    Abstract In this paper, we present electromagnetic signal and information theory (ESIT). ESIT is an interdisciplinary scientific discipline, which amalgamates electromagnetic theory, signal processing theory, and information theory. ESIT is aimed at studying and designing physically consistent communication schemes for the transmission and processing of information in communication networks. In simple terms, ESIT can be defined as physics-aware information theory and signal processing for communications. We consider three relevant problems in contemporary communication theory, and we show how they can be tackled under the lenses of ESIT. Specifically, we focus on (i) the theoretical and practical motivations behind antenna designs based on subwavelength radiating elements and interdistances; (ii) the modeling and role played by the electromagnetic mutual coupling, and the appropriateness of multiport network theory for modeling it; and (iii) the analytical tools for unveiling the performance limits and realizing spatial multiplexing in near field, line-of-sight, channels. To exemplify the role played by ESIT and the need for electromagnetic consistency, we consider case studies related to reconfigurable intelligent surfaces and holographic surfaces, and we highlight the inconsistencies of widely utilized communication models, as opposed to communication models that originate from first electromagnetic principles.

    Abstract In this paper, we exploit the enhanced penetration reachable through inhomogeneous waves to induce hyperthermia in biological tissues. We will present a leaky-wave antenna inspired by the Menzel antenna which has been shortened through opportune design and optimizations and that has been designed to optimize the penetration at the interface with the skin, allowing penetration in the skin layer at a constant temperature, and enhanced penetration in the overall structure considered. Past papers both numerically and analytically demonstrated the possibility of reducing the attenuation that the electromagnetic waves are subject to when travelling inside a lossy medium by using inhomogeneous waves. In those papers, a structure (the leaky-wave antenna) is shown to allow the effect, but such a radiator suffers from low efficiency. Also, at the frequencies that are most used for hyperthermia application, a classical leaky-wave antenna would be too long; here is where the idea of the shortened leaky-wave arises. To numerically analyze the penetration in biological tissues, this paper considers a numerical prototype of a sample of flesh, composed of superficial skin layers, followed by fat and an undefined layer of muscles.

    Abstract Background: In a world where every municipality is pursuing the goals of more sustainable mobility, bicycles play a fundamental role in getting rid of private cars and travelling by an eco-friendly mode of transport. Additionally, private and shared bikes can be used as a feeder transit system, solving the problem of the first- and last-mile trips. Thanks to GIS (Geographic Information System) software, it is possible to evaluate the effectiveness of such a sustainable means of transport in future users’ modal choice. Methods: Running an accessibility analysis of cycling and rail transport services, the potential mobility demand attracted by these services and the possible multimodality between bicycle and rail transport systems can be assessed. Moreover, thanks to a modal choice model calibrated for high school students, it could be verified if students will be really motivated to adopt this solution for their home-to-school trips. Results: The GIS-based analysis showed that almost half of the active population in the study area might potentially abandon the use of their private car in favour of a bike and its combination with public transport systems; furthermore, the percentage of the students of one high school of Palermo, the Einstein High School, sharply increases from 1.5% up to 10.1%, thanks also to the combination with the rail transport service. Conclusions: The GIS-based methodology shows that multimodal transport can be an effective way to pursue a more sustainable mobility in cities and efficiently connect suburbs with low-frequent public transport services to the main public transport nodes.

    Abstract Introduction: Limy bile syndrome (LBS) is an unusual condition in which gallbladder and/or bile ducts are filled with paste-like radiopaque material with a high calcium carbonate content. It can be rarely associated with PTH disorder and hypercalcemia.
    Presentation of case: A 35-year-old woman presented with epigastric and right hypochondrium pain since a few hours. Similar attacks occurred in the past months soon after a pregnancy with vaginal delivery. Laboratory findings were not significant. The abdominal ultrasound highlighted a micro-lithiasis of gallbladder without complications. Considering the recurrent biliary attacks, laparoscopic cholecystectomy was performed with intraoperative diagnosis of LBS. A subsequent endocrinological screening highlighted a normocalcemic hyperparathyroidism associated with Vitamin D deficiency, likely related to the recent pregnancy and not to LBS.
    Discussion: LBS is a rare condition with not clear etiology, frequently associated with cholelithiasis, of which it shares clinical presentation and potential complications. Diagnosis of LBS is based on abdominal X-ray/computed tomography scan, or it could be an intraoperative finding. The gold standard treatment is represented by laparoscopic cholecystectomy. The pregnancy with its related cholestatic phenotype could facilitate the LBS manifestation. An endocrinological screening should be performed to rule out a concomitant calcium metabolism disorder.
    Conclusion: Knowledge of this rare condition could help general surgeons handle it properly.

