    Abstract The study aims to understand the relationship of childhood diarrhoea (under-five children) with water, sanitation and hygiene factors in the light of other contextual factors in an urban setting in the district of Hooghly in West Bengal, India. This primary study was carried out by SIGMA Foundation, Kolkata from 4 to 24 January 2023 across 404 households having at least one under-five child. The findings suggested that the water score was ‘good’ in 85.1% of the households whereas the hand hygiene score was ‘good’ in 14.6% of households. More than 90% of the households had piped water supply. Less than half of them treated water before consumption among which 45.3% used cloths for straining water; 59.2% of the caregivers followed safe disposal of child's faeces; 66.8% of households had no handwashing arrangement, and 30.5% had taps and wash basins for handwashing; 20.3% of the under-five children had suffered from diarrhoea in the last month before the survey and its prevalence was higher in children aged 12–23 months. Multivariate results suggested diarrhoea prevalence was lower in households that were pucca and had good water and hand hygiene scores, lower in children that had received iron fortification and whose caregivers followed safe child's stool disposal. HIGHLIGHTS Occurrence of diarrhoea in under-five children was linked to WASH practices in the urban setting of the Hooghly district of West Bengal, India.; Although most of the households had piped water supply, proper sanitation facilities and waste segregation, their handwashing practices at child-related critical times were not adequate.;

    Abstract Considerable fundamental studies have focused on the mechanisms governing cognitive flexibility and the associated costs of switching between tasks. Task-switching costs refer to the phenomenon that reaction times and accuracy decrease briefly following the switch from one task to another. However, cognitive flexibility also impacts day-to-day life in many complex work environments where operators have to perform several different tasks. One major difference between typical tasks examined in fundamental studies and real-world applications is that fundamental studies often rely on much more similar tasks, which is not the case for real-world applications. In the latter, operators may switch between vastly dissimilar tasks. Therefore, this behavioural study aims to test if task-switching costs are different for switches between similar and dissimilar tasks. The proposed protocol has participants switch between 2 pairs of two tasks each. Between pairs, there is more dissimilarity, while the two tasks within each pair are more similar. In addition, this study examines the impact of mental fatigue and interference in form of confounding information on cognitive flexibility. To induce mental fatigue the participants' breaks between blocks will be limited. We expect that dissimilarity between tasks will result in greater task-switching costs.

    Abstract Abstract Brain-computer interfaces (BCIs) allow direct communication between one’s central nervous system and a computer without any muscle movement hence by-passing the peripheral nervous system. They can restore disabled people’s ability to interact with their environment, e.g. communication and wheelchair control. However, to this day their performance is still hindered by the non-stationarity of electroencephalography (EEG) signals, as well as their susceptibility to noise from the users’ environment and from their own physiological activity. Moreover, a non-negligible amount of users struggle to use BCI systems based on motor imagery. In this paper, a new method based on the path signature is introduced to tackle this problem by using features which are different from the usual power-based ones. The path signature is a series of iterated integrals computed from a multidimensional path. It is invariant under translation and time reparametrization, which makes it a robust feature for multichannel EEG time series. The performance can be further boosted by combining the path signature with the gold standard Riemannian classifier in the BCI field exploiting the geometric structure of symmetric positive definite (SPD) matrices. The results obtained on publicly available datasets show that the signature method is more robust to inter-user variability than classical ones, especially on noisy and low-quality data. Hence, this study paves the way towards the use of mathematical tools that until now have been neglected, in order to tackle the EEG-based BCI variability issue. It also sheds light on the lead-lag relationship captured by path signature which seems relevant to assess the underlying neural mechanisms.

