    Abstract Abstract Chemotherapy induced painful peripheral neuropathy (CIPN) is a common dose-limiting side effect of several chemotherapeutic agents. Despite large amounts of human and animal studies, there is no sufficiently effective pharmacological treatment for CIPN. Although reducing pain is often a focus of CIPN treatment, remarkably few analgesics have been tested for this indication in clinical trials. We conducted a systematic review and meta-analyses regarding the effects of analgesics on stimulus evoked pain-like behaviour during CIPN in animal models. This will form a scientific basis for the development of prospective human clinical trials. A comprehensive search identified forty-six studies. Risk of bias (RoB) analyses revealed that the design and conduct of the included experiments were poorly reported, and therefore RoB was unclear in most studies. Meta-analyses showed that administration of analgesics significantly increases pain threshold for mechanical (SMD: 1.68 [1.41; 1.82]) and cold (SMD: 1. 41 [0.99; 1.83]) evoked pain. Subgroup analyses revealed that dexmedetomidine, celecoxib, fentanyl, morphine, oxycodone and tramadol increased the pain threshold for mechanically evoked pain, and lidocaine and morphine for cold evoked pain. Altogether, this meta-analysis shows that there is ground to investigate the use of morphine in clinical trials. Lidocaine, dexmedetomidine, celecoxib, fentanyl, oxycodone and tramadol might be good alternatives, but more animal-based research is necessary.

    Abstract Despite the large amount of human and experimental studies no effective (prophylactic) treatment exists for chemotherapy induced peripheral neuropathy (CIPN), a disabling side effect of many cancer treatments. One of the underlying reasons for this could be that often the preclinical animal models used are not the best representation of the clinical situation. We therefore present a systematic summary and comparison of all animal models currently described in literature for CIPN focusing on stimulus evoked pain-like behaviour and neurophysiological alterations in nerve function (650 included papers, and a comparison of 183 models), that resulted in a clear overview of the most effective and robust CIPN models using an administration route used in clinical practice. Using our three-step approach (step 1: efficacy; step; 2 robustness and step 3: mimicking the clinical situation) we show that all mice CIPN models treated with either paclitaxel or cisplatin using an administration route used in clinical practice seem suitable models. Three specific models using paclitaxel or cisplatin that stand out are 1) C57BL/6 female mice receiving paclitaxel and 2) CD1 male mice receiving paclitaxel and 3) C57BL/6 male mice receiving cisplatin. This overview may help scientists selecting suitable CIPN models for their research. We hypothesize that by using effective and robust animal models that mimic the clinical situation as much as possible, the translational value of preclinical study results with respect to the potential of identifying promising treatments for CIPN in the future, will prove. The methodology described in this paper, aimed at comparing animal models, is novel and can be used by scientist in other research fields as well.

    Abstract Objective: Our study aimed to estimate the impact of addressing modifiable risk factors on the future burden of cardiovascular diseases (CVD) in the general population and in two high-risk populations (heterozygous familial hypercholesterolemia and secondary prevention) for Turkey. Methods: One model investigated the impact of reaching the World Health Organization (WHO) voluntary targets for tobacco use, hypertension, type 2 diabetes, obesity and physical inactivity in the general population. Another model estimated the impact of reducing LDL-cholesterol in two high-risk populations through increased access to effective treatment. Inputs for the models include disease and risk factor prevalence rates, a population forecast, baseline CVD event rates, and treatment effectiveness, primarily derived from the published literature. Direct costs to the public health care system and indirect costs from lost production are included, although the cost of programs and pharmacological interventions to reduce risk factors were not considered. Results: The value of reaching WHO risk factor reduction targets is estimated at US$9.3 billion over the next 20 years, while the value of reducing LDL-cholesterol is estimated at up to US$8.1 billion for high-risk secondary prevention patients and US$691 million for heterozygous familial hypercholesterolemia patients. Conclusion: Efforts to achieve WHO risk factor targets and further lower LDL-cholesterol through increased access to treatment for high-risk patients are projected to greatly reduce the growing clinical and economic burden of CVD in Turkey.

    Abstract In this study, the effect of sulfation due to air pollution in the portals of the Ince Minareli Madrasa built of pyroclastic rocks and the Karatay Madrasas (Konya, Turkey) built of marble was investigated. Since the sulfur element in sulfate salts is represented as sulfate, the sulfur contour maps (SCM), obtained by transferring the portable X-ray fluorescence measurements onto the orthophotographs, also show the sulfated crust formation, distribution and anomaly regions on the portal surfaces. Additionally, the anomaly regions originating from the difference in thermal conduction between the sulfated crusts and portal surfaces on thermal camera images were determined, and they were observed to be compatible with the anomaly regions on SCM. However, the size of this correlation depends on the chemistry of the building stone and the thickness of the sulfated crusts. Then, it was determined that the anomaly regions on SCM were distributed in relation to the washing out of sulfated crusts by rainfall and capillary moisture content. Accordingly, portal surfaces were divided into three as the sheltered, washing and capillary regions. Consequently, it was demonstrated that the sulfation effect due to air pollution on historical building surfaces which have appropriate rock chemistry can be determined by non-destructive testing methods like using the SCM supported by infrared thermography. The fact that the method is simple and repeatable and is easily applicable to other buildings was considered as the most important success of the method.

