    Abstract This study developed a high-temperature and high-pressure (HTHP) cell for in situ neutron imaging of hydrothermal reactions. The cell's maximum temperature and pressure were 500 °C and 50 MPa, respectively, and its vessel for observing reactions comprised SUS316 stainless steel. Neutron transmission images were obtained to observe the behavior of sub- and supercritical water and the decomposition of two plastics (polypropylene and polyethylene) at HTHP. The images showed that water's density and phase changed with temperature and pressure, affecting neutron transmission (and thus image brightness). The plastics began to melt and change shape at 150-200 °C, and they decomposed at 500 °C and 20 MPa. This study provides a basis for future research using the HTHP cell to examine various reactions such as the decomposition of biomass samples, the reforming of heavy oil, and the synthesis of nano-materials using sub- and supercritical water.

    Abstract There are few reports of the adoption of continuous processes in bioproduction, particularly the implementation of end-to-end continuous or integrated processes, due to difficulties such as feed adjustment and incorporating virus filtration. Here, we propose an end-to-end integrated continuous process for a monoclonal antibody (mAb) with three integrated process segments: upstream production processes with pool-less direct connection, pooled low pH virus inactivation with pH control and a total flow-through integrated polishing process in which two columns were directly connected with a virus filter. The pooled virus inactivation step defines the batch, and high impurities reduction and mAb recovery were achieved for batches conducted in succession. Viral clearance tests also confirmed robust virus reduction for the flow-through two-column chromatography and the virus filtration steps. Additionally, viral clearance tests with two different hollow fiber virus filters operated at flux ranging from 1.5 to 40 LMH (liters per effective surface area of filter in square meters per hour) confirmed robust virus reduction over these ranges. Complete clearance with virus logarithmic reduction value ≥4 was achieved even with a process pause at the lowest flux. The end-to-end integrated continuous process proposed in this study is amenable to production processes, and the investigated virus filters have excellent applicability to continuous processes conducted at constant flux.

    Abstract Rib stress fractures can occur in sports involving repetitive trunk muscle contractions. Few documented cases show rib fractures from repetitive upper-limb training. This report presents a case of a rib stress fracture from micro blunt trauma during bench press exercises. The patient was a 24-year-old elite female sprinter with a history of left navicular stress fracture and secondary amenorrhea treatment a year prior. She modified her bench press technique to involve bouncing the barbell on her rib cage. Within weeks, she experienced anterior chest pain, exacerbated by sneezing. Computed tomography revealed a fifth rib fracture with callus formation. After refraining from bench presses for 4 weeks, her chest pain subsided, allowing her to resume upper-limb training. When athletes performing bench press exercises experience chest pain, rib fractures should be considered, even without acute trauma. Additionally, research on the safety of the bouncing bench pressing technique is required.

    Abstract Auditory verbal hallucination is one of the core symptoms of schizophrenia, same as delusions, and also occurs in many other psychiatric disorders. Significant numbers of people with congenital deafness experience auditory verbal hallucinations; however, there are only a few reports regarding the course of psychosis in people with congenital deafness. Herein, we report the case of a patient with congenital deafness and auditory verbal hallucinations whose diagnosis was changed from psychotic major depression to schizophrenia 7 years after the onset of the disease. His psychotic symptoms decreased when his primary medication was changed from an antidepressant to antipsychotic drugs, based on the change of diagnosis. In the treatment of congenitally deaf patients with auditory verbal hallucinations, the inability to communicate through spoken language may interfere with proper diagnosis and treatment. The ability to collect detailed information in ways other than through verbal language is imperative for psychiatrists to determine the appropriate diagnosis and treatment for these patients during the longitudinal course of illness.

    Abstract Gas exchange between soil and atmosphere governs the growth of plants and global circulation of the gaseous component of air. Enhancement of gas exchange by wind has been reported in media with large pores. In clayey paddy fields, large aggregates (clods) and inter-aggregate pores are prone to be formed by tillage. However, the effect of wind on gas transport has not been considered in such fields. This study quantified the wind-induced gas movement through the paddy soil layer composed of large clods. The applicability of the analysis using the gas diffusion equation was also verified regarding the mean clod size and wind condition. Undisturbed soil columns were taken from the ‘poorly tilled’ and ‘fairly tilled’ paddy field under the cultivation of soybean. The mean diameters of the clods were 3.4 and 1.9 cm for the ‘poorly’ and ‘fairly’ tilled columns, respectively. A repacked silica sand column was also prepared as a reference. Diffusion tests were conducted for the columns in the field under natural wind and in the laboratory under calm conditions, with CO₂ as a purge gas and atmospheric oxygen as a tracer gas. The gas transfer efficiency was evaluated by and , which is the ratio of the time until the oxygen concentration reaches 50% and 90% of that in the atmosphere in the field to that in the laboratory, respectively. The results showed that gas transfer through the paddy soil layers, comprised a few centimeters of clods, under natural wind was up to 11 times quicker than under the calm conditions. The wind-induced gas exchange remained at maximum under the wind speed of more than 1.2 m s⁻¹ at 20 cm above the soil surface in the poorly tilled soil. In contrast, in the fairly tilled soil, the effect of wind gradually increased with wind speed and finally became close to that in the poorly tilled soil at the wind speed around 2.0 m s⁻¹. The applicability of the gas diffusion equation deteriorated as the wind speed increased from 1.2 to 2.6 m s⁻¹ or the mean aggregate size increased from 1.9 to 3.4 cm.

