    Abstract Abstract Background Polypharmacy (PP) is increasingly common in Iran, and contributes to the substantial burden of drug-related morbidity, increasing the potential for drug interactions and potentially inappropriate medications. Machine learning algorithms (ML) can be employed as an alternative solution for the prediction of PP. Therefore, our study aimed to compare several ML algorithms to predict the PP using the health insurance claims data and choose the best-performing algorithm as a predictive tool for decision-making. Methods This population-based cross-sectional study was performed between April 2021 and March 2022. After feature selection, information about 550 thousand patients were obtained from National Center for Health Insurance Research (NCHIR). Afterwards, several ML algorithms were trained to predict PP. Finally, to assess the models’ performance, the metrics derived from the confusion matrix were calculated. Results The study sample comprised 554 133 adults with a median (IQR) age of 51 years (40 – 62) that nested in 27 cities within the Khuzestan province of Iran. Most of the patients were female (62.5%), married (63.5%), and employed (83.2%) during the last year. The prevalence of PP in all populations was about 36.0%. After performing the feature selection, out of 23 features, the number of prescriptions, Insurance coverage for prescription drugs, and hypertension were found as the top three predictors. Experimental results showed that Random Forest (RF) performed better than other ML algorithms with recall, specificity, accuracy, precision and F1-score of 63.92%, 89.92%, 79.99%, 63.92% and 63.92% respectively. Conclusion It was found that ML provides a reasonable level of accuracy in predicting polypharmacy. Therefore, the prediction models based on ML, especially the RF algorithm, performed better than other methods for predicting PP in Iranian people in terms of the performance criteria.

    Abstract Background and aims: Patients with chronic illnesses such as stroke have different physical and mental problems and need the care and support of their family members. Family caregivers face many different problems and experience burnout during caregiving to their patients. The aim of this study was to investigate the effects of care-oriented group discussion (GD) on burnout among the caregivers of patients with stroke. Methods: This two-group pretest-posttest quasi-experimental study was conducted in 2019. Participants were forty family caregivers of patients with stroke randomly selected from the neurology and stroke care ward of Sina hospital, Hamadan, Iran. They were randomly allocated to two twenty-person groups. Participants in the control group received routine care services, while participants in the intervention group received routine care services in addition to care-oriented GD in six two-hour sessions. Burnout was assessed before and two weeks after the study intervention and the SPSS program (v. 22.0) was used to analyze the data through the Kolmogorov-Smirnov, paired-sample t, independent-sample t, and Chi-square tests. Results: Groups did not significantly differ from each other respecting the pretest mean scores of burnout and all its dimensions as well as the posttest mean scores of the personal and financial dimensions of burnout (P > 0.05). However, the posttest mean scores of burnout and its social and emotional dimensions in the intervention group were significantly less than the control group (P < 0.05). Conclusion: As an effective strategy, care-oriented GD can be used to reduce burnout among the family caregivers of patients with stroke.

    Abstract Abstract Semi-continuous data characterized by an excessive proportion of zeros and right-skewed continuous positive values appear frequently in medical research. One example would be the pharmaceutical expenditure (PE) data for which a substantial proportion of subjects investigated may report zero. Two-part mixed-effects models have been developed to analyse clustered measures of semi-continuous data from multilevel studies. In this study, we propose a new flexible two-part mixed-effects model with skew distributions for nested semi-continuous cost data under the framework of a Bayesian approach. The proposed model specification consists of two mixed-effects models linked by the correlated random effects: Part I) a model on the occurrence of positive values using a generalized logistic mixed model; and Part II) a model on the magnitude of positive values using a linear mixed model where the model errors follow skew distributions including beta-prime (BP). The proposed method is illustrated with pharmaceutical expenditure data from a multilevel observational study and the analytic results are reported by comparing potential models under different skew distributions. Simulation studies are conducted to assess the performance of the proposed model. The DIC3, LPML, WAIC, and LOO as the Bayesian model selection criteria and measures of divergence used to compare the models.