    Abstract Objectives: The goal of this quality improvement project (QIP) was to increase awareness of the serious medical consequences of clozapine-associated constipation to front line nursing staff and patients with schizophrenia.
    Methods: The QIP was developed iteratively by psychiatric nurses, psychiatrists and pharmacists with input from patients. The processes involved a literature review, development of educational materials for staff and patients, and the creation of a daily bowel movements log (BML). Implementation involved review of the BML at treatment team meetings, and deployment of pharmacological and non-pharmacological interventions to resolve constipation and increase awareness and knowledge of this clinical concern.
    Outcomes: The initial pilot screened for symptoms of constipation in patients receiving clozapine and non-clozapine antipsychotic agents and intervening as necessary during multidisciplinary team meetings. Patients benefited from relief of constipation and improved bowel habits. Staff benefited from improved knowledge and making requisite changes in workflow and practice. Feedback allowed refinements to be made to the educational materials for patients and staff. Since full implementation, bowel habits are routinely monitored, and interventions are reviewed for effectiveness. Staff satisfaction with this QIP is reflected in answers to a structured questionnaire and in patient reports (n = 50).
    Conclusions: Clozapine, the only approved and efficacious medication for treatment-resistant schizophrenia is significantly underutilized. Medically consequential constipation can be a serious barrier to retention of patients benefiting from clozapine. Increased awareness and use of educational materials for patients and staff, routine monitoring of bowel habits combined with pharmacological and non-pharmacological interventions can successfully address this clinical problem.

    Abstract While clozapine is the most effective antipsychotic treatment for treatment-resistant schizophrenia, it remains underutilized across the United States, warranting a more comprehensive understanding of variation in use at the county level, as well as characterization of existing prescribing patterns. Here, we examined both Medicaid and Medicare databases to (1) characterize temporal and geographic variation in clozapine prescribing and, (2) identify patient-level characteristics associated with clozapine use. We included Medicaid and Fee for Service Medicare data in the state of Pennsylvania from January 1, 2013, through December 31, 2019. We focused on individuals with continuous enrollment, schizophrenia diagnosis, and multiple antipsychotic trials. Geographic variation was examined across counties of Pennsylvania. Regression models were constructed to determine demographic and clinical characteristics associated with clozapine use. Out of 8,255 individuals who may benefit from clozapine, 642 received treatment. We observed high medication burden, overall, including multiple antipsychotic trials. We also identified variation in clozapine use across regions in Pennsylvania with a disproportionate number of prescribers in urban areas and several counties with no identified clozapine prescribers. Finally, demographic, and clinical determinants of clozapine use were observed including less use in people identified as non-Hispanic Black, Hispanic, or with a substance use disorder. In addition, greater medical comorbidity was associated with increased clozapine use. Our work leveraged both Medicaid and Medicare data to characterize and surveil clozapine prescribing. Our findings support efforts monitor disparities and opportunities for the optimization of clozapine within municipalities to enhance clinical outcomes.

    Abstract Safety studies have identified attention as a recurring cause of incidents and accidents in air traffic control. However, little is known of the precise attentional states that lead to degraded ATC performance. Therefore, we surveyed 150 French en route air traffic controllers on the causes of and impacts on perceived cooperation, safety, and performance of seven degraded attentional states from the literature: task-related and task-unrelated mind wandering, mental overload, inattentional deafness and blindness, attentional entropy, and perseveration. Our findings indicated that task-related and task-unrelated mind wandering were the most prevalent but had the least impact on perceived safety. Conversely, inattentional blindness and attentional entropy were less reported but were considered a significant safety concern, while inattentional deafness affected cooperation. Most states were experienced in workload levels consistent with the literature. However, no other factor such as shift work was identified as a cause of these states. Overall, these findings suggest that “attention” is not a specific enough subject for ATC, as attentional issues can occur in various conditions and have different impacts. As far as safety is concerned, inattentional blindness should be the prime target for further research. Neuroergonomics in particular could help develop dynamic countermeasures to mitigate its impact.

    Abstract Abstract Brain-Computer Interfaces and especially passive Brain-Computer interfaces (pBCI), with their ability to estimate and monitor user mental states, are receiving increasing attention from both the fundamental research and the applied research and development communities. Testing new pipelines and benchmarking classifiers and feature extraction algorithms is central to further research within this domain. Unfortunately, data sharing in pBCI research is still scarce. The COG-BCI database encompasses the recordings of 29 participants over 3 separate sessions with 4 different tasks (MATB, N-Back, PVT, Flanker) designed to elicit different mental states, for a total of over 100 hours of open EEG data. This dataset was validated on a subjective, behavioral and physiological level, to ensure its usefulness to the pBCI community. Furthermore, a proof of concept is given with an example of mental workload estimation pipeline and results, to ensure that the data can be used for the design and evaluation of pBCI pipelines. This body of work presents a large effort to promote the use of pBCIs in an open science framework.