    Abstract The goal of this study was to assess the relationship of postoperative atrial fibrillation (AF) with the use of blood products and postoperative hemoglobin levels following coronary artery bypass grafting (CABG). Eighty patients undergoing isolated CABG from June 2013 to December 2013 in our institution were included in this study, and divided into two groups that contain 40 patients according to their postoperative blood hemoglobin levels in this prospective study. In Group 1 and 2, there were patients that whose postoperative hemoglobin levels were below 10 g/dL and above 10 g/dL, respectively. Groups were compared with each other according to the use of blood products and occurrence of AF, then the relationship between use of blood products and occurrence of AF was analyzed. Moreover, patients that occur postoperative AF and patients that do not occur postoperative AF were compared with each other; thus, risk factor analysis for postoperative AF were performed. Postoperative AF was observed in 12 (30%) patients in Group 1 (mean age: 62.0±9.6 years, 24 male) and 7 (17.5%) in Group 2 (mean age: 60.3±9.0 years, 27 male), but this difference was not found to be statistically significant. When patients that occurred AF and did not occur AF were compared, according to the number of patients that had received blood products, the patients that had occurred AF had received more blood products than the patients whom had not occurred AF although again this difference was not statistically significant. In risk factor analysis; advanced age, low ejection fraction, obesity and the non-use of preoperative beta blocker were found to be risk factors associated with postoperative AF. In accordance with this study, the use of blood products and postoperative hemoglobin levels do not have an important impact on AF that occurs in patients undergoing isolated CABG. However, further studies with larger patient series are needed to elucidate this relationship. [Med-Science 2019; 8(1.000): 16-20]

    Abstract Currarino syndrome (triad) is an extremely rare condition characterized by presacral mass, anorectal malformation, and sacral bone deformation. The complete form of this syndrome displays all three irregularities. Herein, we report a male case who was admitted to our hospital with symptoms of urinary system infection and persistent constipation 2 years after colostomy operation performed with the indication of rectovestibular fistula and anal atresia, diagnosed as Currarino syndrome based on imaging modalities. In a patient who was admitted because of the presence of anal atresia, in order to preclude potential complications, probable concomitancy of this syndrome should not be forgotten. Early diagnosis is important for the prevention of meningitis, urinary tract infections, and malignant change.

    Abstract Objective: This study aims to estimate the current and future burden of cardiovascular diseases (CVD) in Turkey.
    Methods: A burden-of-disease model was developed that included inputs on population growth, prevalence, and incidence of ischemic disease (IHD) and cerebrovascular disease (CeVD), prevalence of modifiable risk factors, mortality rates, and relationship between risk factors and IHD/CeVD. Direct costs to the public health-care system and indirect costs from lost production due to premature mortality, hospitalizations, disability, and absenteeism were considered.
    Results: We estimated that in 2016, 3.4 million Turkish adults were living with CVD, including 2.5 million affected by IHD, and 0.9 million by CeVD. This prevalence is projected to increase to 5.4 million by 2035. The economic burden of CVD was estimated at US$10.2 billion in 2016, projected to increase twofold to US$19.4 billion by 2035.
    Conclusion: Our study confirms that the current burden of CVD is significant, and that it is projected to increase at a steep rate over the next two decades. This growing burden of disease will likely create significant pressure on the public health-care system in the form of direct health-care costs, as well as on society in the form of lost productivity.

    Abstract Purpose: Meatal stenosis is still a common problem in tubularized incised plate urethroplasty. In this study, we aimed to seek for a relationship between the size of urethral catheter and meatal stenosis formation in children undergoing tubularized incised plate urethroplasty.
    Materials and methods: We retrospectively reviewed 83 children who underwent tubularized incised plate urethroplasty for hypospadias. The whole group was classified into the groups A and B based on the catheter size. One group (group A) consisted of 44 patients (mean age, 4.82 ± 3.83 years) with tubularized neourethra over a 6 Fr catheter, while the other group (group B) included 39 patients (mean age, 5.19 ± 3.83 years) with tubularized neourethra over a 8 Fr catheter.
    Results: There were no significant differences between the groups according to their age, location of urethral meatus, dehiscence and urethrocutaneous fistula formation. Meatal stenosis formation in group B was markedly higher than that in group A. Number of meatal dilatation was higher in group B compared to group A.
    Conclusion: We suggest that the tubularization of urethral plate over a small-sized (6 Fr) catheter, regardless of the age of the patients, prevents meatal stenosis by reducing foreign body reaction and pressure injury and by hindering secondary healing.
