    Abstract Reversible cerebral vasoconstriction syndromes (RCVS) show reversible multifocal narrowing of the cerebral arteries with clinical manifestations that characteristically include thunderclap headache and less commonly focal neurologic deficits related to brain edema, stroke, or seizure. RCVS predominantly affects women (female to male ratio 2:1 to 10:1, depending on the case series). The mean age across published studies is 42 to 44 years. RCVS has been associated with a multiplicity of conditions including pregnancy, migraine ,use of vasoconstrictive drugs and other medications, neurosurgical procedures, hypercalcemia, unruptured saccular aneurysms, cervical artery dissection, cerebral venous thrombosis, and others. Between 30 and 70 percent of patients with RCVS have no abnormality on initial brain scans, despite having widespread cerebral vasoconstriction. However, approximately 75 percent of admitted patients eventually develop parenchymal lesions. The most frequent lesions are ischemic stroke and cortical surface (nonaneurysmal or convexal) subarachnoid hemorrhage, followed by reversible vasogenic brain edema and parenchymal hemorrhage. Infarcts are often bilateral and symmetrical, located in arterial "watershed" regions of the cerebral hemispheres or in the cortical-subcortical junction. Larger infarcts are often wedge-shaped. Cerebral angiographic abnormalities are dynamic and progress proximally, resulting in a "sausage on a string" appearance of the circle of Willis arteries and their branches. There is no proven therapy for RCVS. Supportive care is directed towards managing blood pressure, severe headaches, and other complications such as seizures. Oral calcium channel blockers are often administered to treat vasoconstriction but the supporting evidence for this strategy is weak.

    Abstract Background: The most common symptom of the novel Coronavirus Disease 2019 (COVID-19) infection is fever and dyspnea that leads to hypoxia in severe cases. Some COVID-19 patients experience neurological symptoms, including ischemic stroke and intracerebral hemorrhage. Sickle Cell Disease (SCD) is a hypercoagulable state, however, it has not been approved as a significant cause of Cerebral Venous Thrombosis (CVT). Case presentation: In this case report, we described CVT in an SCD patient who had COVID-19, as well. We reported a 32-year-old man with a history of sickle cell anemia presented with left hemiparesis, headache, and seizure. After evaluation of the patient, CVT accompanied by COVID-19 infection was diagnosed. He was treated with intravenous unsaturated heparin, antiepileptic drugs, and antiviral agents with a favorable outcome. Based on our knowledge, this is the first case study to describe an association between CVT and COVID-19 infection in a patient with SCD. Conclusion: During the recent pandemic, vaso-occlusive attacks in SCD patients can be evaluated for COVID-19 as an etiological factor.

    Abstract Abstract Lead is a heavy metal that affects many organs such as nervous system, liver, and kidney. The most important affected organ is central nervous system. The present study reported a case similar to Amyotrophic lateral sclerosis (ALS) due to lead exposure in an opium addicted person. The patient complaint was the weakness of upper and lower limbs in addition to the unsteadiness of gait. Nerve conduction studies showed normal sensory and motor action potential but needle electromyography showed fasciculation in distal and proximal muscles. The patient was encephalopathic and had psychiatric disorder. The blood lead level was also found to be higher than the normal upper limit. Hence, lead poisoning could occur in a patient addicted to opium with symptoms similar to ALS.

    Abstract Background. Knowledge and attitudes are required for relatives of people with epilepsy to allow them to better understand and cope with this condition. This study evaluated the knowledge and attitudes of family members of people with epilepsy about the disease. Methods. This cross-sectional survey was conducted using a self-administered questionnaire completed by close family members of people with epilepsy at the outpatient clinic of a medical university. The questionnaire included 25 items that determined the demographics and information on the level of knowledge and attitudes about epilepsy. Results. The 124 participants had an average age of 36.88±10.68 years. The mean knowledge score was 10.32±2.25 (range: 4 to 15). 87.1% of respondents answered that epilepsy is a brain disorder, 39 (31.5%) said epilepsy is inherited. As a whole, 62 (50%) had good knowledge about the disease. The mean score of attitude was 7.25±1.54 (range: 2 to 10). 83.9% of respondents believed that a person with epilepsy can get married and get pregnant (76.6%). Overall, 15 (12.1%) had negative attitudes and 109 (87.9%) had positive attitudes. Conclusion. The main findings of this study indicated good knowledge and a positive attitude about epilepsy among family members of people with epilepsy.