    Abstract Introduction: Researchers and faculty members are faced with the achievement of academic authority in higher education and fulfilling the needs seems indispensable. In this study, attitudes of the faculty members' Hamadan University of Medical Sciences (UMSHA) towards the factors of academic authority in a researcher-made questionnaire was solicited. The purpose of this study was to develop and evaluate the psychometric properties of the academic authority questionnaire among faculty members. Methods: This cross-sectional analytical study was conducted on 205 faculty members of UMSHA in 2018 using multivariable factor analysis. The instrumentation was a 17-item questionnaire of academic authority. All the analyses were at value of p<0.05 was considered statistically significant. Results: After completing the questionnaire by the faculty members, factor analysis was conducted to evaluate the effective components of the authority. It is of note that 125 (61%) units were male and 170 (82.8%) had a PhD degree. The mean age of the samples was 43.7 with a standard deviation of ±8.1. The results of validity and reliability test confirmed the content validity and showed acceptable level of internal correlation of questionnaire (CVI≥0.78, Cronbach's alpha=0.948). The analysis of the major components with varimax rotation, according to the KMO and the eigenvalues, three factors were extracted which accounted for 71.16% of the total variance. Conclusion: Findings showed three important components. Along these lines, according to views of faculty members, to achieve academic authority, the ability to develop sciences, the meager relationship between theory and practice in the professional fields are the major challenges in higher education.

    Abstract Abstract Objective The present work aimed at determining the prevalence of menopausal symptoms and factors associated with the Quality of life among postmenopausal women. Materials and methods This cross-sectional work was carried out on 270 postmenopausal females referring to health centers of Hamadan city chosen by stratified random sampling. A questionnaire of the Menopausal Quality of Life Questionnaire (MENQOL) was used as the data collection method. Although, the Mann–Whitney test and the Kruskal–Wallis tests were used to compare MENQOL item scores. The significance level of statistical tests was regarded as less than 0.05. Results The mean MENQOL Score in menopausal was 2.45 ± 1.04. Also, vasomotor symptoms had the highest score, and sexual symptoms had the lowest score rather than other dimensions. There was a significant association between the total menopausal quality of life score and job, economy status, smoking, exercise, supplemental Omega-3 s intake, and Postmenopausal stage (p < 0.01). As smokers, women had increasing levels of bother experienced from the MENQOL symptom than non-smokers (mean = 3.67 ± 0.85 vs. 2.36 ± 0.99; p < 0.001). While the MENQOL scores for menopausal females who exercised more often (mean = 1.56 ± 0.7) had lower than those who exercised less than 3 times per week (mean = 3.27 ± 0.9; p < 0.001). However the lowest score was menopausal females who had taking supplemental Omega-3 s than those who hadn't taken it (mean = 2.15 ± 1.06 vs. 2.65 ± 0.97; p < 0.001). Though women who had postmenopausal stage less than 5 years stage (mean = 2.28 ± 0.87) had significantly lower MENQOL scores from those who had postmenopausal stage 5 or more years (mean = 2.63 ± 1.16; p < 0.001). Conclusion Based on the results, vasomotor symptoms were the most dominant symptom. Therefore, it is necessary to improve physical activity levels, focusing on job status, recommend taking an omega 3 s supplement, and planning education and promotion intervention for cessation or prevention of ...

    Abstract Background and Objective: The relationship between parent health literacy and adherence to rehabilitation in children with special needs has not been fully explored. The aim of this study was to determine the association of parent health literacy and other predicted factors with follow-up of occupational therapy (OT) and speech therapy (ST) in children with special needs between 3 and 6 years old, Hamadan city, 2020.Materials and Methods: In this descriptive-analytical cross-sectional study, 92 parents (64 mothers and 28 fathers), who have children refering to occupational therapy and speech therapy centers, were randomly selected. Data gathering was conducted by demographic and parent health literacy questionnaires, and SPSS software version 16 all data was used to analyze data by independent t-test, ANOVA, Pearson correlation coefficient at a significance level of 0.05. Results: The mean of father and mother’s health literacy were 55.18±7.59 and 61.72 ± 13.56, respectively. A significant difference was observed in parents' health literacy, parent's gender (p = 0.019), parent’s education level (p = 0.05), and father’s occupation. Pearson’s correlation coefficient analyses showed a significant negative correlation between parental health literacy and father age (r = -.345, p < .01), maternal age (r =-.418, p < .01), and parental health literacy positively correlated with the number of OT (r = .238, p < .05) and ST Sessions (r = .468, p < .01). Conclusion: parent's health literacy was inadequate, and level of health literacy was higher in younger and more educated mothers and significantly associated with further follow-up of rehabilitation (occupational therapy and speech therapy). It is necessary to plan educational strategies to increase the level of health literacy in parents who have children with special needs, because of these children are vulnerable population.
