivariate analyses were: serum albumin, serum creatinine, serum C-reactive protein (CRP), heart function (LVEF), lung function (VC, FEV1), and patient age. Hazard ratios were assigned points (0-3) based on their impact on NRM and summed to the individual CORE HCT score. The CORE HCT score stratified patients into three distinct low-, intermediate-, and high-risk groups with two-year NRM rates of 9%, 22%, and 46%, respectively, and OS rates of 73%, 55%, and 35%, respectively (

    Abstract Objective: In a prospective study, SARS-CoV-2 IgG seroprevalence was assessed during the second pandemic wave (W2) in a cohort of Inflammatory Bowel Disease (IBD) patients using biologics. The secondary aim was to compare, in the same cohort, the frequency of seropositivity and of COVID-19 during the second vs. the first (W1) wave.
    Patients and methods: From November 2020 to March 2021, SARS-CoV-2 IgG seropositivity and the prevalence of COVID-19 were assessed in a cohort of IBD patients using biologics already studied at W1.
    Inclusion criteria: age ≥ 18 years; diagnosis of IBD; follow-up; written consent.
    Exclusion criteria: SARS-CoV-2 vaccination. Risk factors for infection, compatible symptoms, history of infection or COVID-19, nasopharyngeal swab test were recorded. Data were expressed as median [range]. The χ2 test, Student's t-test, logistic regression analysis was used.
    Results: IBD cohort at W1 and W2 included 85 patients: 45 CD (52.9%), 40 UC (47.1%). When comparing the same 85 patients at W2 vs. W1, a higher SARS-CoV-2 seroprevalence at W2 was at the limit of the statistical significance (9.4% vs. 2.3%; p=0.05). The prevalence of COVID-19 at W2 vs. W1 was 3.5% (3/85) vs. 0% (0/85) (p=0.08). Contacts with COVID-19 patients and symptoms compatible with COVID-19 were more frequent at W2 vs. W1 (18.8 % vs. 0%; p=0.0001; 34.1% vs. 15.3%; p=0.004). At W2, history of contacts and new onset diarrhea were more frequent in seropositive patients [4/8 (50%) vs. 12/77 (15.6%); p=0.01 and 4/8 (50%) vs. 2/77 (2.6%); p=0.0001]. At W2, the risk factors for seropositivity included cough, fever, new onset diarrhea, rhinitis, arthromyalgia, dysgeusia/anosmia at univariate (p<0.05), but not at multivariate analysis. History of contacts was the only risk factor for seropositivity at univariate (p=0.03), but not at multivariate analysis (p=0.1).
    Conclusions: During W2, characterized by a high viral spread, IBD and biologics appeared not to increase the prevalence of SARS-CoV-2 infection or COVID-19 disease. New onset diarrhea mimicking IBD relapse may be observed in patients with SARS-CoV-2 infection.

    Abstract Allogeneic hematopoietic stem cell transplantation (alloHSCT) remains the only curative treatment for myelofibrosis (MF). Relapse occurs in 10-30% and remains a major factor for dismal outcomes. Previous work suggested that graft-versus-host disease (GVHD) might be associated with risk of relapse. This study included 341 patients undergoing their first (n = 308) or second (n = 33) alloHSCT. Anti-T-lymphocyte or antithymocyte globulin was used for GVHD prophylaxis in almost all patients. Median time to neutrophile and platelet engraftment was 13 days and 19 days, respectively. The cumulative incidence of acute GVHD grade II-IV was 41% (median, 31 days; range, 7-112). Grade III-IV acute GVHD was observed in 22%. The cumulative incidence of chronic GVHD was 61%. Liver was affected in 23% of acute GVHD cases and 46% of chronic GVHD cases. Severe acute GVHD was associated with high non-relapse mortality. The development of acute GVHD grade II and moderate GVHD was an independent factor for reduced risk for relapse after transplantation without increased risk for non-relapse mortality, while especially acute GVHD grade IV was associated with high non-relapse mortality. Last, we identified that ongoing response to ruxolitinib, accelerated-phase MF at time of transplantation and splenectomy prior to transplantation were independent predictors for relapse.

    Abstract Internet of Things technologies are spurring new types of instructional games, namely reality-enhanced serious games (RESGs), that support training directly in the field. This paper investigates a key feature of RESGs, i.e., user performance evaluation using real data, and studies an application of RESGs for promoting fuel-efficient driving, using fuel consumption as an indicator of driver performance. In particular, we propose a reference model for supporting a novel smart sensing dataflow involving the combination of two modules, based on machine learning, to be employed in RESGs in parallel and in real-time. The first module concerns quantitative performance assessment, while the second one targets verbal recommendation. For the assessment module, we compared the performance of three well-established machine learning algorithms: support vector regression, random forest and artificial neural networks. The experiments show that random forest achieves a slightly better performance assessment correlation than the others but requires a higher inference time. The instant recommendation module, implemented using fuzzy logic, triggers advice when inefficient driving patterns are detected. The dataflow has been tested with data from the enviroCar public dataset, exploiting on board diagnostic II (OBD II) standard vehicular interface information. The data covers various driving environments and vehicle models, which makes the system robust for real-world conditions.
