    Abstract This study proposes a novel multiscale framework for investigating streamflow teleconnections employing multivariate Empirical Mode decomposition (MEMD) based Time Dependent Intrinsic Correlation (TDIC). The MEMD-TDIC framework is applied for investigating the teleconnections of streamflows of 1986-2016 period recorded at Kidangoor, Malakkara, Thumpamon and Kallooppara stations of Greater Pamba Basin (GPB) of Southern India. The newly proposed quantitative measure of Mean Significant Correlation (MSC) stemming from TDIC method displayed strong agreement with the coherence measure rooted in Continuous Wavelet Transform (CWT). MSC measure showed a temporal heterogeneity in the dominancy of associations in all the five streamflow-climatic oscillation (CO) links considered. The study found that there is spatial diversity in the CO deciphering streamflow variability within the basin with the dominancy of Indian Ocean Dipole (IOD). The temporal evolution of residuals of COs matches well with that of streamflows of Kidangoor and Malakkara stations with a very high correlation of more than 0.98 irrespective of the CO. A moderate correlation of 0.73 - 0.87 range noted in other stations indicated the control by multiple oscillations in the streamflow variability. Moreover, the nature and strength displayed a dynamic relationship across both the time scale and time domain in the streamflow-CO correlations of all the stations. MEMD has the potential in aligning the common scale from different variables, while TDIC ensures the stationarity within sliding windows of running correlation analysis. The proposed approach could help in developing promising hybrid decomposition models for streamflow forecasting with improved accuracy in capturing the extremes.

    Abstract Tibial periprosthetic fractures are rare but present a complicated problem for orthopaedic surgeons. Literature in relation to combination-type periprosthetic fractures is extremely scarce, and there is limited guidance available on its treatment. We report the case of a woman in her 60s, whose radiographs revealed a periprosthetic fracture of the tibia, which was a Felix type (II+IV)A fracture. The tibial tuberosity fragment was fixed with a cannulated cancellous screw. The fracture adjacent to the stem was managed conservatively as the prosthesis was stable. Follow-up radiographs revealed both fractures to have healed completely by 12 weeks. The patient returned to preinjury functional levels by 4 months. Our demonstrated treatment of a Felix type II+IV periprosthetic fracture could be a viable treatment option for such fractures.

    Abstract Radial longitudinal deficiency (RLD) is commonly associated with thumb hypoplasia. The association between RLD and radial polydactyly (RP) is uncommon, but case reports or case series have been reported. We report our experience of managing patients with this association. A total of 97 patients with RLD were seen in our department, of which six were children with concomitant RLD and RP. Four children had both RLD and RP in the same limb; of them, three also had RLD in the contralateral limb. The mean age at presentation was 11.6 months. Awareness of this association alerts the clinician to look for RLD in the presence of RP and vice versa. This case series supports recent experimental and clinical evidence that RP and RLD may be part of the same developmental spectrum. Further studies may guide its inclusion as a possible new category in the Oberg-Manske-Tonkin (OMT) classification of congenital upper-limb anomalies.

    Abstract Background: Keratoplasty after healed herpes simplex viral keratitis is a challenge due to problems arising preoperatively, intraoperatively, and postoperatively.
    Purpose: In this video, we describe the necessary challenges and the steps, which can be taken to prevent and manage those cases of healed herpes simplex virus (HSV) keratitis that would require a keratoplasty.
    Synopsis: The video talks about both the typical and atypical features of HSV keratitis, clinical examination, the scenarios which would require a keratoplasty, problems arising intraoperatively and how to manage them, and finally how to approach these high-risk grafts postoperatively.
    Highlights: Our video highlights the diagnosis of HSV keratitis, which cases are ready for surgery, and preoperative, intraoperative, and postoperative considerations before corneal transplantation in healed HSV keratitis. Decision-making before corneal transplant in HSV grafts can become more structured if these points are followed.
