    Abstract We aim at modeling the main crustal and thermal interfaces of Sicily (Italy), a key area for understanding the geological complexity at the collisional boundary between the African and European plates. To this end, we analyze the gravity and magnetic fields, integrated with information from well logs, geology, heat flow, and seismic data. In order to make the most accurate description of the crustal structure of the area, we modeled with different methodologies the carbonate and crystalline top surfaces, as well as the Moho and the Curie isotherm surface. The reconstruction of the carbonate platform is achieved using a nonlinear 3D method constrained by the available seismic and borehole data. The crystalline top, the Curie, and the Moho are instead estimated by spectral analysis of both gravity and magnetic data. The results show a complex carbonate basement and a deep crystalline crust in central Sicily, with a prominent uplift beneath the Hyblean Plateau. Maps of the Moho and the Curie isotherm surface define a variable thermal and structural setting of Sicily, with very thin crust in the southern and eastern sectors, where high heat flow is found, and deep and cold crust below the Caltanissetta Basin.

    Abstract Hemophagocytic lymphohistiocytosis (HLH) is a rare and life-threatening condition characterized by uncontrolled activation of the immune system. HLH is a reactive mononuclear phagocytic response that occurs in association with a constellation of conditions such as malignancies and infections. The clinical diagnosis of HLH remains challenging because HLH can present with symptoms that significantly overlap with other causes of cytopenia, such as sepsis, autoimmune diseases, hematological cancers, and multiorgan failure. A 50-year-old man went to the emergency room (ER) for hyperchromic urine, melena, gingivorrhagia, and spontaneous abdominal wall hematomas. The first blood tests showed severe thrombocytopenia, alteration of the INR, and consumption of fibrinogen, and therefore, a diagnosis of disseminated intravascular coagulation (DIC) was made. A bone marrow aspirate showed numerous images of hemophagocytosis. With the suspicion of immune-mediated cytopenia, oral etoposide, intravenous immunoglobulin, and intravenous methylprednisolone were administered. Then, a diagnosis of gastric carcinoma was performed with a lymph node biopsy and gastroscopy. On the 30th day, the patient was transferred to the oncology ward of another hospital. On admission, he had serious piastrinopenia, anemia, hypertriglyceridemia, and hyperferritinemia. He was supported with a platelet transfusion and underwent a bone biopsy that showed a picture compatible with myelophthisis from diffuse medullary localization of a carcinoma of gastric origin. A diagnosis of HLH secondary to solid neoplasm was formulated. The patient started chemotherapy with oxaliplatin, calcium levofolinate, 5-fluorouracil bolus, 5-fluorouracil for 48 h (mFOLFOX6), and methylprednisolone. Six days after the third cycle of mFOLFOX6, the patient was discharged with the stabilization of his piastrinopenia condition. The patient continued chemotherapy with an improvement in his clinical conditions and normalization of hematological values. After 12 cycles of mFOLFOX, it was decided to start maintenance chemotherapy with capecitabine but, unfortunately, after only one cycle, HLH reappeared. The oncologist has to keep in mind the existence of HLH when there is an unusual clinical presentation of cancer, such as cytopenia affecting ≥2 lineages and alterations of ferritin and triglycerides other than fibrinogen and coagulation. Increased attention and additional research as well as a close collaboration with hematologists are needed to benefit patients with solid tumors complicated by HLH.

    Abstract 3D imaging is a powerful tool of high resolution and non-destructive imaging technology for the study of ancient weapons and military technology, which reveals the original microstructures and corrosion patterns that threaten these artefacts. Here we report quantitative analysis of the 3D distribution and the orientation of fractures, and uncorroded metal particles within a wrought iron javelin unearthed at the Phoenician-Punic site of Motya, Italy. The study aimed to gain a better understanding of the relationship between corrosion and local stresses within the artifact and to evaluate its manufacturing technology, as well as the effects of post-treatment with Paraloid B72 on concretion and mineralized layers. The cracks were quantified in terms of content, size, and orientation. The condition of artefact storage was evaluated by a multi-analytical approach, including X-ray microscopy, field emission electron microscopy and micro-Raman spectroscopy. The results indicated that a specific technique was used to create a sturdy, lightweight javelin with a central shaft for piercing or thrusting. The fractures appear elongated in the direction of the longitudinal axis of the blade, showing the forging direction of the original metallic block. The study concluded that the artifact had not yet been stabilized due to the presence of lepidocrocite.

    Abstract Two separate goals should be jointly pursued in wastewater treatment: nutrient removal and energy conservation. An efficient controller performance should cope with process uncertainties, seasonal variations and process nonlinearities. This paper describes the design and testing of a model predictive controller (MPC) based on neuro-fuzzy techniques that is capable of estimating the main process variables and providing the right amount of aeration to achieve an efficient and economical operation. This algorithm has been field tested on a large-scale municipal wastewater treatment plant of about 500,000 PE, with encouraging results in terms of better effluent quality and energy savings.

