    Abstract Background/Aim: Irritable bowel syndrome (IBS) belongs to the gastrointestinal disorders characterised by abdominal discomfort and pain, altered constipation, diarrhoea and stomach distension. The aim was to assess relationship between the selected genetic polymorphisms with IBS, their combined genotype effect as well as to assess a difference in the distribution of allele and genotype frequencies of selected loci between case and control group. Methods: This was a prospective study which included 29 participants, 20 individuals diagnosed with IBS based on Rome III criteria and 9 healthy individuals. The study analysed the selected genetic polymorphisms as possible risk factors for IBS according to the model of the case-control study. Genotyping was performed for FKBP5, DRD2 and DAT polymorphisms qualified as risk factors for IBS in previous researches. Results: The results revealed a significant association between DAT polymorphism with IBS, both, at the allelic level (p = 0.006) and genotype level (p = 0.031). Individuals with 434 allelic variant in the genotype have six time higher probability for developing IBS, in comparison to the individuals without this allelic variant. The statistical association between other analysed polymorphism and IBS was not reached. The analysis of combined effects of selected polymorphisms revealed no association with IBS, except FKBP5 and DAT which result was at the level of statistical significance (p = 0.05). Conclusion: Further analysis which would include DAT polymorphism with larger sample size, as well as other genes involved in dopamine neurotransmitter system would be of great interest to define closer conclusion of IBS aetiology.

    Abstract In clinical practice, ascites treatment is, in majority of cases, unsatisfactory and followed by multiple complications. During the therapy, some side effects, in relation to therapeutic method, may occur. The aim of the study was to compare the level of tolerance and effectiveness of ascites therapy in applying abdominal puncture versus diuretics between two groups of patients to establish connection and differences in applied treatments. Patients and methods. There were 60 patients examined with ascites 3+ and 4+ divided into two equal groups. First group was treated by abdominal puncture several times a week while patients in the other group were administered diuretics either monotherapeutically or in combination. Majority of patients (86.7%) experienced no side effects after applied therapeutic protocol. 6,7% of patients experienced abdominal pain, 3.3% of them had cramps, ailment 1.7% and nausea 1.7% with no statistic difference between two groups of patients (p>0.05). Registered side effects were mild (5%) to moderate (8.3%), while only 1.7% of patients treated by abdominal puncture experienced leaking of ascitic fluid at the puncture site. Conclusion. No major statistic difference between groups of patients was recorded in relation to side effects and complications in applied ascites therapy (p>0.05). Abdominal puncture and diuretics were both equally well tolerated in hospital conditions. Potential risk in ascites therapy can be reduced to the smallest possible extent by intensive observation of the patient.

    Abstract Summary Introduction. The amniotic membrane is used in transplant surgery, ophthalmology and dermatology. Various methods have been developed to preserve amniotic membrane: hypothermic storage, cryopreservation, lyophilization. Transplantation of fresh amniotic membrane showed low inflammatory response. The efficient antibiotic solutions are carefully chosen for the hypothermic storage of amniotic membranes. The aim of this study was to compare the efficacy of two antibiotic solutions for the hypothermic amniotic membrane preservation and the structure of the amniotic membrane after the preservation process. Methods. Fifteen amniotic membranes were prepared and hypothermically stored in penicillin solution in BSS (2000 IU/ml) while the remaining fifteen in an antibiotic solution containing: benzylpenicillin (50 µg/ml), gentamicin (100 µg/ml), ciprofloxacin (200 µg/ml) and fluconazole (100 µg/ml). All amniotic membranes were microbiologically tested after preparation and after hypothermic storage for two weeks. Histological analysis of thirty amniotic membranes was performed after the process of preservation. Results. Fifteen amniotic membranes were sterile after hypothermic preservation in the penicillin solution. Also, fifteen amniotic membranes were sterile after hypothermic preservation in the solution of antibacterial agents (penicillin, gentamicin, ciprofloxacin) and antifungal agent fluconazole. The amniotic membrane had a normal structure and thickness of 35.33±11.03 μm. Conclusion. Both antibiotic solutions, one that contains only penicillin and one that contains multiple antibacterial agents and fluconazole, provide sterility of fresh hypothermically stored amniotic membranes for two weeks. In the preparation of fresh hypothermically stored amniotic membrane, a solution with multiple antibiotics is preferred. The normal tissue structure of the amniotic membrane was histologically confirmed after the process of preservation.

    Abstract Purpose: To evaluate the prognostic value of acute fluid collections (AFC) diagnosed by conventional transabdominal ultrasound in the early assessment of severity acute pancreatitis (AP).
    Methods: We studied 128 consecutive patients with AP between March 2006 and March 2011. The predictor was the number of AFC. Outcome measure was the occurrence of complications. Abdominal sonogram, contrast-enhanced CT, and pancreatitis-specific clinical and laboratory findings were performed.
    Results: AFC were associated with complications (p < 0.0001), Balthazar grade (p = 0.004), Ranson score (p < 0.0001), and the majority of clinical, radiologic, and biochemical parameters for predicting complications of AP (p < 0.05). Univariate logistic regression also revealed significant association between the number of AFC and the occurrence of complications (OR 4.4; 95% CI 2.5-7.6). After the adjustment for covariates, AFC remained prognostic for complications and a cutoff point of >1 AFC was prognostic of their occurrence with 88% sensitivity and 82% specificity.
    Conclusions: AFC are related to the clinical course of AP and can predict its severity

    Abstract We read with great interest the article by Vege et al published in issue 34 of World J Gastroenterol 2010. The article evaluates the ability of contrast-enhanced computerized tomography (CECT) to characterize the nature of peripancreatic collections found at surgery. The results of their study indicate that most of the peripancreatic collections seen on CECT in patients with severe acute pancreatitis who require operative intervention contain necrotic tissue and CECT has a limited role in differentiating various types of collections. However, there are some points that need to be addressed, including data about the stage of acute pancreatitis in which CECT was done and the time span between CECT examination and surgery.
