    Abstract Çağımızın yeni fenomeni sosyal medya tüm dünyada kişi ve kuruluşlar tarafından yoğun ve yaygın biçimde kullanılmaktadır. Bu yeni iletişim ve paylaşım platformu, kullanıcılarına yeni fırsatlar sunmakla beraber bazı tehditler de barındırmaktadır. Dolayısıyla sosyal medya hesaplarının işletim, yönetim ve denetimi önemli hale gelmekte ve bir sosyal medya politikası oluşturma ihtiyacı ortaya çıkmaktadır. Bu bağlamda makalede; Türkiye’deki üniversite kütüphanelerinin bir sosyal medya politikasına sahip olup olmadığı saptanmış ve kütüphanecilerin sosyal medya politikasını birihtiyaç olarak görüp görmediğine ilişkin farkındalık düzeyleri belirlenmeye çalışılmıştır. Araştırma kapsamında sosyal medyayı kullandığı tespit edilen 90 üniversite kütüphanesine anket uygulanmış ve ankete katılan 67 (% 74,4) üniversite kütüphanesinin yanıtları değerlendirilmiştir. Araştırma sonucunda ankete katılan kütüphanelerin yalnızca %7,4’ünün kütüphane için oluşturulmuş bir sosyal medya politikası bulunduğu ve kütüphanecilerin çoğunlukla () kütüphaneler tarafından sosyal medya politikası vb. bir belgenin rehber olarak kullanılması gerektiğine inandığı ortaya çıkmıştır. Bu bağlamda, Türkiye’deki üniversite kütüphanelerine rehber olabileceği düşünülen bir sosyal medya politikası önerisi hazırlanmış ve bu çalışmada sunulmuştur./Our new phenomenon social media is used extensively and widely by people and organizations all over the world. This new communication and sharing platform presents new opportunities for its users as well as some threats. Therefore, the operation, management and control of social media accounts become important and the need to create a social media policy emerges.     Abstract Background: Female genital mutilation/cutting (FGM/C) is an illegal tradition commonly practiced in Sub-Saharan Af-rica and the Middle East. Despite a globalized world and developing social media, this harmful practice is currently still being implemented. Aims: We aimed to evaluate the opinions of university students regarding FGM/C.Methods: This descriptive study included 821 students who studied at Nyala University, Sudan, in January 2016. The stu-dents were questioned for the following: age, faculty, reasons for female circumcision, effects of female circumcision on female sexual functions and their views about the circumcision for their daughter. Results: The rate of FGM/C among female university students was 80.1%. Although 73% of the male students prefer to marry uncircumcised women, they also reported that FGM/C should be continued to be performed, and their future daughters should be circumcised (64.5%). Female students were against FGM/C for their future daughters (77.6%). Conclusions: This study shows that even educated individuals demand FGM/C in spite of knowing the harm. It is there-fore suggested continuing educational studies on this subject.

    Contexte : Les mutilations sexuelles féminines/l’excision constituent une tradition illégale dont la pratique est courante en Afrique subsaharienne et au Moyen-Orient. En dépit de la mondialisation et du développement des médias sociaux, cette pratique néfaste est toujours en cours de nos jours. Objectifs : Notre objectif consistait à évaluer l’opinion des étudiants de l’université concernant les mutilations sexuelles féminines/l’excision.Méthodes : La présente étude descriptive a été effectuée auprès de 821 étudiants inscrits à l’Université de Nyala (Soudan), en janvier 2016.     Abstract Objective:Fetal hydronephrosis (FH) is the most common fetal renal pathology encountered in daily obstetric practice. Urinary and serum carbohydrate antigen 19-9 (CA 19-9) concentrations are elevated in obstructive renal pathologies. Our aim was to assess maternal urinary and serum CA 19-9 concentrations in pregnancies with FH and compare results with controls.Material and Methods:Twenty pregnancies with severe FH, 20 pregnancies with mild-moderate FH, and 20 healthy singleton pregnancies were included in this descriptive, case-control study. The diagnosis and classification of FH was based on the anterioposterior diameter of fetal renal pelvis. Maternal urinary and serum CA 19-9 concentrations were measured and compared between groups.Results:Severe FH cases had significantly higher maternal urinary CA 19-9 concentrations compared to controls (median: 75 vs 24 U/mL; respectively; p=0.014). Concentrations of CA 19-9 did not differ between the mild-moderate FH group and control group. No statistically significant difference was found between the groups with respect to maternal serum CA 19-9 concentrations.Conclusion:Our results show that maternal urinary CA 19-9 concentration is significantly higher in pregnancies with severe FH. However, no difference was detected in serum CA 19-9 concentrations between pregnancies with severe FH, mild-moderate FH and controls. If the mechanisms of transplacental passage and maternal urinary excretion are clarified, maternal urinary CA 19-9 may be a potential marker for indicating fetal kidney damage.

    Abstract Metformin-associated lactic acidosis in a diabetic patient is a rare and severe complication. The severity of the condition is associated with the underlying disease. The diagnosis may be delayed due to its rarity and the fact that its symptoms and signs are frequently confused with other pathologies. In such cases, generally, the underlying renal disease helps to establish the diagnosis. In this paper, we present the case of lactic acidosis in a patient without a serious underlying disease who was using metformin, ramipril and spironolacton for diabetes and hypertension. The level of blood creatinine was found to be 1.4 mg/dL. It should be kept in mind that metformin can cause life-threatening lactic acidosis, especially in patients using renin-angiotensin-aldosterone system blockers, even without a serious deterioration in renal functions.

