    Abstract Toxic Interactions is a review of quantitative research revealing how urban living, trauma, ethnicity, stress and familial influences influence the risk of troubling psychotic experiences. Each of these is reviewed in search of their social implications, and a constructivist approach identifies their common threads. The contributions of newer psychotherapeutic approaches such as Open Dialogue and Recovery programmes are considered, and a consistent interpretation emerges; that it is not the observable features of disturbed mental states that deserve key attention, but how these are generally understood by others, and in particular the 'client's' close associates. Toxic Interactions and the Social Geography of Psychosis will be welcomed by all who find conventional approaches to mental health difficulties unsatisfactory, whether that is as a practitioner frustrated by the counter-productive expectations of their institutional setting, an academic exploring different perspectives, a 'service user' disappointed by not experiencing the care they feel is needed, or as third party perplexed by the contradictions of contemporary psychiatry"--

    Abstract Abstract: Salivary gland tumors are rare, accounting for 6% to 8% of head and neck tumors. Detecting them in primary care is important so that adequate treatment can be provided. This case report describes a 58-year-old woman whose unilateral pulsatile tinnitus and ear fullness led to the diagnosis of a salivary gland tumor, specifically a pleomorphic adenoma of the parotid gland.

    Abstract Background: Wolbachia is a widespread bacterial endosymbiont that can inhibit vector competency when stably transinfected into the mosquito, Aedes aegypti, a primary vector of the dengue virus (DENV) and other arboviruses. Although a complete mechanistic understanding of pathogen blocking is lacking, it is likely to involve host immunity induction and resource competition between Wolbachia and DENV, both of which may be impacted by microbiome composition. The potential impact of Wolbachia transinfection on host fitness is also of importance given the widespread release of mosquitos infected with the Drosophila melanogaster strain of Wolbachia (wMel) in wild populations. Here, population-level genomic data from Ae. aegypti was surveyed to establish the relationship between the density of wMel infection and the composition of the host microbiome.
    Results: Analysis of genomic data from 172 Ae. aegypti females across six populations resulted in an expanded and quantitatively refined, species-level characterization of the bacterial, archaeal, and fungal microbiome. This included 844 species of bacteria across 23 phyla, of which 54 species were found to be ubiquitous microbiome members across these populations. The density of wMel infection was highly variable between individuals and negatively correlated with microbiome diversity. Network analyses revealed wMel as a hub comprised solely of negative interactions with other bacterial species. This contrasted with the large and highly interconnected network of other microbiome species that may represent members of the midgut microbiome community in this population.
    Conclusion: Our bioinformatic survey provided a species-level characterization of Ae. aegypti microbiome composition and variation. wMel load varied substantially across populations and individuals and, importantly, wMel was a major hub of a negative interactions across the microbiome. These interactions may be an inherent consequence of heightened pathogen blocking in densely infected individuals or, alternatively, may result from antagonistic Wolbachia-incompatible bacteria that could impede the efficacy of wMel as a biological control agent in future applications. The relationship between wMel infection variation and the microbiome warrants further investigation in the context of developing wMel as a multivalent control agent against other arboviruses.     Abstract Considerations of well-being or flourishing include Maslow's and Rogers' concepts of self-actualisation and actualising tendency. Recent empirical findings suggest that only a modest proportion of the population might be considered to be flourishing. Separate findings focused upon the nature and determinants of post-traumatic growth identify it as comparable to flourishing, and facilitated by supported accommodation to the trauma. This can be understood as reflecting self-actualisation. Empirical findings such as these provide ontological stability to a set of phenomena that share much with ancient teachings extolling redemption through suffering. This framework challenges conventional healthcare policies and practices, but in ways that offer insights into how patient-centred approaches to chronic illness and disability might be better conceived and enabled. It also throws into doubt the rectitude of an economic model built around services and products designed to provide easy access to sources of immediate gratification.

    Abstract Pharmacy professionals are increasingly moving into advanced roles, including in primary care. In England, the publicly funded Pharmacy Integration Fund (PhIF) enabled employment and training of pharmacy professionals in new patient-facing roles, including general practice and care homes. In recognition of the need for support and supervision during work-based learning and building on established support structures in medicine and nursing, one of the providers of PhIF funded learning developed a supervision structure which mirrors arrangements for postgraduate medical specialty training. This paper describes what informed this supervision model, with a particular focus on educational supervision, its delivery, and the training which was developed to support supervisors. This supervision enabled pharmacy professionals moving into primary care to practise safely, manage workplace challenges, extend their roles and make progress with their education. This model illustrates the benefits of supervision in supporting post-registration learning to facilitate the development of advanced patient-facing clinical roles.

    Abstract During the COVID-19 pandemic, many refugee communities faced intensified economic and social challenges. This longitudinal study began three years prior to the COVID pandemic and examined the effects of COVID on refugee outcomes in the United States including employment, health insurance, safety, and discrimination. The study also examined participant perspectives on COVID-related challenges. Participants included 42 refugees who resettled approximately three years prior to the onset of the pandemic. Data were collected at six months, 12 months, two years, three years, and four years postarrival, with the pandemic beginning between years 3 and 4. Linear growth models examined how the pandemic impacted participant outcomes over time. Descriptive analyses examined perspectives regarding pandemic challenges. Results indicated that during the pandemic, employment and safety significantly decreased. Participant concerns regarding the pandemic centered on health, economic challenges, and isolation. Attention to refugee outcomes during the COVID pandemic highlight the need for social work practitioners to promote equitable access to information and social supports, particularly during times of uncertainty.

    Abstract Background: Demonstrating a person-centred approach in a consultation is a key component of delivering high-quality healthcare. To support development of such an approach requires training underpinned by valid assessment tools. Given the lack of a suitable pharmacy-specific tool, a new global consultation skills assessment tool: the medicines related-consultation assessment tool (MR-CAT) was designed and tested.
    Aim: This study aimed to test the validity and reliability of the MR-CAT using psychometric methods.
    Method: Psychometric testing involved analysis of participants' (n = 13) assessment of fifteen pre-recorded simulated consultations using the MR-CAT. Analysis included discriminant validity testing, intrarater and interrater reliability testing for each of the five sections of the MR-CAT and for the overall global assessment of the consultation. Analysis also included internal consistency testing for the whole tool.
    Results: Internal consistency for the overall global assessment of the consultation was good (Cronbach's alpha = 0.97). The MR-CAT discriminated well for the overall global assessment of the consultation (p < 0.001). Moderate to high intrarater reliability was observed for the overall global assessment of the consultation and for all five sections of the MR-CAT (rho = 0.64-0.84) in the test-retest analysis. Moderate to good interrater reliability (Kendall's W = 0.68-0.90) was observed for the overall global assessment of the consultation and for all five sections of the MR-CAT.
    Conclusion: The MR-CAT is a valid and reliable tool for assessing person-centred pharmacist's consultations. Moreover, its unique design means that the MR-CAT can be used in both formative and summative assessment.