    Abstract Bicyclic aryl furano pyrimidines represent the most potent anti-varicella zoster virus (VZV) agents reported to date. Lead compounds have 50% effective concentration (EC(50)) values in vitro that are in the subnanomolar range and selectivity index values that exceed 1 million. They have an absolute requirement for VZV thymidine kinase and most likely act as their phosphate forms. Some structural modification (such as aryl substitution in the base moiety) is tolerated, whereas little sugar modification is acceptable. The Cf1743 compound has proved to be significantly more potent than all reference anti-VZV compounds, as measured either by inhibition of infectious virus particles and/or viral DNA production; however, the high lipophilicity and very low water solubility of this compound gives poor oral bioavailability (<14%). Use of the modified cyclodextrin captisol and the synthesis of the 5'-monophosphate prodrug of Cf1743 has significantly improved water solubility, but does not give any enhancement in oral bioavailability. By contrast, the synthesis of the ether series does not give any further improvement in terms of solubility. The most promising prodrug to emerge to date is the hydrochloric salt of the 5'-valyl-ester, designated as FV-100. Its uptake into cells has been studied using fluorescent microscopy and biological assays, which have indicated that the compound is efficiently taken up by the cells after a short period of incubation.

    Abstract We consider a line-of-sight communication link between two holographic surfaces (HoloSs). We provide a closed-form expression for the number of effective degrees of freedom (eDoF), i.e., the number of orthogonal communication modes that can be established between the HoloSs. The framework can be applied to general network deployments beyond the widely studied paraxial setting. This is obtained by utilizing a quartic approximation for the wavefront of the electromagnetic waves, and by proving that the number of eDoF corresponds to an instance of Landau's eigenvalue problem applied to a bandlimited kernel determined by the quartic approximation of the wavefront. The proposed approach overcomes the limitations of the widely utilized parabolic approximation for the wavefront, which provides inaccurate estimates in non-paraxial deployments. We specialize the framework to typical network deployments, and provide analytical expressions for the optimal, according to Kolmogorov's $N$-width criterion, basis functions (communication waveforms) for optimal data encoding and decoding. With the aid of numerical analysis, we validate the accuracy of the closed-form expressions for the number of eDoF and waveforms.

    Abstract A novel probabilistic sparsity-promoting method for robust near-field (NF) antenna characterization is proposed. It leverages on the measurements-by-design (MebD) paradigm and it exploits some a-priori information on the antenna under test (AUT) to generate an over-complete representation basis. Accordingly, the problem at hand is reformulated in a compressive sensing (CS) framework as the retrieval of a maximally-sparse distribution (with respect to the overcomplete basis) from a reduced set of measured data and then it is solved by means of a Bayesian strategy. Representative numerical results are presented to, also comparatively, assess the effectiveness of the proposed approach in reducing the "burden/cost" of the acquisition process as well as to mitigate (possible) truncation errors when dealing with space-constrained probing systems.

    Abstract Background: Multiport laparoscopic cholecystectomy (MLC) is the gold standard technique for cholecystectomy. In order to reduce postoperative pain and improve cosmetic results, the application of the single-incision laparoscopic cholecystectomy (SILC) technique was introduced, leading surgeons to face important challenges. Robotic technology has been proposed to overcome some of these limitations. The purpose of this review is to assess the safety of single-incision robotic cholecystectomy (SIRC) for benign disease.
    Methods: An Embase and Pubmed literature search was performed in February 2017. Randomized controlled trial and prospective observational studies were selected and assessed using PRISMA recommendations. Primary outcome was overall postoperative complication rate. Secondary outcomes were postoperative bile leak rate, total conversion rate, operative time, wound complication rate, postoperative hospital stay, and port site hernia rate. The outcomes were analyzed in Forest plots based on fixed and random effects model. Heterogeneity was assessed using the I
    Results: A total of 13 studies provided data about 1010 patients who underwent to SIRC for benign disease of gallbladder. Overall postoperative complications rate was 11.6% but only 4/1010 (0.4%) patients required further surgery. A postoperative bile leak was reported in 3/950 patients (0.3%). Conversion occurred in 4.2% of patients. Mean operative time was 86.7 min including an average of 42 min should be added as for robotic console time. Wound complications occurred in 3.7% of patients. Median postoperative hospital stay was 1 day. Port site hernia at the latest follow-up available was reported in 5.2% of patients.
    Conclusions: The use of the Da Vinci robot in single-port cholecystectomy seems to have similar results in terms of incidence and grade of complications compared to standard laparoscopy. In addition, it seems affected by the same limitations of single-port surgery, consisting of an increased operative time and incidence of port site hernia.

    Abstract A method to filter out the contribution of interference sources in array diagnosis is proposed. The interference-affected near field measured on a surface is treated as a (complex-data) image. This allows to use some modern image processing algorithms. In particular, two strategies widely used in image processing are applied. The first one is the reduction of the amount of information by acquiring only the innovation part of an image, as currently happens in video processing. More specifically, a differential measurement technique is used to formulate the estimation of the array excitations as a sparse recovery problem. The second technique has been recently proposed in video denoising, where the image is split into a low-rank and high-rank part. In particular, in this paper the interference field is filtered out using sparsity as discriminant adopting a mixed minimum ℓ 1 norm and trace norm minimization algorithm. The methodology can be applied to both near and far field measurement ranges. It could be an alternative to the systematic use of anechoic chambers for antenna array testing.