    Abstract We aimed to describe the presentation of long-term complications of female genital mutilation/cutting and the surgical management of clitoral keloids secondary to female genital mutilation/cutting. Twenty-seven women who underwent surgery because of clitoral keloid between May 2014 and September 2015 in Sudan Nyala Turkish Hospital were evaluated in this retrospective descriptive case series study. The prevalence of type 1, type 2, and type 3 female genital mutilation/cutting were 3.7%, 22.2%, and 74.1%, respectively (type 1: 1/27, type 2: 6/27, and type 3: 20/27). All patients had long-term health problems (dysuria, chronic pelvic pain, vaginal discharge, and chronic pruritus) and sexual dysfunction. Keloids were removed by surgical excision. There were no postoperative complications in any patient. Although clitoral keloid lesions can be seen after any type of female genital mutilation/cutting, they usually develop after type 3 female genital mutilation/cutting. Most of these keloids were noticed after menarche. Keloids can be removed by surgical excision and this procedure can alleviate some long-term morbidities of female genital mutilation/cutting.

    Abstract Cardiovascular diseases are the most common cause of mortality and morbidity in patients with chronic kidney disease (CKD) and end-stage renal disease (ESRD), receiving haemodialysis, peritoneal dialysis, and renal transplantation (Rtx). Estimated glomerular filtration rate (eGFR) places Rtx patients in one of the stages of CKD. Therefore, Rtx patients might be considered a subset of CKD patients. Besides the traditional risk factors of hypertension, diabetes, and dyslipidaemia, advanced-age novel risk factors such as endothelial dysfunction, vascular calcification, and increased chronic low-grade inflammation are highly prevalent and seem to play a more important role for vascular disease in CKD and Rtx patients compared to the general population. The role of Rtx in terms of atherogenesis and chronic ongoing low-grade inflammation is still unclear. To date, in the literature, the data are scant regarding the relationship between atherosclerosis, chronic inflammation, and cardiovascular events in Rtx patients with well-functioning kidneys. This review will discuss classical and recent epidemiological, pathophysiological, and clinical aspects of atherosclerosis and inflammation in Rtx patients.

    Abstract Presented here is a case of long-term lithium use, with multiple emerging lithium-associated side effects. An 82-year-old woman was brought into the emergency department because of loss of consciousness. According to the physical examination and laboratory analyses, patient was diagnosed with lithium-associated hypercalcemia, hyperparathyroidism, nephrogenic diabetes insipidus (NDI), symptomatic sinus bradycardia, and thyroid dysfunction. In the literature, there is a limited number of case reports with lithium induced multiple clinical conditions. Multiple clinical manifestations due to the side effects of chronic lithium use might be seen. Health care professionals should keep in mind that lithium-related side effects might trigger or exacerbate each other. To avoid toxicity, close follow-up and clinical supervision are important for the early diagnosis and treatment of these side effects, due to the narrow therapeutic index and obscure clinical signs and symptoms of toxicity.

    Abstract The prevalence of diabetes mellitus (DM) is increasing secondary to increased consumption of food and decreased physical activity worldwide. Hyperglycaemia, insulin resistance and hypertrophy of pancreatic beta cells occur in the early phase of diabetes. However, with the progression of diabetes, dysfunction and loss of beta cells occur in both types 1 and 2 DM. Programmed cell death also named apoptosis is found to be associated with diabetes, and apoptosis of beta cells might be the main mechanism of relative insulin deficiency in DM. Autophagic cell death and apoptosis are not entirely distinct programmed cell death mechanisms and share many of the regulator proteins. These processes can occur in both physiologic and pathologic conditions including DM. Besides these two important pathways, endoplasmic reticulum (ER) also acts as a cell sensor to monitor and maintain cellular homeostasis. ER stress has been found to be associated with autophagy and apoptosis. This review was aimed to describe the interactions between apoptosis, autophagy and ER stress pathways in DM.

    Abstract Primary hyperparathyroidism is a quite rare, life-threateningsituation for mother and fetus in pregnancy. Thegold Standard in radiological diagnosis is technetium-99msestamibi scintigraphy screening method. However, it iscontraindicated in pregnancy.     Abstract The occurrence of metastatic insulinoma in a patient with pre-existing diabetes is extremely rare. We report the case of a diabetic patient who had frequent hypoglycemic episodes, apparently related to metastatic insulinoma. A 47-year-old woman with type 2 diabetes mellitus who had been treated with a sulfonylurea for 10 years began experiencing frequent episodes of hypoglycemia even after complete withdrawal of the hypoglycemic agent. Endogenous hyperinsulinism was found using a prolonged fasting test. Computed tomography identified a pancreatic tumor with metastatic liver lesions and peripancreatic lymphadenopathies. Ultrasound-guided hepatic mass biopsy was performed and the pathology examination of the tumor demonstrated a neuroendocrine tumor. The patient refused surgery; octreotide (10 mg) was administered. A few days later, the patient had no further episodes of hypoglycemia. Although the coincidence of insulinoma and diabetes is extremely rare, after excluding common reasons for hypoglycemia in diabetic patients, insulinoma must be considered. Turk Jem 2012; 16: 82-4
