    Abstract Recently, the remarkable success of chimeric antigen receptor T cell (CAR-T) therapy in treating certain tumors has led to numerous studies exploring its potential application to treat non-oncology diseases. This review discusses the progress and evolution of CAR-T cell therapies for treating non-oncology diseases over the past 5 years. Additionally, we summarize the advantages and disadvantages of CAR-T cell therapy in treating non-oncological diseases and identify any difficulties that should be overcome. After conducting an in-depth analysis of the most recent studies on CAR-T technology, we discuss the key elements of CAR-T therapy, such as developing an effective CAR design for non-oncological diseases, controlling the rate and duration of response, and implementing safety measures to reduce toxicity. These studies provide new insights into different delivery strategies, the discovery of new target molecules, and improvements in the safety of CAR-T therapy for non-oncological diseases.

    Abstract Monocytes are circulating myeloid cells that are derived from dedicated progenitors in the bone marrow. Originally thought of as mere precursors for the replacement of tissue macrophages, it is increasingly clear that monocytes execute distinct effector functions and may give rise to monocyte-derived cells with unique properties from tissue-resident macrophages. Recently, the advent of novel experimental approaches such as single-cell analysis and fate-mapping tools has uncovered an astonishing display of monocyte plasticity and heterogeneity, which we believe has emerged as a key theme in the field of monocyte biology in the last decade. Monocyte heterogeneity is now recognized to develop as early as the progenitor stage through specific imprinting mechanisms, giving rise to specialized effector cells in the tissue. At the same time, monocytes must overcome their susceptibility towards cellular death to persist as monocyte-derived cells in the tissues. Environmental signals that preserve their heterogenic phenotypes and govern their eventual fates remain incompletely understood. In this review, we will summarize recent advances on the developmental trajectory of monocytes and discuss emerging concepts that contributes to the burgeoning field of monocyte plasticity and heterogeneity.

    Abstract Accurate real-time rotor position is indispensable for switched reluctance motors (SRM) speed and torque control. Traditional position sensors add complexity and potential failure risk to the system. Owing to the added advantages of high stability and low cost, the position sensorless method of SRMs has been extensively studied to advance its use in vehicles and construction machinery. This paper presents an overview of position sensorless control techniques from the perspective of whether the method requires the establishment of complex mathematical models. Various types of methods are compared for performance, such as speed regulation range, algorithm complexity, and requirement of the pre-stored parameter. A discussion is presented concerning current trends in technological development, which will facilitate the research addressing potentially effective methods for position estimation in SRM drive systems.

    Abstract Efferocytosis is the effective clearance of apoptotic cells by professional and non-professional phagocytes. The process is mechanically different from other forms of phagocytosis and involves the localization, binding, internalization, and degradation of apoptotic cells. Defective efferocytosis has been demonstrated to associate with the pathogenesis of various inflammatory disorders. In the current review, we summarize recent findings with regard to efferocytosis networks and discuss the relationship between efferocytosis and different immune cell populations, as well as describe how efferocytosis helps resolve inflammatory response and modulate immune balance. Our knowledge so far about efferocytosis suggests that it may be a useful target in the treatment of numerous inflammatory diseases.

    Abstract Transition-metal-catalysed carbene insertion reaction is a straightforward and efficient protocol for the construction of carbon-carbon or carbon-heteroatom bonds. Compared to the intensively studied and well-established "common" carbene insertion reactions, including carbene insertion into C-H, Si-H, N-H, O-H, and S-H bonds, several "uncommon" carbene insertion reactions, including carbene insertion into B-H, Sn-H, Ge-H, P-H, F-H, C-C, and M-M bonds, have been neglected for a long time. However, more and more studies on uncommon carbene insertion reactions have been disclosed recently, and clearly demonstrate the great synthetic potential of these reactions. The current perspective reviews the history and the newest advances of uncommon carbene insertion reactions, discusses their potential applications and challenges, and also presents an outlook of this promising field.

    Abstract Aim: We aimed to compare the outcomes of different therapeutic modalities in rectal carcinoid tumors.
    Method: We retrospectively collected 145 patients with rectal carcinoid tumors which were pathologically diagnosed from 2005/01/01 to 2016/12/31. We compared tumor size, complete resection rate and recurrent rate between different therapeutic modalities. Then, prospectively compared the treatment outcomes of 28 patients treated with ligation assisted endoscopic mucosal resection (LEMR) and 25 patients treated with endoscopic mucosal resection with cap (EMRC).
    Result: The mean size of tumors was 6.5 mm (1-25 mm), and the mean follow-up duration was 26 months (6-118 months). The therapeutic modalities included ligation-assisted endoscopic mucosal resection (LEMR) (25 tumors, 17%), endoscopic mucosal resection (EMR) (31 tumors, 21%), snare polypectomy (30 tumors, 21%), biopsy forceps removal (46 tumors, 32%) and surgical resection (13 tumors, 11%), including 6 tumors treated with transanal endoscopic microsurgery (TEM) method. In view of pathologically complete resection rate, LEMR was highest (100%), followed by surgical resection (85%). However, EMR only had 42% pathologically complete resection rate. Besides, LEMR and surgical resection had no local recurrence and significantly higher clinically complete resection rate, compared to other treatments. For the further prospective study, complete resection was noted in 28 (100%) patients in LEMR group and 13 (52%) patients in EMRC group.
    Conclusion: In the treatment of rectal carcinoid tumors, LEMR is safe and effective compared with traditional endoscopic treatments.

