    Abstract This article discusses topical issues related to the assessment of the quality of service in the field of real estate transactions, in particular, the theoretical and methodological approaches to determining the quantitative value of the service quality indicator. The authors summarized the existing methods for determining the estimated value of the service quality indicator, and outlined the subjective aspects of the considered methods. The author’s vision of the methodological support for establishing the estimated values of service quality indicators in the field of real estate transactions is proposed.

    Abstract Aim. To assess the changes in endothelial dysfunction in patients undergoing cardiac surgery with minimally invasive extracorporeal circulation (MiECC). Methods. The study included 50 patients who were undergoing coronary artery bypass grafting (CABG) surgery with cardiopulmonary bypass (CPB). The patients were divided assigned to either a minimally invasive cardiopulmonary bypass system (group 1, n=15) or standard extracorporeal circuit (group 2, n=35). Changes in the laboratory parameters were assessed 5 times: before the operation, 5 minutes after protamine sulfate administration, 12 hours after the operation, 7 days after the patient's discharged from the hospital and one month after the operation. The activity of von Willebrand factor, factor VIII, and the number of activated platelets were examined in all patients in venous blood. Results. After protamine sulfate administration, the activity of von Willebrand factor was increased to 164% in the group 1, and up to 193% in the group 2, with a tendency to increase the indicator after 12 hours. The peak of endothelial dysfunction, with the growth of von Willebrand factor and factor VIII, occurs on the 7th day after the operation. In patients of the group with MiECC, von Willebrand factor activity was decreased at the hospital discharge and returned to normal in 1 month. The number of activated platelets increases mainly in group 2 (6% versus 4% in group 1, p=0.29). The expression of P-selectin was significantly higher in group 2 at the hospital discharge (5.5% versus 3.1% in group 1, p 0.001), and in 1 month (4.5% versus 2.3% in group 1, p 0.001). Conclusion. In patients with minimally invasive cardiopulmonary bypass, platelet activation decreases, endothelial dysfunction, accompanied by an increase in the von Willebrand factor and factor VIII activity, is less pronounced; the seventh day after surgery is a period of the high risk of thrombogenic complications.

    Abstract This article considers topical issues related to the conduct of comparative assessment of managing companies in the Russian Federation, in particular, it describes theoretical and methodological aspects of rating of managing companies in the sphere of provision of housing and communal services. During the study, the authors summarized and systematized existing methods of compiling ratings of management companies in Russia, identified «bottlenecks» that do not allow to obtain an objective assessment of the activities of management companies. The author’s methodology of comparative assessment of activities of managing companies in the sphere of provision of housing and communal services is proposed, based on selection and systematization of key evaluation criteria reflecting such aspects of activity as information openness of the managing organization to the population, quality of work with clients, financial condition of the managing company, additional criteria. The work describes the peculiarities of grouping of estimated parameters within each category and methods of their calculation. The results of testing of the methodology are given on the example of the management companies of Omsk

    Abstract Inflammatory bowel disease (Crohn's disease and ulcerative colitis) has a tendency to manifest at earlier age. In childhood (< 6 years of age) it has an especially severe course and is characterized by high grade inflammation, predominantly in the colon, by complication and extra-intestinal autoimmune injury. At younger age, Crohn's disease and ulcerative colitis require more aggressive treatment with frequently poor results. From genetic point of view, monogenic mutations controlling the immune response are characteristic for these diseases with an early onset; therefore, they are frequently associated with primary immunodeficiency. This implies various immunologic deficits, such as breakdown of the epithelial barrier, phagocytic dysfunction and dysfunction of Т and В lymphocytes and regulatory Т cells. Depending on this, a number of primary immunodeficiencies are identified associated with monogenic mutations of more than 50 genes. There some age-related specific features at manifestation. Thus, defects in interleukin 10 and FOXP3 manifest in the first months of life, whereas severe combined immunodeficiencies and phagocytosis defects become evident somewhat later. Virtually all 24 children with very early onset of inflammatory bowel disease, whom we examined, had immunologic defects and one child had a XIAP gene mutation. After identification of a specific immunologic defect, one can understand the mechanism of the disease and suspect one or another genetic defect with subsequent reasonable assessment of mutations in candidate genes. Detection of immunologic and genetic defects in children with a very early onset of inflammatory bowel disease allows for choosing an adequate strategy of non-conventional treatment that may differ depending on the mechanism of the disease.

    Abstract Androgen blockade has been widely used in the treatment of. prostatic cancer (PC) for more than 60 years. However, most patients develop high-grade androgen-resistant PC a few years after the initiation of hormonal therapy. This study was undertaken to explore the mechanisms of this process. Fourteen patients with Stages 3-4 PC were followed up. They all underwent orchiectomy. Comparison of the data obtained before and 1 month has shown than the decreased level of testosterone enhances mitotic activity, impairs the regulation of a cell cycle, and induces apoptosis, while its significantly lowered level causes antitumor immunity decompensation.