    Abstract Polyallylamine (PAA) has been utilized as a salt tolerant anion exchange chromatography ligand in downstream processing of biopharmaceuticals. We have developed novel MMC resins based on PAA polymer ligand partially modified with hydrophobic butyl or phenyl group. The resulting hydrophobic modified PAA ligand reduced HCP level to 12% (21–23 ppm) under 6 mS/cm in a flow-through polishing step of mAb, while not modified PAA ligand showed only 79% (145 ppm). We also found that structure of hydrophobic groups in the ligand mainly influenced on mAb yield. That is 25% increase of phenyl group modification ratio reduces mAb yield from 95% to 90%. On the other hand, modification with butyl group kept mAb yield more than 95%. The optimized ligand structure displayed a wide operational conductivity range. Extended purification studies of mAb using the MMC resin in the flow-through polishing step were carried out under optimized pH and conductivity condition as determined in a DOE study. The study revealed that the MMC resin was effective for developing one-step flow-through polishing workflow for mAb purification. In addition, the MMC flow-through polishing step could be directly coupled with a specified CEX chromatography step to efficiently remove mAb aggregates from 2.3% to <1.0% to achieve a biopharmaceutical-grade quality and a high yield of mAb (>93%) with a high loading capacity around 1000 mg/mL-resin. This new MMC resin will be useful in future mAb manufacturing platforms comprising of a robust and cost-effective flow-through polishing step.

    Abstract Osteosarcoma (OS) is the most common malignant bone tumor and often occurs in children. Chemotherapy with methotrexate, cisplatin, doxorubicin, and ifosfamide has greatly improved the prognosis of patients with OS, and most patients have been able to preserve their limbs. However, no progress has been made in the treatment for OS in the past few decades, and the prognosis of patients with metastasis and/or local recurrence remains poor. Therefore, studies aimed at developing new treatment methods for OS are urgently required. Here, we discuss the current status of immunotherapies for OS as well as the current limitations in the field.In recent years, immunotherapy has been shown to be effective for treating several cancers, and its indication is continually increasing. Immunotherapy is also expected to be widely used for treating OS, however, the efficacy of immunotherapy for OS has not been established.

    Abstract Background: Spinal metastases can impair mobility, worsening the Karnofsky Performance Status (KPS). Surgery for spinal metastases has the potential to improve KPS and extend prognosis, but it is crucial to recognize the elevated risk of perioperative complications. Therefore, the development of a new scoring system to accurately predict perioperative complications in spinal metastatic surgery is essential.
    Methods: We conducted a retrospective observational study with 86 patients who underwent surgical intervention for spinal metastases. Patients were divided into two groups based on the presence or absence of perioperative complications within 14 days after surgery. Various factors related to perioperative complications were assessed through univariate and multivariate analyses. We established a clinical prognostic scoring system called the Perioperative Complications following Metastatic Spinal Surgery (PERCOM) score and evaluated its precision using receiver operating characteristic (ROC) analysis.
    Results: Five variables (age, KPS, primary prostate cancer, Albumin, and Hemoglobin) identified in the univariate analysis were assigned binary values of 0 or 1. The PERCOM score was then calculated for each patient by summing the individual points, ranging from 0 to 5. The optimal threshold determined by ROC curve analysis for the PERCOM score was 2 points, with a sensitivity of 86 % and a specificity of 56 %.
    Conclusions: The composite PERCOM score effectively predicted perioperative complications in spinal metastasis surgery. To further validate its precision, a prospective multicenter study is needed.

    Abstract Introduction: Synovitis, acne, pustulosis, hyperostosis, and osteitis (SAPHO) syndrome is a spectrum of heterogeneous diseases commonly recognized by skin and osteoarticular lesions. There have been reports of some surgical cases of the progressive, destructive spondylitis associated with SAPHO syndrome, wherein the destructive spondylitis was considered to have developed due to the progression of spondylitis with SAPHO syndrome as the pathogenic bacteria were not isolated. We herein report a surgical case of destructive cervical spondylitis associated with SAPHO syndrome.
    Case report: A 54-year-old woman with a history of palmoplantar pustulosis suffered severe neck pain for 6 months. Radiography and computerized tomography (CT) showed sclerosed and collapsed cervical vertebrae, and the patient was referred to our hospital for further evaluation and management upon suspicion of infection or spondylitis with SAPHO syndrome. For the severe neck pain and progressive destruction of cervical vertebrae, we performed posterior fusion surgery with subsequent anterior fusion. Cutibacterium acnes (C. acnes) was isolated by enrichment culture with thioglycolate broth from both the anterior and posterior tissue samples. We diagnosed pyogenic spondylitis secondary to P. acnes infection and administered doxycycline for 6 weeks after the first surgery. The neck pain was resolved and cervical fusion was achieved one year postoperatively.
    Conclusion: C. acnes infection could elicit destructive spondylitis. An enrichment culture should be performed to isolate the pathogenic bacteria in cases of destructive spondylitis with SAPHO syndrome.
