    Abstract Objective: To review the materials and fabrications methods of occlusal splints with their advantages and shortcomings and to clarify the indications for each.
    Background: Temporomandibular joint disorders (TMDs) encompass a range of conditions affecting the masticatory system. Occlusal splints are currently recognised as a viable option for the management of TMDs when they are adopted in conjunction with additional approaches varying from conservative procedures (counselling, biofeedback, physical therapy, pharmacotherapy) to irreversible and less conservative ones (occlusal adjustments, orthodontics, arthroscopy and surgery). These splints can vary in design, function and material. The components used to fabricate the splints must withstand occlusal forces, be aesthetically pleasing, comfortable and minimally interfering with function and phonetics. Traditional methods for fabricating splints include sprinkle-on, thermoforming and lost wax techniques. However, with the advancement of CAD/CAM technology, additive (3D printing) and subtractive (milling) manufacturing methods expand the range of possibilities by introducing novel solutions for elaborating splints.
    Methods: An electronic search was conducted on PubMed using the following keywords: "occlusal splint", "guard and "materials", and "manufacturing." Thirteen in vitro publications were screened and they consisted of four clinical studies, nine reviews (three of them were systematic reviews), and five case reports.
    Conclusion: The choice of material is crucial for the success of splint therapy. Factors such as biocompatibility, ease of fabrication, adjustability, cost and patient preference should be considered. Newer materials and techniques are emerging because of advancements in material science and manufacturing techniques. However, it should be noted that most of the evidence is based on in vitro studies with different methodologies, limiting their validity in daily practice.

    Abstract Documentation enables sharing knowledge between the developers of a technology and its users. Creating quality documents, however, is challenging: Documents must satisfy the needs of a large audience without being overwhelming for individuals. We address this challenge with a new document format, named Casdoc. Casdoc documents are interactive resources centered around code examples for programmers. Explanations of the code elements are presented as annotations that the readers reveal based on their needs. We evaluated Casdoc in a field study with over 300 participants who used 126 documents as part of a software design course. The majority of participants adopted Casdoc instead of a baseline format during the study. We observed that interactive documents can contain more information than static documents without being distracting to readers. We also gathered insights into five aspects of Casdoc that can be applied to other formats, and propose five guidelines to improve navigability in online documents.

    Abstract Objectives: Osteoarthritis (OA) is the most widespread joint disease and is a major cause of joint pain and disability in the middle aged and elderly population. The diagnosis of OA is based on clinical and radiographic changes that occur late after disease progression, and hence does not allow early detection of structural damage. Therefore, there is an acute need for reliable biochemical markers that can facilitate its earlier diagnosis. This study was commenced to identify fibulin-3 levels in serum and urine of patients with primary knee OA and to investigate their relationship with severity of the disease.
    Material and methods: Fifty female patients with primary knee OA were compared to 25 healthy female controls. Fibulin-3 in serum and urine were measured using enzyme-linked immunosorbent assay (ELISA). Severity of knee pain was assessed by the Western Ontario and McMaster Universities Osteoarthritis (WOMAC) index and the Knee Outcome Survey Activities of Daily Living (KOS-ADL) scale. Radiographic grading of knee OA was performed by Kellgren-Lawrence (K-L) criteria.
    Results: Serum and urine fibulin-3 were higher in patients (286.00 ±145.00 ng/ml and 104.60 ±32.73 ng/ml, respectively) than in controls (26.00 ±5.77 ng/ml and 13.60 ±4.21 ng/ml, respectively). Fibulin-3 in serum and urine correlated (
    Conclusions: Fibulin-3 serves as a biomarker of knee OA disease severity and could predict disease progression. Assessing urine fibulin-3 could be an applicable and easy method to diagnose knee OA and to follow up disease progression.

