    Expert opinion: Poor compliance is correlated with significantly lower cure rates, and this is a unique modifiable source of H. pylori treatment failure. Eradication treatments have become complex, involving multiple drugs and dosing intervals. Thus, patient education is crucial; doctors must explain to the patient about potential temporary and most often harmless side effects.

    Title translation New advances in hereditary colorectal cancer.
    Abstract Colorectal cancer is the most frequent malignancy in both sexes in Spain. Between 20% and 25% of affected individuals have a family history of the disease, and 5% to 6% have a germ mutation, i.e. the disease develops in the context of a hereditary syndrome. The importance of identifying patients with hereditary syndromes predisposing them to colorectal cancer lies in the possibility of applying preventive measures, screening, and more appropriate management of both patients and their families. The present article outlines the most important studies presented at the congress of the American Gastroenterological Association.

    Abstract The adenoma-carcinoma sequence is currently well established, and therefore, the potential for adenomas to progress to colorectal cancer (CRC). In the last decade, there has been a change in the paradigm that considered these lesions as the only precursors of CRC and nowadays serrated polyps are also considered to have the potential for malignant transformation. The studies presented this year at the congress of the American Gastroenterological Association provide important information to improve the characterization of CRC precursor lesions, especially as regards their detection, prevalence and treatment and surveillance strategies. Notable among these studies were those focussing on evaluating the epidemiological characteristics of serrated polyps and those attempting to improve the quality of colonoscopy -with the ultimate aim of achieving greater detection of these precursor lesions- by evaluating colon cleansing, the polyp detection rate, withdrawal times, the use of endoscopic caps, narrow band imaging, high-definition endoscopy and, lastly, adequate endoscopic surveillance.

    Abstract Pancreatic cancer (PC) is a highly lethal disease, mostly incurable when detected. Thus, despite advances in PC treatments, only around 7% of patients survive 5-years after diagnosis. This morbid outcome is secondary to multifactorial reasons, such as late-stage diagnosis, rapid progression and minimal response to chemotherapy. Based on these factors, it is of special relevance to identify PC high-risk individuals in order to establish preventive and early detection measures. Although most PC are sporadic, approximately 10% cases have a familial basis. No main causative gene of PC has been identified but several known germline pathogenic mutations are related with an increased risk of this tumor. These inherited cancer syndromes represent 3% of all PC. On the other hand, in 7% of cases of PC, there is a strong family history without a causative germline mutation, a situation known as familial pancreatic cancer (FPC). In recent years, there is increasing evidence supporting the benefit of genetic germline analysis in PC patients, and periodic pancreatic screening in PC high-risk patients (mainly those with a lifetime risk greater than 5%), although there is no general agreement in the group of patients and individuals to study and screen. In the present review, we expose an update in the field of hereditary and FPC, with the aim of describing the current strategies and implications in genetic counseling, surveillance and therapeutic interventions.

    Abstract Hyperplastic polyps of the colon have traditionally been considered benign lesions without risk of progression to malignancy. However, hyperplastic polyps are only part of the spectrum of polyps known as serrated polyps. These polyps, which share a common sawtooth appearance, include distinct subtypes of lesions that can potentially undergo malignant transformation to colorectal carcinoma through the serrated pathway. Therefore, to prevent this process, affected patients must undergo surveillance and lesions must be identified and resected. Studies presented this year at the congress of the American Gastroenterological Association have provided useful information to better characterize these lesions -particularly important for the prevention of colorectal cancer- and to establish appropriate surveillance strategies.

    Abstract Colorectal cancer (CRC) is one of the most frequent neoplasia in developed countries and the second leading cause of cancer death in Spain. It develops through the accumulation of genetic and epigenetic alterations in the normal mucosa, producing advanced adenomas and invasive cancers. This multistep process makes CRC highly traceable, making it a good candidate for the implementation of preventive measures such as screening programs. Although there are different strategies, none of them meet all the criteria for CRC screening purposes. In this context, many molecular alterations involved in CRC carcinogenesis could contribute to the generation of new non-invasive methods with a high sensitivity and specificity. In this review, some of the most promising diagnostic non-invasive biomarkers for CRC will be briefly discussed.

    Abstract Gastric cancer (GC) is a major public health problem worldwide, with high mortality rates due to late diagnosis and limited treatment options. Biomarker research is essential to improve the early detection of GC. Technological advances and research methodologies have improved diagnostic tools, identifying several potential biomarkers for GC, including microRNA, DNA methylation markers, and protein-based biomarkers. Although most studies have focused on identifying biomarkers in biofluids, the low specificity of these markers has limited their use in clinical practice. This is because many cancers share similar alterations and biomarkers, so obtaining them from the site of disease origin could yield more specific results. As a result, recent research efforts have shifted towards exploring gastric juice (GJ) as an alternative source for biomarker identification. Since GJ is a waste product during a gastroscopic examination, it could provide a "liquid biopsy" enriched with disease-specific biomarkers generated directly at the damaged site. Furthermore, as it contains secretions from the stomach lining, it could reflect changes associated with the developmental stage of GC. This narrative review describes some potential biomarkers for gastric cancer screening identified in gastric juice.

    Abstract Introduction: Permanent liquor diversion is associated with a high risk of failure and often requires re-intervention. The ventriculo-gallbladder shunt (VGS) has been recognized as a last-resort alternative for treating hydrocephalus when the peritoneum or other distal sites are no longer suitable for receiving shunts. This article aims to report a case from a neurosurgery referral service in Brazil and review the literature on this issue.
    Methods: A systematic literature review was conducted in accordance with the PRISMA statement. The PubMed, Embase, and Web of Science databases were searched for data screening and extraction. The reported case was conducted with ethical approval from the neurosurgical hospital's ethics committee.
    Results: G.B.S, male, 43 years old, no comorbidities, who has been dealing with a 12-year history of hydrocephalus, with post-surgical chronic fungal meningitis. Two years ago, he underwent a ventriculoatrial shunt (VAS) placement due to multiple ventriculoperitoneal shunt (VPS) failures. Endocarditis was suspected, and the VAS was removed. As an alternative, VGS was implanted 6 months ago, and since then, there has been no need for a new system review. The gallbladder has an absorptive capacity of 1500 cc of liquid daily, which is more than the normal daily production of cerebrospinal fluid (CSF). Therefore, it is a good alternative when the ventriculoperitoneal shunt is not feasible due to postsurgical peritoneal adhesions or when there are contraindications for ventriculoatrial shunts.
    Conclusion: VGS is an alternative for patients who cannot undergo the most common surgical interventions, such as VPS.