    Abstract Low-to-medium temperature fluid reservoirs hosted in carbonate rocks are some of the most promising and unknown geothermal systems. Western Sicily is considered a key exploration area. This paper illustrate a multidisciplinary and integrated review of the existing geological, geochemical and geophysical data, mainly acquired during oil and gas explorations since the 1950s, specifically re-analyzed for geothermal purposes, has led to understanding the western Sicily geothermal system as a whole, and to reconstructing the modalities and particular features of the deep fluid circulation within the regional reservoir. The data review suggests the presence of wide groundwater flow systems in the reservoir beneath impervious cap rocks. We identified the main recharge areas, reconstructed the temperature distribution at depth, recognized zones of convective geothermal flow, and depicted the main geothermal fluid flow paths within the reservoir.We believe that our reconstruction of geothermal fluid circulation is an example of the general behavior of low-to-medium enthalpy geothermal systems hosted in carbonate units on a regional scale. Due to the recent technological developments of binary plants, these systems have become more profitable, not only for geothermal direct uses but also for power production.

    Abstract Collective movement of social groups requires coordination between individuals. When cohesion is imperative, consensus must be reached, and specific individuals may exert disproportionate influence during decision-making. Animals living in multi-level societies, however, often split into consistent social subunits during travel, which may impact group coordination processes. We studied collective movement in the socially tolerant multi-level society of Guinea baboons (Papio papio). Using 146 group departures and 100 group progressions from 131 Guinea baboons ranging in Senegal's Niokolo-Koba National Park, we examined individual success at initiating group departures and position within progressions. Two-thirds of attempted departures were initiated by adult males and one third by adult females. Both sexes were equally successful at initiating departures (> 80% of initiations). During group progressions, bachelor males were predominantly found in front, while reproductively active 'primary' males and females were observed with similar frequency across the whole group. The pattern of collective movement in Guinea baboons was more similar to those described for baboons living in uni-level societies than to hamadryas baboons, the only other multi-level baboon species, where males initiate and decide almost all group departures. Social organization alone therefore does not determine which category of individuals influence group coordination.

    Abstract Bile acid malabsorption (BAM) represents a common cause of chronic diarrhea whose prevalence is under-investigated. We reviewed the evidence available regarding the pathophysiology and clinical management of bile acid diarrhea (BAD). BAD results from dysregulation of the enterohepatic recirculation of bile acids. It has been estimated that 25-33% of patients with functional diarrhea and irritable bowel syndrome with diarrhea have BAM. Currently, the selenium homotaurocholic acid test is the gold standard for BAD diagnosis and severity assessment. However, it is an expensive method and not widely available. The validation of the utility in the clinical practice of several other serum markers, such as 7α-hydroxy-4-cholesten-3-one (C4) and the fibroblast growth factor 19 (FGF19) is ongoing. The first-line treatment of patients with BAD is bile acid sequestrants. Patients that are refractory to first-line therapy should undergo further diagnostics to confirm the diagnosis and to treat the underlying cause of BAD. An early and correct diagnosis of BAD would improve patient's quality of life, avoiding additional diagnostic tests that burden health care systems. Considering the limited availability and tolerability of specific medications for BAD treatment, future research is awaited to identify other therapeutic approaches, such as gut microbiota modulating therapies.

    Abstract Background and aims: Perioperative treatment is currently the gold standard approach in Europe for locally advanced gastric cancer (GC). Unfortunately, the phenomenon of patients dropping out of treatment has been frequently observed. The primary aims of this study were to verify if routine blood parameters, inflammatory response markers, sarcopenia, and the depletion of adipose tissues were associated with compliance to neoadjuvant/perioperative chemotherapy.
    Methods and study design: Blood samples were considered before the first and second cycles of chemotherapy. Sarcopenia and adipose indices were calculated with a CT scan before starting chemotherapy and before surgery. Odds ratios (OR) from univariable and multivariable models were calculated with a 95% confidence interval (95% CI).
    Results: A total of 84 patients with locally advanced GC were identified between September 2010 and January 2021. Forty-four patients (52.4%) did not complete the treatment according to the number of cycles planned/performed. Eight patients (9.5%) decided to suspend chemotherapy, seven patients (8.3%) discontinued because of clinical decisions, fourteen patients (16.7%) discontinued because of toxicity and fifteen patients (17.9%) discontinued for miscellaneous causes. Seventy-nine (94%) out of eighty-four patients underwent gastrectomy, with four patients having surgical complications, which led to a suspension of treatment. Sarcopenia was present in 38 patients (50.7%) before chemotherapy began, while it was present in 47 patients (60%) at the CT scan before the gastrectomy. At the univariable analysis, patients with basal platelet to lymphocyte ratio (PLR) ≥ 152 (
    Conclusions: Changes in PLR values which tend to increase more than the cut-off seem to be an immediate indicator of incomplete compliance with neoadjuvant/perioperative treatment. Fat loss and sarcopenia do not appear to be related to compliance. More information is needed to reduce the causes of interruption.