    Abstract Heart failure is one of the most common comorbidities of diabetes mellitus. Glucose-lowering therapies that can prevent heart failure or improve outcomes in patients with established heart failure are of critical importance among those with type 2 diabetes. Several types of glucose-lowering drugs have been assessed in this setting. Metformin has been shown to modestly improve the outcomes of patients with heart failure, whereas the effect of insulin in those with established heart failure is less clear. The effect of sulfonylureas on improving heart failure is controversial; observational reports have suggested that they are harmful in these patients, but these data have not been confirmed in randomized, controlled trials. Thiazolidinediones are contraindicated in patients with established heart failure and have also been known to cause heart failure. Furthermore, certain dipeptidyl peptidase 4 inhibitors seem to increase heart failure hospitalization. The effects of glucagon-like peptide 1 receptor agonists might differ in patients with or without established heart failure, particularly those with decompensated heart failure with a reduced ejection fraction. However, perhaps the most important finding has been that sodium/glucose cotransporter 2 (SGLT2; also known as SLC5A2) inhibitors reduce heart failure hospitalizations and, in the case of empagliflozin, markedly reduce the rate of cardiovascular death. Given the known neutral (or even harmful) effects of other glucose-lowering drugs on heart failure outcomes, SGLT2 inhibitors might well be considered the drug class of choice in patients with diabetes and heart failure, or in those at high risk of developing heart failure.

    Abstract Introduction: Abdominal wall invasion by cancerous cells arising from the colon with an overlying secondary infection that presents as an abdominal wall abscess has been encountered previously, but such a symptom is rarely the first presentation of colon cancer. There are very few cases reported in the literature.
    Case presentation: In this case report, we present a case of a 66-year-old male presenting with abdominal wall abscess that was refractory to treatment. The patient later was found to have an abdominal wall invasion by an underlying colonic carcinoma.
    Conclusion: The purpose of this review is to set forth the proper approach when encountering such cases and emphasize on the significance of keeping a high index of suspicion. We also highlight the need for utilizing proper diagnostic imaging modalities prior to invasive intervention.

    Abstract Parkinson's disease (PD) is caused by the degeneration of dopaminergic neurons due to an accumulation of intraneuronal abnormal alpha-synuclein (α-syn) protein aggregates. It has been reported that the levels of exosomal α-syn of neuronal origin in plasma correlate significantly with motor dysfunction, highlighting the exosomes containing α-syn as a potential biomarker of PD. In addition, it has been found that the selective autophagy-lysosomal pathway (ALP) contributes to the secretion of misfolded proteins involved in neurodegenerative diseases. In this review, we describe the evidence that supports the relationship between the ALP and α-syn exosomal secretion on the PD progression and its implications in the diagnosis and progression of this pathology.

    Abstract Maternal morbidities and mortalities remain high globally, yet are preventable events. Maternal haemorrhage is a primary cause of both maternal morbidity and mortality. In this collaborative project, multipronged evidence-based interventions, inclusive of embedded morbidity surveillance trigger tools were implemented to increase maternal morbidity reporting and improve the safety culture, while structured morbidity and mortality reviews aided in the reduction maternal mortality.

    Abstract Background: There has been growing Interest in patient-centered clinical trials using mobile technologies to reduce the need for in-person visits. The CHIEF-HF (Canagliflozin Impact on Health Status, Quality of Life and Functional Status in Heart Failure) trial was designed as a double-blind, randomized, fully decentralized clinical trial (DCT) that identified, consented, treated, and followed participants without any in-person visits. Patient-reported questionnaires were the primary outcome, which were collected by a mobile application. To inform future DCTs, we sought to describe the strategies used in successful trial recruitment.
    Methods: This article describes the operational structure and novel strategies employed in a completely DCT by summarizing the recruitment, enrollment, engagement, retention, and follow-up processes used in the execution of the trial at 18 centers.
    Results: A total of 18 sites contacted 130,832 potential participants, of which 2572 (2.0%) opened a hyperlink to the study website, completed a brief survey, and agreed to be contacted for potential inclusion. Of these, 1333 were eligible, and 658 consented; there were 182 screen failures, due primarily to baseline Kansas City Cardiomyopathy Questionnaire scores' not meeting inclusion criteria, resulting in 476 participants' being enrolled (18.5%). There was significant site-level variation in the number of patients invited (median = 2976; range 73-46,920) and in those agreeing to be contacted (median = 2.4%; range 0.05%-16.4%). At the site with the highest enrollment, patients contacted by electronic medical record portal messaging were more likely to opt into the study successfully than those contacted by e-mail alone (7.8% vs 4.4%).
    Conclusions: CHIEF-HF used a novel design and operational structure to test the efficacy of a therapeutic treatment, but marked variability across sites and strategies for recruiting participants was observed. This approach may be advantageous for clinical research across a broader range of therapeutic areas, but further optimization of recruitment efforts is warranted.