    Abstract Background: Universal Health Coverage (UHC) has received much attention and many countries are striving to achieve it. The Southeast Asian region, in particular, comprises many developing countries with limited resources, exacerbating challenges around attaining UHC. This paper aims to specifically explore the health financing challenges these countries face in achieving UHC via a systematic review approach and formulate recommendations that will be useful for policymakers.
    Methods: The systematic review followed the guidelines as recommended by PRISMA. The narrative synthesis approach was used for data synthesis, followed by identifying common themes.
    Results: The initial search returned 160 articles, and 32 articles were included after the screening process. The identified challenges in health financing towards achieving UHC in the Southeast Asian region are categorised into six main themes, namely (1) Unsustainability of revenue-raising methods, (2) Fragmented health insurance schemes, (3) Incongruity between insurance benefits and people's needs, (4) Political and legislative indifference, (5) Intractable and rapidly rising healthcare cost, (6) Morally reprehensible behaviours.
    Conclusions: The challenges identified are diverse and therefore require a multifaceted approach. Regional collaborative efforts between countries will play an essential role in the progress towards UHC and in narrowing the inequity gap. At the national level, individual countries must work towards sustainable health financing strategies by leveraging innovative digital technologies and constantly adapting to dynamic health trends.

    Abstract Objectives. This study aims at investigating the internal heat acupuncture (IHA) combined with the high-voltage long-duration pulsed radiofrequency (PRF) therapeutic effect on subacute postherpetic neuralgia (PHN). Methods. This retrospective study comprised 81 cases with PHN. They were divided into three groups: IHA combined with the high-voltage long-duration PRF group (IHA-PRF), intradermal injection combined with the high-voltage long-duration PRF group (II-PRF), and the high-voltage long-duration PRF group. The pain numerical rating score (NRS), IL-6, Gal-3, and blood glucose levels were recorded before and after treatment. Results. Compared with before treatment, NRS scores of the three groups were all decreased at each time point. NRS scores of the IHA-PRF group patients decreased significantly in comparison to the PRF group at 1, 4, 8, and 12 weeks following treatment, while group II-PRF only decreased significantly at one week following treatment. Compared with groups II-PRF and PRF, respectively, IL-6 and Gal-3 levels in plasma of patients in group IHA-PRF were significantly decreased at 4 and 12 weeks after treatment. The effective rate of group IHA-PRF was 88.9%, which was considerably more than the other groups, II-PRF (63.0%) and PRF (63.0%). Compared with group II-PRF, patients’ blood glucose levels in IHA-PRF and PRF groups significantly decreased three days and one week after treatment. Conclusion. Internal heat acupuncture combined with high-voltage long-duration pulsed radiofrequency has a satisfactory therapeutic effect on subacute PHN and has no obvious adverse reactions, which is especially suitable for patients with poor blood glucose control.

    Abstract The primary objective is to find out the epidemiology, characteristics, preparedness, and survival of elderly patients initiated on dialysis from a tertiary center in Sabah. All newly confirmed patients aged 65 and above who were initiated on renal replacement therapy (RRT) from January 1, 2014, to December 31, 2017, were included. There were two groups of patients: Group 1 – optimally prepared for RRT; Group 2 – suboptimally prepared for RRT. Survival outcome was recorded at the 3rd-, 6th-, and 12th-month interval. Diabetes mellitus accounted for majority of the primary cause of end-stage renal disease, 67.3%. The median duration of follow-up was 550 days (interquartile range = 787). There were 81 patients (30.5%) in Group 1. 182 patients (69.2%) in Group 2 were considered as suboptimally prepared for RRT. The survival of this elderly group was 85.6% at 3 months, 74.5% at six months, and 65% at 1 year. Group 2 patients had much poorer survival outcome compared to Group 1 (56% vs. 85.2% at one year, P <0.001). Age and RRT preparation appeared to be associated with the survival outcome. Decision in initiating dialysis in this group of patients is not as straightforward as in the younger age groups. A more realistic view on the actual survival, as well as consideration regarding the quality of life after the initiation of dialysis, needs to be taken into account. If dialysis therapy is decided, early preparation is important.
