    Abstract INTRODUCTION: The ratio of neutrophils-lymphocytes (NLR) and platelet-lymphocytes (PLR) is a new parameter in the assessment of patients with Hashimoto's thyroiditis OBJECTIVE: The aim of this study was to investigate the effect of NLR and PLR in patients with Hashimoto's thyroiditis MATERIALS AND METHODS: In this cross-sectional study, subjects were subjected to tests of thyroid gland function, antithyroid antibodies, as well as laboratory analyzes of blood count with determination of NLR and PLR. The respondents were grouped into two groups. The first group was patients with Hashimoto's thyroiditis (HT), while the second group consisted of healthy individuals who represented the control group. RESULTS: NLR was statistically significantly higher in patients with HT compared to the control group (2.62±0.8 and 2.43±0.8, respectively; p=0.02), while PLR was higher in people with HT compared to the control group, but without statistical significance significance (169±42.5; 159±40.3; p=0.08). Among the examined patients with HT, the group with hypothyroidism showed statistically higher NLR values compared to the group of patients with euthyroid status (2.7±0.9

    2.31±0.7 p=0.03). Among the examined patients with HT, the group with hypothyroidism showed statistically higher PLR values compared to the group of patients with euthyroid status, as well as the group with subclinical hypothyroidism (177.8±48.2; 148.3±39.3; 155.5±42.5 p=0.04). NLR and PLR show a statistically significant positive correlation with the level of TSH, Anti TPO and TG At in the group with HT. CONCLUSION: NLR and PLR can serve as practical and valuable markers of the clinical course of the disease, but also markers of autoimmune diseases that progress with chronic inflammation.

    Abstract Introduction/Objective. Vitamin D not only plays a role in calcium and phosphorus metabolism, but also has antiproliferative, prodifferentiation, anti-inflammatory and immunomodulatory effects. The aim of this study was to investigate the association between vitamin D deficiency in individuals with autoimmune Hashimoto’s thyroiditis. Methods. A total of 156 patients were enrolled and divided into two groups. First group included 108 patients with 25 (OH) D insufficiency, and second included 48 individuals with normal 25 (OH) D levels. All participants underwent a detailed clinical examination, laboratory tests for thyroid function [T3, fT4, TSH, thyroid antibodies (TPO-Ab, and TG-Ab)], as well as ultrasound scanning (thyroid volume and Doppler characteristics). Results. The patients with vitamin D insufficiency (n = 108 (69.2%)) were predominantly female and had a higher body mass index than the patients with normal vitamin D levels. The group with vitamin D insufficiency had statistically significantly higher TSH levels. The prevalence of positive thyroid antibodies was higher in the vitamin D insufficiency group, while thyroid volume, superior thyroid artery, and inferior thyroid arteries resistance index, as well as the prevalence of positive circular dichroism signals, were significantly higher in the vitamin D insufficiency group. Out of the 156 subjects, 44 were diagnosed with thyroiditis (28.2%). The mean serum level of 25 (OH) D was statistically notably lower in patients with thyroiditis (20.23 ± 8.10 ng/mL) than in the group without thyroiditis (25.44 ± 8.38 ng/mL), p < 0.001. Conclusion. There was an association between vitamin D insufficiency and hypothyroidism in subjects with Hashimoto’s thyroiditis.

    Abstract Having a baby who is prenatally or postnatally diagnosed with a medical condition places considerable stress on the parents, infants, and their developing relationship. Infant mental health (IMH) services offer an opportunity to address the challenges and support the parent-infant relationship. The present study outlined a continuum of care IMH program embedded within various medical settings of a large metropolitan children's hospital. Applications of IMH principles within the fetal care center, neonatal intensive care unit, high risk infant follow-up clinic, and the patient's home are described. Descriptive data about families served across settings and a case study are provided in order to illustrate the implementation of this unique IMH intervention model.

    Abstract This study aimed to (1) comparatively analyze properties of Sr- and Mg-substituted hydroxyapatite (HAP)-based dental inserts; (2) evaluate insert bonding to restorative materials, and (3) evaluate the effect of doped HAP inserts on fracture resistance (FR) of human molars with large occlusal restorations. By ion-doping with Sr or Mg, 3 insert types were obtained and characterized using XRD, SEM, Vickers hardness and fracture toughness. Shear bond strength (SBS) was determined between acid etched or unetched inserts and following materials: Maxcem cement (Kerr); Filtek Z250 (3M) bonded with Single Bond Universal (SBU; 3M) or Clearfil Universal (Cf; Kuraray). Modified Class I cavities were prepared in 16 intact molars and restored using insert + composite or composite only (control) (n = 8/group).     Abstract Hospitalization in the Neonatal Intensive Care Unit (NICU) is a stressful and potentially traumatic experience for infants as well as their parents. The highly specialized medical environment can threaten the development of a nurturing and secure caregiving relationship and potentially derail an infant's development. Well-timed, dose-specific interventions that include an infant mental health approach can buffer the impact of medical traumatic stress and separations and support the attachment relationship. Many psychological interventions in the NICU setting focus on either the parent's mental health or the infant's neurodevelopmental functioning. An alternative approach is to implement a relationship-based, dyadic intervention model that focuses on the developing parent-infant relationship. Child-parent psychotherapy (CPP) is an evidence-based trauma-informed dyadic intervention model for infants and young children who have experienced a traumatic event. This article describes the adaptation of CPP for the NICU environment.

    Abstract Parents and infants in the neonatal intensive care unit (NICU) are exposed to considerable stress, and infant-family mental health (IFMH) services foster emotional well-being in the context of the parent-infant relationship. This mixed-methods study examined the role of an IFMH program introduced in a level 4 NICU. The study included (1) retrospective medical record review of NICU patients who were referred to the IFMH program and (2) qualitative interviews with NICU nurse managers, neonatologists, and medical social workers to explore their understanding of the IFMH program, explore the referral pathways and factors that supported family engagement, and identify specific recommendations for program improvement. Of the 311 infant-parent dyads referred to the IFMH program, 62% had at least one session and Spanish-speaking families were more likely to engage. Of those families receiving services, about one-third had brief intervention, one-third had 4 to 10 sessions, and one-third had long-term services, including in-home after-discharge services. Qualitative interviews with health providers identified unique qualities of the IFMH program and why families were and were not referred to the program. Recommendations centered on adding a full-time IFMH mental health provider to the NICU and increasing communication and integration between the IFMH program and the medical team.

    Abstract Introduction. Obesity is a complex metabolic disorder and one of the most common modern health problems. Numerous studies indicate association between chronic low-grade inflammatory state and obesity. Myeloperoxidase (MPO) and its reactive oxidants participate in tissue damage in the course of inflammatory processes. The aim of this study was to examine MPO activity in the serum of obese participants with metabolic syndrome and its relationship with other indicators of inflammation. Material and Methods: Participants were divided into three groups according to the anthropometric parameters and biochemical indicators: normally fed ones (n=30), participants with abdominal obesity (n = 30) and participants with metabolic syndrome (n = 30). In the serum of patients was being determined chlorination activity of MPO by spectrophotometry. Results: Significant differences were found in MPO activity in all three groups of participants such as: the maximum activity was measured in patients with metabolic syndrome (p <0.001). There was a positive correlation between MPO activity and atherogenic index, as well as between the MPO and the concentration of LDL-cholesterol, while the negative correlation was found between MPO and the concentration of HDL-cholesterol. Conclusion: The examination has shown that the activity of MPO progressively increases with obesity and metabolic syndrome. The obtained results suggest that MPO can be of great importance for the pathophysiological mechanisms leading to the development of complications of obesity and metabolic syndrome.