    Abstract Background: Epilepsy comprised the highest proportion of neurological problem of childhood stage, which observed mostly in the first decade of life. The dramatic effect of having a seizure in the classroom can be very traumatic for any child. The knowledge and attitude of teachers toward epilepsy have a direct impact on the life of students with epilepsy. Methods: A cross-sectional descriptive survey was conducted in Kermanshah (West of Iran). 305 teachers from 25 public schools were randomly participated in this study. The questionnaire included 39 items and three sections (demographic information, knowledge, and attitude about epilepsy). Results: In this study, 97% participants had heard or read about epilepsy. Attitude and knowledge about epilepsy was positive in weighted sum of the item responses, but there were deficits in individual items and first-aid management of seizure attacks. There was no meaningful relationship between attitude scores and demographic items, but higher level of education, female gender, and marital status had a positive influence on teachers’ knowledge toward children with epilepsy. Conclusion: The main findings indicated a good knowledge and positive attitude about epilepsy among school’s teachers. Nevertheless, there is still a need to improve certain aspects of knowledge and attitude and first aid management of an epileptic attack among teachers.

    Abstract Tumefactive demyelinating lesions are a rare presentation of multiple sclerosis (MS). Diagnosis of tumefactive is commonly carried out using magnetic resonance image (MRI). Tumefactive diagnosis is difficult because of may similar to the clinical and MRI characteristics of glioma or a cerebral abscess. We presented a 35-years-old female with one episode of secondary generalized seizure after delivery. In MRI with contrast, two gadolinium-enhanced lesions were observed in right temporal lobe with open-ring appearance.

    Abstract Background: Restaurant sector is one of the most rapidly developing sectors in the world and there is evidence that restaurant industry has high levels of work-related diseases and injuries. This study examined the prevalence of self-reported work-related injuries and illness (WRIIs) and their association with demographic variables among restaurant workers in Shiraz, the capital of Fars Province, Iran. Methods: In this cross-sectional study, 300 randomly selected restaurant workers completed a self-statement, research-made questionnaire regarding the prevalence of self-reported WRIIs, in Shiraz, Iran. Statistical analyses were performed using SPSS version 20. Results: A high prevalence of work-related injuries (84%) and musculoskeletal disorders (70%) was reported among restaurant workers. Cuts and lacerations, arising from accidents with knives, were the most common injuries seen, followed by burns, falls, slips and trips. Moreover, the prevalence of occupational accidents had a significant association with work experience (p = 0.012), cooking (p < 0.001), as well as preparation and washing (p = 0.011). Age (p < 0.001) and work experience (p < 0.001) had a significance association with the prevalence of musculoskeletal disorders. Conclusion: Preventive measures and polices, through providing occupational health and safety services including trainings, personal protective equipment and health examinations, should be taken as to restaurants and catering industry in Iran.

    Abstract Multiple sclerosis (MS) is a chronic inflammatory demyelinating disease of the central nervous system that inflammation, demyelination, oligodendrocyte loss, gliosis, axonal injury and neurodegeneration are the main histopathological hallmarks of the disease. Although MS was classically thought as a demyelinating disease, but axonal injury occurs commonly in acute inflammatory lesions. In MS microglial activation is not only responsible for inflammatory cascade but also creates degenerative cascade. The evidence has shown mitochondrial dysfunction plays an important role in axonal degeneration in all stages of MS due to neuronal cell loss and activation pro-inflammatory cytokines. Neuronal loss occurs as a result of apoptosis and necrosis and mitochondrial pathway is the main important system for apoptosis and this way was involved in neurodegenerative disorders such as MS. Hence in multiple sclerosis, mitochondrial dysfunction causes energy failure and then increases inflammation and demyelination in neurons.

    Abstract Introduction : Epilepsy is a chronic neurological disorder in which patients experience spontaneous recurrent seizures and deficiency in learning and memory. Although the most commonly recommended therapy is drug treatment, some patients do not achieve adequate control of their seizures on existing drugs. New medications with novel mechanisms of action are needed to help those patients whose seizures are resistant to currently-available drugs. While alpha-lipoic acid as a antioxidant has some neuroprotective properties, but this action has not been investigated in models of epilepsy. Therefore, the protective effect of pretreatment with alpha-lipoic acid was evaluated in experimental model of temporal lobe epilepsy in male rats. Methods: In the present study, Wistar male rats were injected intrahippocampally with 0.9% saline(Sham-operated group), kainic acid(4 μg) alone, or α-lipoic acid (25mg and 50mg/kg) in association with kainic acid(4μg). We performed behavior monitoring(spontaneous seizure, learning and memory by Y-maze and passive avoidance test), intracranial electroencepholography (iEEG) recording, histological analysis, to evaluate the anti- epilepsy effect of α-lipoic acid in kainate-induced epileptic rats. Results: Behavior data showed that the kainate rats exhibit spontaneous seizures, lower spontaneous alternation score inY-maze tasks (p<0.01), impaired retention and recall capability in the passive avoidance test (p<0.05). Administration of alpha-lipoic acid, in both doses, significantly decrease the number of spontaneous seizures, improved alternation score in Y-maze task (p<0.005) and impaired retention and recall capability in the passive avoidance test (p<0.01) in kainite rats. Moreover, lipoic acid could improve the lipid peroxidation and nitrite level and superoxid dismutase activity.Conclusion: This study indicates that lipoic acid pretreatment attenuates kainic acid-induced impairment of short-term spatial memory in rats probably due to its antioxidant activity.
